滲出性中耳炎 (OME) は、急性炎症がなくても中耳腔に非化膿性液体が蓄積することを特徴とする状態です。 OME は年間 220 万回診断されています (米国 保健社会福祉省、2000 年)。 これは、子供の 50% 以上が生後 1 年以内に経験する状態です (国立衛生研究所、1993 年)。 多くのエピソードは自然に解決しますが、30% から 40% は持続し、エピソードの 5% から 10% は 1 年以上続きます (Yoshinaga-Itano, 1995)。 1 歳から 3 歳までの子供で、上気道感染症の有病率が高い季節に特によく見られます。 10% から 30% の発生率で。 7 歳までに頻繁に発生し、有病率は 3% から 8% です (Fiellau et al、1997; Fiellau et al、1983; Lous et al、1981; Teele et al、1989)。

- 1 歳から 17 歳までの子供は、McMaster 耳鼻咽喉科クリニックに紹介されました。
-少なくとも3か月間の両側性OMEの存在:
- 病歴:患者は、聴力の低下、耳閉感および/または耳圧、バランスの問題、耳を引っ張る/こするなどの症状を呈する場合がありますが、通常、耳痛や発熱はありません。
- 空気圧耳鏡検査: OME を示唆する観察には、非化膿性浸出液 (漿液性または粘液性) の存在を伴う鈍い鼓膜の存在、一定レベルの浸出液の存在、鼓膜の減少または非運動性、鼓膜の退縮が含まれます。
- ティンパノメトリー:外耳道の空気量が正常なタイプ B またはタイプ C のティンパノグラム。
- 純音聴力検査:典型的には軽度から中等度まで変化する伝音難聴。
除外基準:
-病歴および身体診察によって決定される急性中耳炎の存在:
- 重大な耳痛、聴力の低下、発熱、過敏症の急速な急性発症の病歴。
- 化膿性滲出液、鼓膜の血管過多を伴う黄色および/または発赤、正常なランドマークの減少を伴う鼓膜の膨らみを明らかにする空気圧耳鏡検査。
- 頭蓋顔面異常の存在
- 中耳手術の既往(鼓膜切開およびチューブを除く)
- ランソプラゾールに対するアレルギー反応、およびランソプラゾールに対するその他の有害な薬物相互作用。

ランソプラゾールの投与量は、ランソプラゾールの公式添付文書に記載されている処方薬情報によって定められたガイドラインに基づいて投与され、3 か月間は各患者で同じままであり、次のとおりです。 a) 1 歳から 11 歳、体重が 30 kg 以下、1 日 1 回 15 mg を経口投与。
b) 1 歳から 11 歳、体重が 30 kg を超える、1 日 1 回経口で 30 mg。
c) 12 歳以上、1 日 1 回 15 mg 経口。
ランソプラゾールの投与量は、ハミルトン ヘルス サイエンスの入院薬局で液体の形で調製されます。
