提案された研究は、高リスク切除可能 NSCLC 患者におけるニボルマブ +/- イピリムマブの術前投与の安全性と実現可能性を評価し、これらの患者の腫瘍免疫環境と循環免疫細胞および可溶性因子の包括的な探索的特性評価を促進します。 この研究で得られたデータは、NSCLC における抗 PD-1 およびその他の免疫療法のさらなる前向き臨床試験を計画するための貴重な情報を提供します。 最終的には、効果を得る可能性が最も高い NSCLC 患者が抗 PD-1 治療を受けられるようにし、逆に、抗 PD-1 治療を受ける患者の毒性と無効な治療のリスクを最小限に抑えるために、応答と毒性の有望なバイオマーカーを発見することが非常に望ましい。恩恵を受ける可能性は低い。
さらに、この研究の修正により、切除可能な NSCLC の治療のためのネオアジュバント設定におけるニボルマブと抗 CTLA4 抗体であるイピリムマブの併用の評価が可能になります。 大規模なマルチコホートの第 1 相試験では、PD-L1 ステータスによって選択されていない患者におけるイピリムマブとニボルマブの併用療法の ORR は 39 ~ 47% の範囲でした。 グレード 3 ~ 4 の毒性の発生率は、イピリムマブとニボルマブの併用コホート全体で 33 ~ 37% の範囲であり、この疾患設定でのプラチナ ダブレット化学療法による毒性の発生率と比べても遜色ありません。

-組織学的に証明された非小細胞肺癌(コア生検が必要)。
- 扁平上皮または非扁平上皮の組織学
- 診断用コア生検標本は、SKCCC または MSKCC の教員病理学者によって審査される必要があります
- バイオマーカーの評価には、ホルマリン固定パラフィン ブロックまたは腫瘍生検サンプルの 5 ミクロンの組織切片 (スライド) が最低 15 枚必要です (研究病理学者は、サンプリングの妥当性を確認する必要があります)。 これは、アーカイブされた組織から、または必要に応じて新しい生検から取得できます。

除外基準:
- アクティブな、既知の、または疑われる自己免疫疾患がある場合、被験者は除外されます。 白斑、I型糖尿病、ホルモン補充のみを必要とする自己免疫状態による残存甲状腺機能低下症、全身治療を必要としない乾癬、または外部トリガーがなければ再発が予想されない状態がある場合、被験者は登録を許可されます。
- 治験薬投与から 14 日以内に、コルチコステロイド(1 日あたり 10 mg を超えるプレドニゾン相当量)またはその他の免疫抑制薬による全身治療が必要な状態にある場合、被験者は除外されます。 吸入または局所ステロイドおよび副腎置換用量 > 10 mg の毎日のプレドニゾン相当量は、アクティブな自己免疫疾患がない場合に許可されます。 ニボルマブまたはニボルマブ/イピリムマブの併用による肝毒性の可能性があるため、ニボルマブを含むレジメンで治療されている患者では、肝毒性の素因を持つ薬物を慎重に使用する必要があります。
