Definitie van de voorgestelde samengestelde multimodale biomarker - De onderzoekers stellen voor om een kwantitatieve classificator op weefselniveau te ontwikkelen, gebaseerd op kwantitatieve metrieken (kenmerken) afgeleid van ultrasone elastografie, Doppler, bio-impedantiespectroscopie en hogedichtheidelektromyografie, als indicator voor het normale biologische proces in myofasciale weefsels en pathogene processen in actieve en latente fasen van myofasciale pijn.
Algemene aanpak en wetenschappelijke nauwkeurigheid: In Doel 1 zullen de onderzoekers methoden ontwikkelen om reproduceerbare metrieken (kenmerken) te genereren uit de metingen op ruw weefselniveau en de minimaal detecteerbare verandering in deze kenmerken te bepalen in een pilotstudie. In Doel 2 zullen de onderzoekers een longitudinaal observationeel onderzoek uitvoeren met twee groepen proefpersonen (controle- en myofasciale pijn). De onderzoekers zullen een classificatie-algoritme ontwikkelen dat optimaal onderscheid maakt tussen de actieve en latente fase van myofasciale pijn en normaal myofasciaal weefsel.
Bestudeer populatie en anatomische locatie. De gekozen pijnconditie is chronische nek- en schouderpijn. De onderzoekers zullen twee groepen proefpersonen rekruteren: Groep 1: Chronische myofasciale pijn zoals bepaald door klinisch basisonderzoek met behulp van de criteria van Travell en Simon7 en Groep 2: pijnvrije controles. De onderzoekers zullen zich concentreren op twee gestandaardiseerde anatomische locaties (Figuur 4). Dit maakt beeldvorming mogelijk van de mediale bovenste trapezius- en de infraspinatus-spieren, die veel voorkomende locaties zijn voor MTrPs55, evenals de levator-scapulae. Deze drie spieren hebben een heel verschillende morfologie en fascia45. De levator is een spoelvormige spier met goed gedefinieerde fascia die de spier omvat, terwijl de trapezius een dunnere fascia heeft van waar perimysium septae de spierbuik kruist. De infraspinatus heeft meerdere fasciale lagen op het oppervlak en heeft duidelijke segmentale verbanden met het C5-6-segment56 Geschiktheidscriteria: De onderzoekers zullen volwassenen van 18-65 jaar oud rekruteren. Uitsluitingscriteria: (1) diagnose van fibromyalgie, chronisch vermoeidheidssyndroom of chronische ziekte van Lyme; (2) Diagnose van cervicale radiculopathie, neuropathie of neuritis; (3) Voorgeschiedenis van operaties aan het hoofd, de nek, de halswervelkolom of de schoudergordel; (4) Atypische gezichtsneuralgie; (5) Nieuwe medicatie of verandering in medicatie in de afgelopen 6 weken; (6) Huidige keel- of oorinfectie.
Maskeren en matchen: dit is een enkelblind longitudinaal observationeel onderzoek. Het team dat de gegevensverzameling en -analyse uitvoert, kent de groepsindeling van de proefpersonen niet en zal blind zijn voor de resultaten van de klinische evaluaties. De twee groepen zullen qua leeftijd en geslacht op elkaar worden afgestemd met behulp van een gepaarde rekruteringsstrategie57. De onderzoekers zullen een groep geschikte controlepersonen zonder voorgeschiedenis van pijn identificeren en deze verdelen in geslachts- en leeftijdscategorieën (18-30; 31-50; en >50). Voor elke proefpersoon uit Groep 1 die in een reeks wordt gerekruteerd, zullen de onderzoekers een overeenkomende proefpersoon uit Groep 2 uit de pool rekruteren.
Seks als biologische variabele: Myofasciale pijn komt wijdverbreid voor in de gemeenschap en treft zowel mannen als vrouwen. Trapeziusmyalgie komt vaker voor bij vrouwen58. De onderzoekers zullen gebruik maken van op leeftijd en geslacht afgestemde groepen, en zullen de prestaties van de classificatie testen voor zowel de gepoolde populatie als afzonderlijk per geslacht om eventuele genderspecifieke verschillen in de biomarkermetingen te identificeren.
Uitkomstmaten: De primaire uitkomstmaat zal de samengestelde classificator zijn, gebaseerd op de kwantitatieve biomarkers op weefselniveau. De onderzoekers zullen gedurende drie maanden elke maand herhaalde gegevensverzamelingen uitvoeren. Het klinische fenotype van de proefpersonen (normaal, latent, episodisch actief en aanhoudend actief) zal worden bepaald door een uitgebreid lichamelijk onderzoeksprotocol12. De onderzoekers zullen de NIH HEAL Common Data Elements voor chronische pijn bij volwassenen gebruiken om zelfrapportages te verzamelen. Om het klinische fenotype verder te karakteriseren, als secundaire uitkomstmaat, zullen de onderzoekers een toepassing voor ecologische tijdelijke beoordeling (EMA) (Metricwire) op een smartphone gebruiken om een dagelijkse pijnbeoordeling te verkrijgen die op willekeurige punten gedurende de dag wordt geactiveerd en om geautomatiseerde activiteitsmonitoring te verzamelen van de smartphonesensoren. De onderzoekers zullen ook wekelijks de pijnintensiteit en -interferentie van 3 items verzamelen59.
Procedures voor gegevensverzameling Gegevensbeheer: Dit is een onderzoek op één locatie. Alle onderzoeksprocedures worden uitgevoerd bij Mason. De onderzoeksbiostatisticus (Rosenberger) zal de juiste maskeercontroles en elektronische casusrapportformulieren (eCRF's) opzetten in het elektronische gegevensverzamelingssysteem (REDCap). Alle onderzoeksgegevens worden met behulp van eCRF's in REDCap ingevoerd. Het studiepersoneel zal beschikken over passende, op rollen gebaseerde toegangscontroles in REDCap. Bronvalidatie wordt uitgevoerd met behulp van de ingebouwde controles van REDCap.
Maskeren: Eén enkele arts (Gerber) zal de toestemming van elke proefpersoon verkrijgen en de anamnese en het lichamelijk onderzoek uitvoeren. Er kan een extra arts (DeStefano) aanwezig zijn om te helpen, en er zal een onderzoeksassistent aanwezig zijn om aantekeningen te maken en gegevens in te voeren. Het engineeringteam, onder toezicht van de PI (Sikdar) en co-I Chitnis, verzamelt de uitkomstmaten in een aparte ruimte en wordt gemaskeerd voor de geschiedenis van de patiënt en de resultaten van het lichamelijk onderzoek. Voor het onderzoek zal een handboek met operationele procedures worden ontwikkeld.
Procedures voor gegevensanalyse. Alle data-analyse zal worden uitgevoerd door een afgestudeerde onderzoeksassistent in de biostatistiek onder supervisie van de datawetenschapper (Lee) en de studiebiostatisticus (Rosenberger).
Primaire analyse: De onderzoekers zullen een classificatie-algoritme met meerdere klassen construeren en rigoureus valideren, gebaseerd op functionele tijdreeksen en statistische leermethoden. Hier kunnen de tijdreeksen van biomarkers worden weergegeven als een combinatie van unieke temporele patronen/signalen of basisfuncties. Deze functies omvatten tijdsinvariante eigenbasisfuncties80, afvlakkingsplines81, wavelets82 of functionele hoofdcomponenten83. Met behulp van functionele data-analyse zal een samengestelde voorspellende variabele worden geconstrueerd die de relevante informatie in de biomarker-tijdreeksen samenvat. Vervolgens zal een classificatiemethode met meerdere klassen worden geconstrueerd met behulp van begeleide leerbenaderingen, zoals ondersteuningsvectormachines84, discriminantanalyse85,86, neurale netwerken87,88 en regressiebomen89. Het classificatiealgoritme zal de samengestelde voorspeller gebruiken om proefpersonen in de vier relevante categorieën te codificeren (pijn - episodisch, pijn - actief, controle-episodisch en controle-actief). K-voudige kruisvalidatie zal worden gebruikt om de nauwkeurigheid van de classificator te beoordelen op basis van gevoeligheid, specificiteit, F1-score en het gebied onder de ROC-curve voor scenario's met meerdere klassen90,91.
Secundaire analyse: Er zullen verschillende secundaire analyses worden uitgevoerd, waaronder: (1) Bepaal normatieve waarden van biomarkers in de controlegroep (Groep 2); (2) Evalueer de convergente validiteit van primaire en secundaire biomarkers. Omdat de onderliggende grondwaarheid niet direct kan worden gemeten, zullen de primaire en secundaire biomarkers worden gebruikt om de convergente validiteit te evalueren; (3) Correlatie met overeenkomstige klinische maatstaf (bewegingsbereik, drukpijndrempel.

We zullen de mate van opwinding kwantificeren (temporele sommatie) door de versterking van pijn te meten naar een reeks nociceptieve stimuli (een logaritmisch geschaalde reeks gewogen speldenprikken) die worden toegepast op dermatomen die segmentaal verbonden zijn met de beoogde trapezius- en infraspinatus-spieren.
De proefpersonen zullen na elke stimulus een numerieke pijnbeoordelingsscore (NPRS, 11-puntsschaal) geven.
De liquidatieratio (WUR) wordt berekend als de verhouding tussen de gemiddelde NPRS en de basislijn.
