Beschrijving
Inclusiecriteria:
HBV-gerelateerde of/en HCV-gerelateerde patiënten met levercirrose op basis van pathologie of klinische diagnose;
- Antivirale therapie;
- Man of vrouw;
- ES toonde de aanwezigheid van slokdarm- en maagvarices en / of rode tekenen;
- Kind-Pugh < 10, en versmelting < 29;
- Handtekening van geïnformeerde toestemming.
Uitsluitingscriteria:
• Gebruikte antibiotica, prebiotica, probiotica en protonpompremmers binnen 2 weken;
- Alle contra-indicaties voor bètablokkers, waaronder astma, chronische obstructieve longziekte, allergische rhinitis, NYHA (New York Heart Association) klasse IV hartfalen, atrioventriculair blok, sinusbradycardie (HR < 50 / min), cardiogene shock, hypotensie (SBP < 85 mmHg), zieke sinussyndroom, insulineafhankelijke diabetes, perifere vasculaire ziekte;
- Onstabiele hoge bloeddruk en langdurig autorijden;
- Elke maligniteit die de overleving beïnvloedt, met uitzondering van de genezen;
- Patiënten met portale trombose;
- PT-verlenging langer dan 4 seconden, PLT<30×10^9/L;
- Zwangere en zogende patiënten;
- Geschiedenis van chirurgie voor portale hypertensie; Geschiedenis van eerdere EVL (endoscopische varicesligatie) of sclerotherapie, geschiedenis van chirurgie voor portale hypertensie inclusief portosystemische shunts, ontkoppeling en miltresectie en transjugulaire intrahepatische portosystemische shunt;
- Patiënten met ernstige ziekten van vitale organen zoals hart, longen, nieren, hersenen, bloed en zenuwstelsel;
- Allergisch voor carvedilol en berberine;
- Ernstige systemische ziekten;
- hemolytische anemie en gebrek aan glucose - 6 - fosfaatdehydrogenasepatiënten
- Weigering om deel te nemen aan het onderzoek.

Experimenteel: Carvedilol + berberine
Carvedilol wordt gestart met een dosis van 6,25 mg eenmaal daags.
Na 1 week wordt dit verhoogd tot een dosis van 6,25 mg tweemaal daags per dag als de systolische bloeddruk niet lager is dan 85 mm Hg en HR 55/min.
Berberine wordt gestart met een dosis van 0,3 g tweemaal daags.

Als de derde generatie NSBB is carvedilol effectiever in het verminderen van HVPG dan propranolol, dat door Baveno VI wordt aanbevolen als het eerstelijnsgeneesmiddel voor primaire preventie van EVB.
Berberine kan de darmflora reguleren, wat veilig en effectief is bij klinische toepassing.
