Descrizione
Criteri di inclusione Coorte A
- I pazienti devono avere comorbilità tali che il chirurgo e il cardiologo Co-PIs concordino sul fatto che il rischio previsto di mortalità operatoria sia ≥15% e/o un punteggio STS minimo di 10
- Il paziente presenta una stenosi valvolare aortica degenerativa senile con criteri derivati dall'ecocardiografia: gradiente medio >40 mmHg o velocità del getto maggiore di 4,0 m/s o area valvolare aortica iniziale < 0,8 cm2
- Il paziente è sintomatico a causa della sua stenosi valvolare aortica, come dimostrato dalla Classe funzionale NYHA II o superiore
- Il soggetto o il rappresentante legale del soggetto è stato informato della natura dello studio, accetta le sue disposizioni e ha fornito il consenso informato scritto come approvato dall'IRB del rispettivo centro clinico
Il soggetto e il medico curante concordano che il soggetto tornerà per tutte le visite di follow-up post-procedura richieste
Coorte B Tutti i candidati per la Coorte B di questo studio devono soddisfare n. 2, 3, 4, 5 dei criteri di cui sopra e
- Il soggetto, dopo consultazioni formali da parte di un cardiologo e due chirurghi cardiovascolari, concorda sul fatto che i fattori medici precludono l'operazione, sulla base della conclusione che la probabilità di morte o di morbilità grave e irreversibile supera la probabilità di un miglioramento significativo. Nello specifico, la probabilità di morte o di morbilità grave e irreversibile dovrebbe superare il 50%.
Criteri di esclusione
- Evidenza di infarto miocardico acuto ≤ 1 mese prima del trattamento previsto.
- La valvola aortica è una valvola unicuspide o bicuspide congenita; o non è calcificato
- Malattia mista della valvola aortica (stenosi aortica e rigurgito aortico con rigurgito aortico predominante >3+).
- Qualsiasi procedura cardiaca terapeutica invasiva eseguita entro 30 giorni dalla procedura indice (o 6 mesi se la procedura consisteva in un impianto di stent coronarico a rilascio di farmaco).
- Valvola cardiaca protesica preesistente in qualsiasi posizione, anello protesico o insufficienza mitralica grave (superiore a 3+).
