Évaluation de l'efficacité de l'acide hyaluronique intra-articulaire (Sinovial®) vs Synvisc® dans le traitement de l'arthrose symptomatique du genou. Une étude de non-infériorité en double aveugle, contrôlée, randomisée, en groupes parallèles
Le but de cette étude est de prouver la non-infériorité de l'injection intra-articulaire d'acide hyaluronique (Sinovial®) dans le traitement symptomatique de la gonarthrose par rapport à Synvisc®.

La description
Critère d'intégration:
- Âge entre 40 et 80 ans
- Arthrose primaire du genou du compartiment fémoro-tibial médial ou latéral
- Symptômes depuis au moins 3 mois
- Diagnostic selon les critères ACR
- Kellgren & Lawrence grade radiologique 2-3
- Patients n'ayant pas suffisamment répondu (non répondeurs) aux antalgiques et/ou AINS pris régulièrement ou répondeurs intolérants à l'utilisation régulière d'analgésiques et/ou d'AINS.
- Sous-score moyen de douleur WOMAC au niveau du genou cible > 40 mm et < 80 mm sur une EVA à l'inclusion après le sevrage des analgésiques/AINS · Si arthrose bilatérale du genou : définition de l'articulation la plus symptomatique au dépistage en fonction de l'évaluation du patient et de l'examen clinique de l'investigateur jugement et sous-score moyen de douleur WOMAC à l'inclusion dans le genou controlatéral < 30 mm
- Consentement éclairé écrit
- Sujet capable de comprendre, coopératif et fiable
Critère d'exclusion:
- IMC >= 32 kg/m2
- OA secondaire (post-traumatique) du genou
- Douleur/syndrome fémoro-patellaire prédominant
- Aucune largeur d'espace de joint restante
- Coxarthrose symptomatique ou autre problème de santé interférant
- Déformation varus/valgus sévère (>15°)
- Antécédents/preuves actuelles de (genou cible) : maladies articulaires inflammatoires, infectieuses ou métaboliques ; chondrocalcinose clinique récurrente ; les arthropathies cristallines; les pathologies ostéo-articulaires différentes de l'arthrose ; fracture articulaire; ochronose; acromégalie; hémochromatose; la maladie de Wilson ; ostéochondromatose primaire; troubles héréditaires; mutations du gène du collagène.
- Maladie rhumatismale concomitante
- Blessure importante au genou cible au cours des 6 derniers mois
- Arthroplastie/arthroplastie précédente (genou cible)
- Arthroscopie/ostéotomie/chirurgie au cours de la dernière année (genou cible)
- Toute intervention chirurgicale prévue dans les 6 prochains mois
- Stase veineuse ou lymphatique du membre concerné
- Infection cutanée/maladie/traumatisme au site d'injection
- Systémique ou i.a. (genou cible) corticoïdes au cours des 3 derniers mois
- I.a. corticostéroïdes (articulation non cible) au cours des 4 dernières semaines
- Viscosupplémentation (genou cible) au cours de la dernière année
- Physiothérapie récemment commencée (au cours des 3 derniers mois) (genou cible)
- Récemment (au cours des 3 derniers mois) a commencé un traitement ou a changé la posologie de SYSADOA
- Traitement anticoagulant en cours
- Utilisation chronique ou récurrente d'AINS/analgésiques/narcotiques en raison de maladies différentes de l'arthrose du genou cible
- Antécédents d'allergie ou d'hypersensibilité à l'acide hyaluronique ou au paracétamol ou aux protéines aviaires
- Participation à une étude clinique au cours des 3 derniers mois
- Femmes enceintes ou allaitantes et femmes en âge de procréer qui ne souhaitent pas utiliser une contraception adéquate
- Patients incapables de rester dans l'étude pendant 6 mois, non coopérants, incapables de comprendre
