Návrat genomických výsledků a souhrnná penetrace v populačních kohortách (PopSeq)

Cíle tohoto projektu jsou: 1) Vrátit klinicky použitelné genomické výsledky účastníkům a sledovat výsledky. Mezi žijícími účastníky FHS/JHS, kteří souhlasili s gRoR, budeme kontaktovat ty, u kterých je objevena škodlivá akceschopná varianta v jednom z genů uvedených na seznamu sekundárních nálezů doporučených ACMG (odhad 2 % účastníků). 2) Zlepšit vysoce výkonné metody pro identifikaci platných patogenních variací. Zdokonalte a aplikujte metody pro vysoce výkonný screening genomů FHS/JHS způsobem, který si zachová vysokou citlivost pro detekci škodlivých variant v ~3500 genech spojených s Mendelovou chorobou a zároveň sníží míru falešných objevů variant, které nejsou patogenní/pravděpodobně patogenní. 3) Prozkoumejte penetraci agregátů pro mendelovské choroby. Zkontrolujte údaje o fenotypu od podskupiny účastníků FHS a JHS a porovnejte je s genotypovými údaji.
Údaje, které mají být shromážděny, zahrnují výsledky a fenotypové údaje o jednotlivcích, kteří souhlasí s gRoR a kteří se dozvěděli, že mají škodlivou variantu v jednom z genů uvedených v ACMG. Tyto údaje budou vykazovány prostřednictvím průzkumů a budou přezkoumány dostupné lékařské záznamy. Další fenotypová data mohou být shromážděna a přezkoumána pro jiné geny mendelovské choroby, které nelze ovlivnit, za účelem prozkoumání genomové penetrace.
Účastníci výzkumu, kteří jsou identifikováni se škodlivou variantou v použitelném genu, mohou získat přímé zdravotní přínosy z učení se této informace; nicméně vracení genomických výsledků zdravým jedincům, kteří nemají lékařskou indikaci, může představovat neočekávané škody související s variantně zaměřeným zvýšením screeningu a léčby. Tato studie je zaměřena na zkoumání přínosů a jakýchkoli potenciálních škod souvisejících s navracením genomických informací v populačních kohortách. Umožní nám to také lépe porozumět penetraci těchto variant ve dvou populacích, které nebyly vybrány pro stav onemocnění, a umožní nám to porovnat výsledky u primárně afroamerické populace vs. kavkazské populace. Vývoj metod pro zefektivnění analýzy variant pomůže zlepšit efektivitu laboratoře a pokročí v oblasti ošetřování a analýzy variant.

Popis
Kritéria pro zařazení:
- Žijící jedinci zapsaní do Framingham Heart Study a Jackson Heart Study, kterým byly sekvenovány genomy v rámci programu TOPMed.
- Dospělí starší 18 let
- Ti, kteří souhlasili s použitím jejich vzorků DNA pro výzkumné účely (a ti, kteří se účastní gRoR, kteří souhlasili s přijímáním genomických informací).
Kritéria vyloučení:
- Účastníci Framingham Heart Study nebo Jackson Heart Study, kterým nebyly sekvenovány jejich genomy v rámci TOPMed
- Účastníci, kteří se nerozhodli pro genomický/genetický výzkum
- Účastníci, kteří souhlasili/nesouhlasili s obdržením genomického výsledku (pouze pro část gRoR této studie)

Experimentální: Účastníci FHS a JHS s funkčním genomickým nálezem

Total Costs of Program Implementation

We will determine the costs and associated time demands of implementing gRoR using a microcosting approach in which study staff track the amount of time they spend and the resources they use for each step of the protocol.
Ranges are based on 50% to 200% of point estimates given variability in wages and prices of services.

The survey assessed the disclosure-specific impact of information on distress and positive emotions using an adapted 12-item version of the FaCTOR, a validated instrument developed for genomic sequencing that is sensitive to responses to high- and low-risk genetic risk results.
Subscales assess negative emotions (range: 3 to 15), positive feelings (reverse-scored, range: 4 to 20), uncertainty (range: 3 to 15), and privacy concerns (range: 2 to 10), with higher scores on each subscale indicating more negative experiences.

General Anxiety

We measure general anxiety using a 4-item version of the General Anxiety Disorder Scale, a validated scale that allows investigators to identify individuals with a potential mood disorder.
