Dosisrespons op de noradrenalineprecursor Droxidopa bij hypotensieve personen met ruggenmergletsel
Het doel van deze studie is om de werkzaamheid van het medicijn Droxidopa (Northera) te bepalen bij het verhogen van de bloeddruk bij personen met hypotensie, lage bloeddruk, die is geclassificeerd als bloeddruk lager dan 110/70 bij mannen en 100/70 bij vrouwen. Het eerste doel is het bepalen van het deel van de proefpersonen met ruggenmergletsel (SCI) die een normotensieve reactie op Droxidopa hebben. De tweede is om het deel van de proefpersonen met dwarslaesie te bepalen die een hypertensieve reactie op Droxidopa vertonen. A Normale bloeddruk varieert van 111-139 bij mannen en 101-139 bij vrouwen en een hypertensieve bloeddruk is iets hoger dan 140 bij mannen en vrouwen.
De studie zou plaatsvinden in het James J. Peters VA Medical Center (JJPVAMC) en de Icahn School of Medicine op Mount Sinai (ISMMS) in Manhattan, New York.

Studie 1:
- Man of vrouw, leeftijd 18 tot 89 met traumatische dwarslaesie.
SCI-onderwerpen (n=40):
- Elk letselniveau;
- Elke American Spinal Injury Association Impairment Scale (AIS) graad van dwarslaesie;
- Niet afhankelijk van ventilator
- Voornamelijk rolstoelafhankelijk voor mobiliteit;
- Duur van letsel < 1 jaar
Lage bloeddruk:
- Systolische bloeddruk lager dan 110 mmHg en/of diastolische bloeddruk lager dan 70 mmHg voor mannen.
- Systolische bloeddruk lager dan 100 mmHg en/of diastolische bloeddruk lager dan 70 mmHg voor vrouwen.
- Primaire taal is Engels.

- Systolische bloeddruk lager dan 100 mmHg en/of diastolische bloeddruk lager dan 70 mmHg voor vrouwen.
- Primaire taal is Engels.
- In staat om geïnformeerde toestemming te geven
- Vertoonde een normotensieve bloeddruk als reactie op Droxidopa tijdens studie 1.
Uitsluitingscriteria:
- Huidige ziekte of infectie
Personen met frequente of ernstige autonome dysreflexie:
- Meer dan 3 symptomatische voorvallen per week
- BP ≥140/90 mmHg
- Melding van significante ongunstige subjectieve symptomen
- Hypertensie
- Elke andere neurologische aandoening dan SCI (ziekte van Alzheimer, dementie, beroerte, multiple sclerose, ziekte van Parkinson, enz.)
- Geschiedenis van epilepsie of andere convulsies
- Geschiedenis van traumatisch hersenletsel (TBI)
- Lever- of nierziekte
- Blaasproblemen waaronder verstopping van de urine en/of zwakke urinestraal.
- Diagnose van een psychiatrische stoornis zoals schizofrenie of bipolaire stoornis
- Bekende slagaderziekte, hartfalen, atrioventriculair blok en onregelmatige hartslag
- Allergieën voor droxidopa, asprin, polyethyleenoxide, polyethyleenglycol, hydroxypropylcellulose, gebutyleerd hydroxytolueen, magnesiumstearaat, hypromellose, geel ijzeroxide en rood ijzeroxide
- Grote operatie in de afgelopen 30 dagen
- Illegaal drugsgebruik in de afgelopen 6 maanden
- Zwanger
Uw voorgeschreven medicijnen zullen worden beoordeeld door de onderzoeksonderzoekers en het onderzoekspersoneel. Als u momenteel medicijnen gebruikt om een van de volgende aandoeningen te behandelen, meld dit dan aan de onderzoekers:

Experimenteel: Studie 1: dosisoptimalisatie van Northera
Proefpersonen zullen oraal droxidopa toegediend krijgen in een dosisescalatie, open-label manier beginnend met 200 mg.
De dosis zal bij volgende bezoeken naar boven worden aangepast met 100 mg totdat de gemiddelde systolische bloeddruk (SBP) die 60-120 minuten na toediening van de dosis wordt geregistreerd, 111-139 mmHg is bij mannen en 101-139 mmHg bij vrouwen, aanhoudende verhoging (≥ 30 opeenvolgende minuten ) bij zittende SBP ≥ 140/100 mmHg wordt de maximale dosis van 800 mg bereikt zonder adequate SBP-respons.
Proefpersonen zullen het testlaboratorium bezoeken op slechts 1 (200 mg) en maar liefst 7 (800 mg) dagen.
Zittende cardiovasculaire beoordelingen zullen gedurende 4 uur worden gecontroleerd en geregistreerd met tussenpozen van 15 minuten, en de vragenlijst over bijwerkingen zal elk uur worden afgenomen tijdens het onderzoek van 4 uur.
Elk studiebezoek duurt ongeveer 5 uur.

Placebo-vergelijker: Studie 2: Geblindeerde Placebo & Northera
De deelnemers krijgen vervolgens een orale optimale dosis Northera (Droxidopa) of een bijpassende placebo op een dubbelblinde manier toegediend en blijven gedurende 60 minuten in rugligging.
Proefpersonen blijven in hun rolstoel voor instrumentatie, waaronder: 1) ECG, 2) brachiale bloeddruk, 3) vingerarteriolaire bloeddruk en 4) Cerebrale bloedstroomsnelheid (CBFv).

Studie 1 is een dosisoptimalisatie, open-label studie van Northera met een dosisbereik van 200 mg tot 800 mg.
