Een onderzoek naar de effecten van sarcosine op symptomen en hersenglycineniveaus bij mensen met schizofrenie (Sarc)
De effecten van remming van glycinetransport op de glycineconcentratie in de hersenen
Er is vastgesteld dat de NMDA-receptor een rol speelt bij stoornissen in het gebruik van middelen en bij schizofrenie. Een voorbeeld van het eerste is het effect van nicotine op de dopaminerge activiteit, niet alleen door de afgifte van dopamine in de beloningscentra van de middenhersenen te verhogen, maar ook door minder directe mechanismen die de alfa-7-nicotinereceptoren, NMDA-receptoren en glycine, een co-agonist voor de NMDA-receptoren. In termen van schizofrenie is de hypothese dat hypofunctie van de NMDA-receptor een rol speelt in het mechanisme voor negatieve symptomen en cognitieve disfunctie bij deze patiënten. De NMDA-hypofunctie kan worden omgekeerd met een verhoogde beschikbaarheid van synaptische glycine.
Sarcosine, of n-methyl-glycine, is een GlyT-1- en systeem A-transportremmer waarvan kan worden verwacht dat deze de beschikbaarheid van glycine in de synaptische ruimte vergroot. Sarcosine is een voedingssupplement dat in verschillende voedingsmiddelen voorkomt, zoals eidooiers en kalkoen.
Ons samenwerkingsteam heeft een nieuwe, niet-invasieve magnetische resonantiespectroscopie (MRS)-techniek ontwikkeld voor het meten van hersenglycineveranderingen waarmee we de glycinehomeostase kunnen bestuderen. Het doel van deze studie is om het effect van sarcosine (n-methyl-glycine) op de glycineconcentraties in de hersenen te onderzoeken. Het is onze hypothese dat orale sarcosine, bij een dosis van 2 gram per dag, goed wordt verdragen en gepaard gaat met verhoogde hersenglycineconcentraties. Het is onze secundaire verkennende hypothese dat verhogingen van hersenglycine geassocieerd zullen zijn met gedragstekenen van verhoogde NMDA- en dopamine-activiteit. Deze modulatie zou in de toekomst therapeutisch potentieel kunnen hebben voor aandoeningen van de hedonische en cognitieve functie.

Beschrijving
INSLUITINGSCRITERIA:
- Vrouwen en mannen van 18-65 jaar met DSM-IV-diagnose van schizofrenie of schizoaffectieve stoornis door diagnostisch interview en beoordeling van de kaart.
- Klinisch stabiel op een stabiele dosis antipsychotische medicatie gedurende ten minste één maand, momenteel geen actieve zelfmoordgedachten.
- Bevoegd om geïnformeerde toestemming te geven.
- Vrouwen in de vruchtbare leeftijd moeten een negatieve zwangerschapstest hebben bij de screening en moeten ermee instemmen een goedgekeurde vorm van anticonceptie te gebruiken tijdens het onderzoek.
- Screening van laboratoria binnen normale grenzen voor leeftijd en geslacht, behalve voor leverfunctietesten zoals hieronder gespecificeerd.
UITSLUITINGSCRITERIA:
- Diagnose van bipolaire stoornis, dementie, neurodegeneratieve ziekte of andere organische psychische stoornis.
- Geschiedenis van convulsies of CZS-tumor.
- Leverfunctietesten meer dan tweemaal normaal verhoogd.
- Boulimia of depressieve stoornis in de afgelopen 6 maanden.
- Levensbedreigende aritmie, cerebrovasculaire of cardiovasculaire gebeurtenis binnen 6 maanden. Huidige ernstige onstabiele medische ziekte, waaronder cardiovasculaire, lever-, nier-, ademhalings-, endocriene, neurologische of hematologische ziekte, zodat ziekenhuisopname voor behandeling van die ziekte waarschijnlijk binnen de komende 2 maanden is. Levenslange geschiedenis van meerdere hoofdletsels met neurologische gevolgen of een enkel ernstig hoofdletsel met blijvende neurologische gevolgen.
- Gebruik van onderzoeksmedicatie binnen 30 dagen na inschrijving.
- Gebruik van clozapine.
- Stoornis in het gebruik van andere middelen dan nicotine of cafeïne in de afgelopen 6 maanden (door zelfrapportage en speekseltest voor drugs en alcohol).
- Met een actueel risico op moord of zelfmoord.
