Rilevazione precoce di cinque tumori comuni utilizzando il test di analisi del ctDNA (K-DETEK)
Valutazione di un nuovo esame del sangue per la diagnosi precoce dei cinque tumori comuni basato sull'analisi del DNA tumorale circolante
Si tratta di uno studio di coorte prospettico multicentrico, che mira a valutare un esame del sangue nella diagnosi precoce dei quattro tumori comuni, basato sull'indagine del DNA tumorale circolante (ctDNA).
Obiettivo primario: valutare le caratteristiche prestazionali del test del ctDNA nel sangue nella diagnosi precoce dei tumori.
Obiettivi secondari:
- Valutare le caratteristiche prestazionali del test nel determinare l'origine del tumore, rispetto ai risultati dei test diagnostici per immagini.
- Determinare il rischio di sviluppo di tumori nella popolazione ad alto rischio, rispetto a quella nel gruppo a rischio moderato.

Si tratta di uno studio di coorte prospettico multicentrico che recluta partecipanti di età pari o superiore a 40 anni, che si presentano presso l'ambulatorio degli ospedali dello studio per visite periodiche di follow-up per le loro condizioni croniche o per controlli sanitari annuali. Lo studio sarà condotto presso il Medical Genetic Institute, in collaborazione con l'Università di Medicina e Farmacia di Ho Chi Minh City, Vietnam e altri ospedali che collaborano.
I potenziali partecipanti, che soddisfano tutti i criteri di inclusione/esclusione saranno reclutati in questo studio. Questi potenziali partecipanti non dovrebbero avere una storia di cancro prima di essere arruolati nello studio.
Al momento dell'iscrizione, ogni partecipante risponderà a un questionario pre-progettato per fornire informazioni demografiche, anamnesi medica passata e segni premonitori clinici di tumori, quindi fornirà 10 ml di sangue per l'analisi del DNA tumorale circolante (ctDNA).
Ogni partecipante riceverà il ctDNA rilevato o il ctDNA non rilevato.
Per coloro che hanno il ctDNA rilevato nel sangue, saranno quindi sottoposti ai test di screening e diagnostici per i tumori basati sulla previsione dell'origine del tumore fornita nel rapporto sui risultati del test. Se i tumori sono confermati in laboratorio, i pazienti verranno seguiti e gestiti per la loro malattia secondo le linee guida del Ministero della Salute del Vietnam e/o le linee guida internazionali sui tumori. A 12 mesi (+/- 1 settimana) dall'arruolamento, questi pazienti saranno contattati per raccogliere informazioni sulla loro salute, in generale, e sul loro stato oncologico, in particolare.
Per coloro che hanno rilevato il ctDNA nel sangue ma non è stato possibile rilevare la presenza di una massa all'interno del corpo dei partecipanti utilizzando i test diagnostici per immagini; e per coloro che non hanno ctDNA presente nel sangue, le telefonate verranno effettuate a 6 mesi (+/- 1 settimana) e 12 mesi (+/- 1 settimana) dopo l'arruolamento per raccogliere informazioni relative alla salute dei partecipanti stato così come il progresso del cancro (se possibile).
L'iscrizione è prevista per una durata di circa 6 mesi.

- 40 anni o più al momento dell'iscrizione
- Non sono stati segnalati né il sospetto clinico di cancro né la storia di cancro confermato
- Accetta di essere contattato a 6 mesi e 12 mesi dopo l'iscrizione per raccogliere informazioni sullo stato di salute generale e sull'andamento del cancro (se possibile)
- Fornire il consenso informato scritto
Criteri di esclusione: se un potenziale partecipante soddisfa uno dei seguenti criteri, non potrà essere arruolato nello studio:
- Storia di cancro (il cancro è stato confermato o è stato trattato negli ultimi 3 anni)
- Storia di trasfusioni di sangue o trapianto di midollo osseo negli ultimi 3 anni
- Non accetta di partecipare allo studio
- Il soggetto presenta manifestazioni cliniche di demenza.
