Bottransplantatie vóór plaatsing van implantaten is de afgelopen 20 jaar een routineprocedure geworden. Er is een 5-jaarsoverleving van maximaal 98,3% gerapporteerd voor implantaten die in getransplanteerd bot zijn geplaatst.
Autologe bottransplantaten worden beschouwd als de gouden standaard. Het slagingspercentage van de transplantatieprocedure kan echter worden beïnvloed door verschillende risicofactoren.
Een bijzondere uitdaging wordt gevormd door een uitgebreid transplantaat van de alveolaire kam, waarbij relatief hoge complicatiepercentages tot 20% worden gemeld, meestal dehiscentie. Ernstigere complicaties zoals dehiscentie of mobilisatie van het transplantaat werden waargenomen bij een derde van de rokers vergeleken met een complicatiepercentage van slechts 7,7% voor niet-rokers. Complicaties zoals lapnecrose, dehiscentie en resorptie zijn vaak complicaties van de weke delen.
Een spanningsvrije wondsluiting is een sleutelfactor in het succes van bottransplantaten. Periosteale incisies zijn een gebruikelijke techniek voor flapextensie. Te veel reliëfincisies in het periosteum kunnen echter ook leiden tot een te dunne of uitgerekte wondlap.
Dit type behandeling van zacht weefsel kan leiden tot perforatie of necrose van de flap boven het bottransplantaat.
In 1987 rapporteerde Härle over een tunneltoegang in verband met een techniek voor preprothetische kaakkamtransplantatie in het mandibulaire zijtandgebied met botvervangende materialen.
In de klinische ervaring van de auteurs kan het gebruik van een tunneltechniek voor preparatie zonder een crestale incisie een alternatief vormen met autologe bottransplantaten voor conventionele chirurgische procedures met een trapeziumflapontwerp.
