Détection précoce de cinq cancers courants à l'aide du test d'analyse de l'ADNct (K-DETEK)
Évaluation d'un nouveau test sanguin dans la détection précoce des cinq cancers courants basée sur l'étude de l'ADN tumoral circulant
Il s'agit d'une étude de cohorte prospective multicentrique, visant à évaluer un test sanguin dans la détection précoce des quatre cancers courants, basé sur l'investigation de l'ADN tumoral circulant (ctDNA).
Objectif principal : Évaluer les caractéristiques de performance du test ADNct sanguin dans la détection précoce des cancers.
Objectifs secondaires :
- Évaluer les caractéristiques de performance du test pour déterminer l'origine de la tumeur, par rapport aux résultats des tests diagnostiques d'imagerie.
- Déterminer le risque de développement de cancers dans la population à haut risque, par rapport à celui du groupe à risque modéré.

Description détaillée
Il s'agit d'une étude de cohorte prospective multicentrique recrutant des participants âgés de 40 ans ou plus, qui se présentent à la clinique externe des hôpitaux de l'étude soit pour des visites de suivi périodiques pour leurs maladies chroniques, soit pour un bilan de santé annuel. L'étude sera menée à l'Institut de génétique médicale, en collaboration avec l'Université de médecine et de pharmacie d'Ho Chi Minh-Ville, au Vietnam, et d'autres hôpitaux collaborateurs.
Les participants potentiels, qui satisfont à tous les critères d'inclusion/exclusion seront recrutés dans cette étude. Ces participants potentiels ne doivent pas avoir d'antécédents de cancer avant d'être inscrits à l'étude.
Lors de l'inscription, chaque participant répondra à un questionnaire préconçu pour fournir des informations démographiques, des antécédents médicaux majeurs et des signes avant-coureurs cliniques de cancers, puis fournira 10 ml de sang pour analyser l'ADN tumoral circulant (ctDNA).
Chaque participant recevra soit ctDNA détecté, soit ctDNA non détecté.
Pour ceux qui ont un ctDNA détecté dans leur sang, ils subiront ensuite les tests de dépistage et de diagnostic des cancers basés sur la prédiction de l'origine de la tumeur fournie dans le rapport des résultats des tests. Si les cancers sont confirmés en laboratoire, les patients seront alors suivis et pris en charge pour leur maladie conformément aux directives du ministère vietnamien de la Santé et/ou aux directives internationales sur les cancers. A 12 mois (+/- 1 semaine) suivant l'inscription, ces patients seront contactés pour recueillir des informations sur leur état de santé en général et leur statut cancéreux en particulier.
Pour ceux dont l'ADNct a été détecté dans leur sang, mais la présence d'une masse à l'intérieur du corps des participants n'a pas pu être détectée à l'aide des tests de diagnostic par imagerie ; et pour ceux qui n'ont pas d'ADNct présent dans le sang, des appels téléphoniques seront effectués 6 mois (+/- 1 semaine) et 12 mois (+/- 1 semaine) après l'inscription pour recueillir des informations relatives à la santé des participants statut ainsi que la progression du cancer (si possible).
L'inscription devrait durer environ 6 mois.

- Âgé de 40 ans ou plus à l'inscription
- Aucune suspicion clinique de cancer ni antécédent de cancer confirmé n'a été rapporté
- Accepter d'être contacté 6 mois et 12 mois après l'inscription pour recueillir des informations sur l'état de santé général et l'évolution du cancer (si possible)
- Fournir un consentement éclairé écrit
Critères d'exclusion : Si un participant potentiel répond à l'un des critères suivants, il ne pourra pas être inscrit à l'étude :
- Antécédents de cancer (le cancer a été confirmé ou traité au cours des 3 dernières années)
- Antécédents de transfusion sanguine ou de greffe de moelle osseuse au cours des 3 dernières années
- N'accepte pas de participer à l'étude
- Le sujet présente des manifestations cliniques de démence.
