Critérios de inclusão Coorte A
- Os pacientes devem ter comorbidades de modo que os Co-PIs do cirurgião e do cardiologista concordem que o risco previsto de mortalidade operatória é ≥15% e/ou uma pontuação STS mínima de 10
- O paciente tem estenose valvular aórtica degenerativa senil com critérios ecocardiográficos: gradiente médio >40 mmHg ou velocidade do jato maior que 4,0 m/s ou área inicial da valva aórtica < 0,8 cm2
- O paciente é sintomático de sua estenose da válvula aórtica, conforme demonstrado pela classe funcional II da NYHA ou superior
- O sujeito ou seu representante legal foi informado sobre a natureza do estudo, concorda com suas disposições e forneceu consentimento informado por escrito conforme aprovado pelo IRB do respectivo centro clínico
O sujeito e o médico assistente concordam que o sujeito retornará para todas as visitas de acompanhamento pós-procedimento necessárias
Coorte B Todos os candidatos para a Coorte B deste estudo devem atender #2, 3, 4, 5 dos critérios acima e
- O sujeito, após consultas formais por um cardiologista e dois cirurgiões cardiovasculares, concorda que fatores médicos impedem a operação, com base na conclusão de que a probabilidade de morte ou morbidade grave e irreversível excede a probabilidade de melhora significativa. Especificamente, a probabilidade de morte ou morbidade grave e irreversível deve exceder 50%.
Critério de exclusão
- Evidência de infarto agudo do miocárdio ≤ 1 mês antes do tratamento pretendido.
- A válvula aórtica é uma válvula unicúspide ou bicúspide congênita; ou não é calcificado
- Doença valvular aórtica mista (estenose aórtica e regurgitação aórtica com regurgitação aórtica predominante >3+).
- Qualquer procedimento cardíaco terapêutico invasivo realizado até 30 dias após o procedimento índice (ou 6 meses se o procedimento for um implante de stent coronário com eluição de medicamento).
- Válvula cardíaca protética pré-existente em qualquer posição, anel protético ou insuficiência mitral grave (maior que 3+).
