この試験の目的は、特定の種類のがんを患う参加者の治療のために、抗体 GEN1055 を単独で使用した場合と、別の抗体抗がん剤であるペムブロリズマブ(化学療法の有無にかかわらず)と併用した場合について学ぶことです。 参加者はGEN1055単独、GEN1055とペムブロリズマブ、またはGEN1055とペムブロリズマブおよび化学療法のいずれかを受けることになる。 すべての参加者は活性薬剤を受け取ります。誰もプラセボを受け取ることはありません。
このトライアルは 2 つのパートで構成されています。 最初のパートの目的は、GEN1055 が安全かどうかを確認し、単独で使用する場合とペムブロリズマブと併用する場合の GEN1055 の用量を確認することです。 第 2 部の目的は、より多くの参加者に GEN1055 を投与して、第 1 部で選択した GEN1055 の用量ががん単独に対してどの程度効果があるのか、またペムブロリズマブと (他の化学療法の有無にかかわらず) とどのように効果があるのかを確認することです。

これは多施設共同試験であり、用量漸増(フェーズ 1a/1b)と用量拡大(フェーズ 2a)の 2 つの部分で実施されます。
この試験の用量漸増部分では、用量制限毒性(DLT)を評価して、局所進行性または転移性固形腫瘍を有する参加者における推奨第2相用量(RP2D)と、到達した場合の最大耐用量(MTD)を決定します。
拡張部分では、選択した腫瘍適応症における選択した用量とスケジュールの安全性、忍容性、作用機序 (MoA)、免疫原性、薬物動態 (PK)、および初期抗腫瘍活性を評価します。

包含基準:
すべてのコホート:
- 18 歳以上であること。
- RECIST v1.1に従って測定可能な疾患を患っている。
- 可能な場合は、前回の治療失敗(つまり、X線撮影で証明された進行性疾患[PD])以降のすべてのベースライン前のスキャンを提供します。
- スクリーニング時およびC1D1前治療時に東部協力腫瘍学グループのパフォーマンスステータス(ECOG PS)スコアが0〜1である。
- 生検(すなわち、ホルマリン固定パラフィン包埋スライド/ブロック)を提供します。 医学的に不可能でない限り、治験依頼者の審査と承認を経ていない限り、スクリーニング期間中に採取された新鮮な生検が望ましい。 これが提供できない場合は、前回の治療が失敗または中止された後に採取され、C1D1 の前 6 か月以内に採取された生検が提供される場合があります。
フェーズ 1a および 1b - 用量漸増:
- 組織学的または細胞学的に確認された非中枢神経系(CNS)原発性固形腫瘍で、転移性または進行性疾患を有する。
- 腫瘍の種類に該当する場合、プラチナベースの化学療法および抗PD/PD-L1療法を含む標準治療(SoC)療法を進めている、または臨床上の利益をもたらす可能性のある利用可能な標準療法が存在しない患者、および研究者の意見では、この患者にはGEN1055またはGEN1055+ペムブロリズマブによる実験的治療が有益である可能性がある。
フェーズ 2a - 拡張:
選択された腫瘍適応症に特有の包含基準が適用される場合があります。
除外基準:
以下を含むがこれらに限定されない、制御不能な併発疾患がある:
- 初回投与前2週間以内に抗感染症療法によるIV治療を必要とする進行中または活動性の感染症(コロナウイルス病2019 [COVID-19]感染を含む)。
以下を含む重大な心血管障害:
i) 症候性うっ血性心不全(ニューヨーク心臓協会により分類されたクラス III または IV)、不安定狭心症、または心臓不整脈。
ii) 最適な医学的管理にもかかわらず、収縮期血圧 ≥ 160 ミリメートル (mmHg) および/または拡張期血圧 ≥ 100 mm Hg として定義される制御されていない高血圧。
iii) フリデリシアの QT 補正式を使用した、ベースラインでの補正された QT 間隔が 470 ミリ秒以上の延長。
- 全身免疫抑制治療による治療を必要とする重大な自己免疫疾患の進行中または最近(スクリーニングから1年以内)の証拠があり、免疫関連有害事象(irAE)のリスクを示唆する可能性がある。
- -チェックポイント阻害剤(CPI)の治療中止につながったグレード3以上のirAEの病歴。 中止に至ったグレード 3 未満の irAE を有する参加者については、スポンサーと話し合う必要があります。 完全に回復したグレード 3 の irAE についても議論される場合があります。
- -慢性肝疾患(例、アルコール性肝炎または非アルコール性脂肪性肝炎)の病歴、薬物関連肝炎または自己免疫性肝炎、または肝硬変の証拠。
- 間質性肺疾患の証拠。
進行中の肺炎(グレードは問わない)。ベースラインでの進行中の肺炎の放射線学的変化、またはステロイドを必要とした非感染性の薬物、免疫、または放射線関連の肺炎の病歴を含む。
- 指定された期間内に以下の以前の治療/治療のいずれかを受けたことがあります。
- 治験治療の投与前に4週間以内の抗がん剤による治療、または薬物の5半減期以内の全身療法のいずれか短い方。
- 最初の治療から14日以内にコルチコステロイド(プレドニゾン1日当たり10 mg以上相当)または他の免疫抑制薬による全身治療を必要とする症状。 活動性の自己免疫疾患がない場合には、吸入または局所ステロイド、および副腎または下垂体置換ステロイドのプレドニゾン相当量を毎日 10 mg 以上使用することが許可されます。
- -最初の治験治療の投与前の2週間以内に顆粒球または顆粒球/マクロファージコロニー刺激因子のサポートを受けているか、または慢性輸血依存性がある。
治験治療の計画された最初の投与前14日以内にRT。 C1D1 の 7 日前までの骨転移の緩和的 RT は許可されます。
- 肝炎(地方保健当局の指示がない限り、B型肝炎またはC型肝炎の検査は必要ありません):
B 型肝炎ウイルス (HBV): HBV の病歴または血清学的検査が陽性である (B 型肝炎表面抗原または HBV デオキシリボ核酸 [DNA] が陽性と定義される)。

A DLT was defined as any of the following events during the DLT evaluation period (21 days).
All grade 5 events, anaphylaxis, grade ≥3 infusion-related reaction (IRR), hematological events (grade 4 neutropenia lasting more than 7 days, grade 4 thrombocytopenia for ≥7 consecutive days, grade 3/4 febrile neutropenia for more than 1 hour, grade 3/4 hemorrhage associated with thrombocytopenia of ≥ grade 3 requiring platelet transfusion, grade 4 anemia), nonhematological events (aspartate aminotransferase [AST] or alanine aminotransferase [ALT] elevations ≥ grade 2 with concomitant bilirubin >2.0×upper limit of normal [ULN] with no signs of cholestasis and no other reason can be found to explain the combination of increased ALT/AST and total bilirubin, ie, a Hy's law case, all nonhematological toxicities of grade ≥3 (severe or life-threatening), with exclusions per protocol).
