Een multicenter, gerandomiseerd, open-label, parallel-gecontroleerd, gebeurtenis-gestuurd, geblindeerd-eindpunt onderzoek naar de werkzaamheid en veiligheid van Ongericimab-injectie bij hoogrisicopatiënten met een beroerte met intracraniële of extracraniële atherosclerotische stenose.

De Oris-studie heeft tot doel te evalueren of het gebruik van Ongericimab-injectie bij patiënten met atherosclerotische ischemische cerebrovasculaire ziekte binnen 3 maanden na het ontstaan het risico op terugkerende grote cardiovasculaire gebeurtenissen kan verminderen door het bereiken van lagere lipidenverlagende streefwaarden (LDL-C < 1,4 mmol/L).

Insluitingscriteria:

  • Verkrijgen van geïnformeerde toestemming;
  • Leeftijd ≥18 jaar;
  • Patiënten met ischemisch herseninfarct binnen 3 maanden (NIHSS<15 voor randomisatie);
  • Aanwezigheid van ≥50% stenose in grote intracraniële of extracraniële arteriën, gerelateerd aan de symptomen van de huidige episode of de locatie van het infarct;
  • LDL-C ≥ 70 mg/dL (1,8 mmol/L) bij screening.

Uitsluitingscriteria:

  • Geschiedenis van hersenbloeding op enig moment (microbloedingen alleen aanwezig op SWI zijn geen uitsluitingscriterium)
  • Bloeding of andere pathologische neurologische aandoeningen op baseline hersen CT/MRI (bijv. vasculaire malformaties, tumoren, abcessen, of veelvoorkomende niet-ischemische hersenaandoeningen zoals multiple sclerose);
  • Aanwezigheid van geïsoleerde sensorische symptomen (bijv. gevoelloosheid), geïsoleerde visuele veranderingen, of geïsoleerde duizeligheid of vertigo, maar geen bewijs van recent infarct op baseline hoofd CT of MRI;
  • Niet in staat om de beoordeling van intracraniële en extracraniële arteriële stenose voor randomisatie te voltooien
  • mRS score ≥2 voor aanvang (gebaseerd op beoordeling van medische geschiedenis);
  • Beroep veroorzaakt door angioplastiek/vaatchirurgie;
  • Cardio-embolisch herseninfarct veroorzaakt door atriumfibrilleren, kunsthartkleppen, endocarditis, mitralisstenose, sinusknoopdisfunctie, etc.
  • Meest recente nuchtere triglyceriden >400 mg/dL (4,5 mmol/L) voor randomisatie;
  • Ongecontroleerde hypertensie (SBP >180 mmHg of DBP >110 mmHg);
  • Hypothyreoïdie gediagnosticeerd binnen 1 maand voor randomisatie.
  • Ernstig nierfalen (eGFR <30 mL/min/1,73m²);
  • Actieve leverziekte of -disfunctie (AST/ALT >3×ULN binnen 30 dagen voor randomisatie);
  • NYHA Klasse III/IV hartfalen binnen 1 jaar voor randomisatie;
  • Levensbeperkende niet-cardiovasculaire ziekten (levensverwachting <1 jaar);
  • Maligniteit binnen 10 jaar (uitgezonderd adequaat behandelde basaalcelcarcinoom, cervicale CIS, DCIS, of stadium 1 prostaatkanker);
  • Actieve fase van infectie (inclusief luchtweginfectie, urineweginfectie, of gastro-enteritis), of momenteel gebruik of gepland gebruik van orale of intraveneuze anti-infectieuze behandeling vanwege infectie;
  • Bekende overgevoeligheid voor monoklonale antilichaamtherapieën;
  • PCSK9-remmer gebruik binnen 3 maanden voor randomisatie.
  • Deelname aan andere geneesmiddelen/apparatuurproeven binnen 30 dagen;
  • Geschiedenis van ernstig drugs- of alcoholmisbruik in het afgelopen jaar.
  • Vrouwen in de vruchtbare leeftijd zonder anticonceptie, zwangerschap, of lactatie;
  • Onvermogen om de studie te begrijpen of te volgen vanwege psychische aandoening, cognitieve of emotionele stoornissen, of andere redenen die door de onderzoeker als ongeschikt voor deelname aan de studie worden beschouwd.
