RESPIMER® Netiflow® vs solutions salines pendant l'irrigation nasale après une ethmoïdectomie bilatérale (polypose nasale) (NETIFLOW)
Une étude comparative, multicentrique et randomisée évaluant l'innocuité et l'efficacité de RESPIMER® Netiflow® par rapport aux solutions salines pendant l'irrigation nasale après une ethmoïdectomie bilatérale dans le traitement de la polypose nasale.

Description détaillée
Il s'agit d'une étude comparative entre deux traitements postopératoires consécutifs à une chirurgie du sinus ethmoïdal. Le but de cette étude est de comparer l'effet de la solution RESPIMER® NetiFlow® sur la réparation des tissus nasaux par rapport à une solution saline (0,9% NaCl), le traitement standard.
Le type de chirurgie retenu pour cette étude est l'ethmoïdectomie endoscopique totale. Il s'agit d'une chirurgie standard fréquemment utilisée qui nécessite un nettoyage nasal pendant 28 jours en moyenne. Ce processus permet la régénération de la muqueuse épithéliale nasale de la plaie (zones d'os exposées), ainsi que la guérison de l'enflure et des cicatrices dues à la chirurgie.
Le suivi dure 28 jours avec 4 visites (J7, J14, J21 et J28).
RESPIMER® NetiFlow® : dispositif de classe I, avec marquage CE, dans l'indication des soins postopératoires des sinus.

Critère d'intégration:
- Hommes ou femmes entre 18 et 65 ans
Sujet nécessitant une chirurgie des sinus sous forme d'ethmoïdectomie totale bilatérale par voie endoscopique endonasale dont la pathologie résulte d'une :
- Polypose sinonasale seule
- Polypose sinonasale associée à un asthme et/ou une intolérance aux AINS
- Consentement écrit libre et éclairé
- Adhérent ou bénéficiaire d'un programme de sécurité sociale
Critère d'exclusion:
- Traitement corticoïde oral depuis moins de 2 mois
- Femmes préménopausées n'utilisant pas de contraception efficace (dispositif oral ou intra-utérin)
- Femmes enceintes ou allaitantes
- Diabète non contrôlé (non traité ou stabilisé par un traitement)
- Statut séropositif
- Maladies auto-immunes affectant le nez et/ou les sinus, dysfonctionnements immunitaires (granulomatose de Wegener, syndrome de Churg-Strauss, Déficit Immunitaire Commun Variable, sarcoïdose...), ou maladies malignes (histologie incertaine)
- Toute contre-indication de RESPIMER® NetiFlow®
- Toutes les maladies entraînant des difficultés à tousser ou à avaler
- Radiothérapie en cours ou passée à la tête et au cou
- Chimiothérapie en cours ou récente (dans une période de trois mois)
- Sujets utilisant des anticoagulants
- Sujets participant à une autre étude qui comprend une période d'exclusion coïncidant avec celle de la période de rodage de cette étude
- Sujets placés sous protection judiciaire
- - Sujets souffrant de problèmes physiques ou psychologiques graves et dont l'état peut, selon l'investigateur, affecter l'observance de l'étude

Comparaison de deux produits utilisés pour le nettoyage des fosses nasales, à raison de 4 lavages/jour (240mL par lavage), en soins postopératoires locaux suite à une chirurgie endoscopique des sinus. Cette chirurgie, connue sous le nom d'ethmoïdectomie, vise à améliorer la ventilation des sinus, à traiter la zone infectée de manière chronique et à éliminer les polypes qui causent l'obstruction nasale. Des informations détaillées concernant le but de l'irrigation nasale, ainsi que la façon d'utiliser et de nettoyer le dispositif, seront données aux patients lors de l'inclusion.
