详细说明
经国家癌症研究所审查委员会批准后,将于 2016 年 11 月至 2017 年 11 月从墨西哥国家癌症研究所乳腺癌科招募符合条件的受试者。 将通过手术安排表确定正在接受全乳房切除术 (TM)、改良根治性乳房切除术 (MRM) 和/或腋窝淋巴结清扫术 (ALND) 并使用手术引流管的确诊癌症患者。 如果受试者符合纳入标准,将由其中一位主要研究人员进行访谈。 在初次访谈中,将阅读并签署知情同意书。 此外,研究协调员将从医疗记录中收集感兴趣的数据,并将其登记在专门为此项目创建的数据收集表中。
在知情同意后,参与者将通过计算机随机化程序随机分配到标准引流护理方案或引流消毒方案,使用动态分配并通过外科手术(根治性手术或保守性手术)进行分层。 患有双侧癌症的受试者在两侧都接受了相同的治疗方案。 手术外科医生将在手术结束时知道分配的治疗组。
手术将包括在手术结束时将葡萄糖酸氯己定封闭性粘性敷料放置在引流管侧。 敷料将每 7 (+-1) 天更换一次,直到引流管被移除。
所有参与者和主要照顾者,尽管参与组,将在术后的第一个小时内,在出发前接受研究小组成员的个人指导。 此外,他们将获得有关引流管和手术伤口一般护理的纸质说明。
实验组每7(+-1)天换药一次;必须用 70% 浓度的异丙醇毛巾清洁插入部位。 与通常在这些患者中进行的一样,将每 12 小时对排出的液体进行一次评估和量化。
如果发生感染性并发症,将从手术开始对患者进行 30 天的随访,直至感染性并发症得到解决。 患者将在术后第一周结束时离开 (POP 7+-1)。 在每次访问中,研究协调员都会在登记表中登记患者的信息。 每次就诊时都会对患者进行评估,手术伤口的特征和引流液的特征将记录在协议登记表中。 调查员将验证参与者是否正确清理了排水管。
所有患者都将在术后第一周和第二周接受引流球的半定量培养。 研究人员还将取下每个取出的引流管内段的远端部分进行半定量培养。 所有参与者都将在随访时评估感染的临床体征和对引流消毒的任何不良反应。 如果发生手术部位感染,患者将接受正确的治疗,并将继续接受监测直至解决。

纳入标准:
- 女性最低年龄 18 岁能够给予知情同意
- 经组织病理学证实的乳腺癌
- 接受单侧或双侧乳房切除术,伴或不伴即刻扩张器重建的患者。
排除标准:
- 男性
- 拒绝知情同意的患者
- 对患病的乳房进行过放射治疗。
- 孕妇或哺乳期妇女
- 接受 Deep Inferior 即刻乳房重建的患者
- 上腹穿孔器 (DIEP) o 横向直肌腹肌技术 (TRAM)。

- 手术日期前十四天使用抗生素
- 在过去三个月内在 INCan 之外有过或怀疑有乳腺癌手术史的患者。
