患有蛋白尿性肾病的患者将被纳入并首先接受阿米洛利或氨苯蝶啶,一种与阿米洛利作用于上皮钠通道 (ENaC) 的类似利尿剂,但不抑制尿激酶纤溶酶原激活物受体 (uPAR),将用作对照。 然后患者将交叉接受另一种药物。 我们假设阿米洛利可能对患有蛋白尿性肾病的患者有益,并且可以用作减少蛋白尿和延缓该患者人群肾病进展的辅助疗法。

学习规划:
研究包括3个阶段。 将招募 30 名患者参加这项研究。 所有患者都需要在研究前至少两个月每天服用血管紧张素转换酶 (ACE) 抑制剂或血管紧张素受体阻滞剂 (ARB)。
第 1 阶段:患者将随机接受阿米洛利 5 毫克,每天两次或氨苯蝶啶 50 毫克,每天两次,持续 8 周。 将在第 1 阶段开始前一周和之后一周监测血清钾。 如果血清钾保持等于或低于 5.0 毫摩尔/升,阿米洛利或氨苯蝶啶将继续以相同剂量服用,直至第 1 阶段结束。 如果血清钾等于或高于 5.5 mmol/L,患者将退出研究,并报告不良事件。 如果血清钾在 5.1-5.4 之间 mmol/L,一周后再监测一次。 如果血清钾高于 5.5 mmol/L,患者将退出研究,并报告不良事件。 如果血清钾保持在相同范围内,患者将继续以相同剂量使用阿米洛利或氨苯蝶啶以完成第 1 阶段。

- 患有任何类型的蛋白尿性肾病的患者
- 18-75岁
- 蛋白尿≥1g/天
- 估计肾小球滤过率 (eGFR) ≥ 30ml/min/1.73m2
排除标准:
- 狼疮性肾炎或 HIV 相关肾病的临床证据
- eGFR <30ml/min/1.73m2
- 盐皮质激素受体拮抗剂(螺内酯、依普利酮)治疗的要求
- 肾移植后状态
- 六个月内接受过糖皮质激素
- 血清 K >4.8 mmol/L
- 总二氧化碳 <17 mmol/L
- 血红蛋白 <10 克/分升
- 对袢利尿剂或保钾利尿剂禁忌或过敏
- 肝功能检查异常
