思春期マスタープロトコル (AMP)
母親から新生児への HIV 感染を防ぐ治療の進歩は画期的なものでした。 その結果、現在、米国で新たに周産期に感染する子供の数は少なくなっています。 その後の HIV 感染乳児および小児の治療改善も同様に目覚ましく、これまでに感染した米国人の小児のほとんどが生き残り、思春期に差し掛かっていることが確認されました。 さらに、世界中で HIV に感染した青少年の数は、資源に乏しい国と医療レベルが向上している国の両方で大幅に増加しています。 したがって、出生時から HIV 感染とともに生きており、思春期まで老化している子どもたちの世界的なコホートが存在します。 HIV 感染とその治療がこれらの小児の成熟過程に及ぼす影響については、明確にわかっていることはほとんどありません。

AMP の主な目的は次のとおりです。
- 周産期 HIV 感染症に罹患した思春期前児童および青年期が、思春期を経て成人期に移行する際の認知的、学術的、社会的発達に加え、成長と思春期の発育 (およびそのホルモン調節) に対する HIV 感染と ART の影響を定義すること。
- 抗レトロウイルス療法 (ART) の毒性を含む、HIV 疾患の感染性および非感染性合併症を特定する。
調べる:
- 服薬遵守、家族および社会的機能、危険な性行動、合法および違法の薬物使用、アルコール使用などの高リスク行動を含む、時間の経過に伴う認知および行動の変化。
- 言語と聴覚の変化。
- グルコース代謝、身体組成、骨の石灰化の変化。
- 脂質代謝の変化および心血管疾患のその他の危険因子。
- HIV の二次感染の危険因子。そして
- 女性における子宮頸部 HPV 感染症の発生と臨床経過。
AMP のドメイン固有の目的は次のとおりです。
- 成長と性的成熟: HIV に曝露されているが感染していない小児と比較した場合、HIV に感染した小児の成長と性的成熟、および成長と成熟に影響を与える要因を長期的に追跡します。
- 心血管疾患の代謝危険因子: 異常なグルコース代謝、脂質異常、体組成、および心血管疾患のその他の危険因子の出現を特徴づけ、HIV に曝露されているが感染していない小児と比較した場合の HIV 感染小児における寄与する影響を特定する。
- 心臓機能: HIV に感染しているが感染していない小児と比較した場合の、HIV に感染した小児および若者における心臓の構造的および機能的異常の有病率を推定します。
- 骨ミネラル密度: HIV に感染しているが感染していない小児と比較した場合の HIV 感染小児の骨ミネラル密度の違いを推定し、異常な骨石灰化に寄与する要因を特定します。
神経学的、神経発達、言語、および行動機能:
- 危険な性的行動、合法的および違法な薬物使用、アルコール使用などの高リスク行動、神経発達障害、学業成績など、HIV に感染した児童および青少年の認知的および行動的結果を調査し、HIV に曝露されているが感染していない児童および青少年と比較する。コントロールコホート。
- ARTを受けているHIV感染児における抗レトロウイルス療法の不遵守と不遵守の予測因子を調査する。
- 感情的および行動的問題に関連する家族および心理社会的要因を調査する。
青少年の婦人科と性感染症:
- STI/膣感染症の発生率と危険因子を評価する (C. 男性と女性の場合は、トラコマティス、淋菌、膣トリコ状菌、梅毒、生殖器疣贅、HPV、および HSV)、さらに女性の場合は細菌性膣炎が含まれます。

包含基準:
- 医療記録に記録されている周産期の HIV 感染。
- 年齢 7 歳(7 歳の誕生日)から入学時の 16 歳の誕生日を除く。
- 介護に従事しART歴がございます。
- いずれか: プロトコールに指定されているように、AMP への登録を許可する承認済み研究のリストに含まれるいずれかの研究への以前または現在の登録。 他の研究に参加している小児は、プロトコールチームの承認を得て登録することができます。 追加の承認されたプロトコルは PHACS Web サイトにリストされています。または: 出生以降の入手可能な医療記録文書 1) ART 曝露歴 2) 日和見感染 (OI) 予防曝露歴 3) ウイルス量および CD4 数の履歴 4) 主要な医療事象の履歴

除外基準:医療記録に記載されている、母子感染以外(血液製剤、性的接触、点滴薬物の使用など)によって感染したHIV。

- 医療記録に記載されているように、HIV に感染しておらず、HIV に感染している母親から生まれた。
