背景 中风的全球发病率正在上升,在以色列,40%的中风患者需要住院康复。 改善活动能力,尤其是中风后的行走能力,对于降低跌倒风险极为重要,并且与更高的患者自主权和改善的生活质量密切相关。 在步行平衡维持期间,是依赖于三种感觉信息形式的复杂功能。 中风患者的感觉加权过程受到损害,因此他们过度依赖视觉信息来控制他们的姿势。 应用前庭输入,例如前庭眼反射 (VOR) 习惯训练,作为前庭聚焦康复 (VR) 的一部分,是触发中风后患者感觉重新加权过程的方法之一。 VOR 在平衡中的作用是在头部运动期间帮助稳定视网膜上的图像。 可能可以解释中风后患者步态期间头部稳定的现象,因为 VOR 功能下降导致动态视力 (DVA) 下降,试图保持凝视稳定性。
目的 本研究的目的首先是调查 VOR 运动对无头晕的脑卒中后患者步态速度和平衡的有效性,其次是测试 DVA 反映的 VOR 效率与步态速度之间的相关性。
方法 一项前瞻性实验研究,随机对照,双盲(评估员和统计学家)。 在以色列 Sheba 医疗中心的老年康复科住院的 60 名 65 岁及以上的成年人将被分为对照组和干预组。 干预将包括物理治疗课程,其中 10 分钟专门用于 VOR 习惯训练,与没有 VOR 训练的物理治疗课程相比。

中风后 0-3 个月内(缺血性或出血性;前循环或后循环;非椎基底动脉中风)。
排除标准:
任何重大疾病或静息时血压>110/200 mmHg;迷你精神状态测试分数 <24,全球;接受性失语或无法遵循两点指令;在没有身体帮助的情况下无法行走 16 米。
