研究者らは、持続性またはコントロールされていない喘息を患う2歳から12歳までの5,000人以上の子供たちを対象としたブロンクスの20の診療所において、介入と強化通常ケア(eUC)を比較するクラスターランダム化試験を実施する予定である。 11 の eUC 実践はガイドライン情報を受け取り、子供の喘息の重症度とコントロールを評価しますが、積極的な介入要素は提供されません。 実践者は、4 年間にわたる 4 つのウェーブに分けて研究に参加します (年間 4 ~ 6 回の実践)。 医療提供者によるガイドラインの採用と、介入および eUC 実践による持続性喘息またはコントロール不良喘息の 2 ~ 12 歳のすべての患者 (約 5,000 人) に対するケアの利用状況は、電子医療記録 (EHR) データと喘息の重症度および実践に基づくスクリーニングを使用して評価され、コントロール。 研究者らはまた、両方の研究群から、持続性またはコントロールされていない喘息を持つ小児の介護者512人のランダムなサブセットを登録し、介護者が報告した小児の罹患率を体系的に評価し、EHRでは利用できない尺度を取得する予定である。

包含基準:
以下にリストされた基準は、eMPI および eUC の実践に参加する約 5,000 人の子供たちに適用されます。
- 医師が喘息と診断した(EHRに基づく)。
- クリニックの評価による持続性または制御されていない喘息。 NHLBI ガイドラインに基づき、以下のいずれか: 過去 1 か月間、喘息の症状が週に 2 日を超える、救急薬の使用が週に 2 日を超える、夜間の症状が月に 2 日を超える、または過去に喘息の症状が 2 日を超える全身性コルチコステロイドが必要になった年。
- 対象年齢は 2 歳から 12 歳までです。
追加の包含基準は、養育者が報告した罹患率を得るために養育者と面談される小児 512 人のサブセットに適用されます。
- 介護者は英語またはスペイン語を話し、理解することができます。 すべての調査は研究担当者によって口頭で実施されるため、文字を読むことができない参加者も対象となります。
- 主な介護者の同意、子供の参加に対する介護者の許可、および子供(7 歳以上)の同意。 資格のある兄弟がいる場合、子供は 1 人だけがランダムに選択されます。
除外基準:
- この子供は、先天性心疾患、嚢胞性線維症、その他の慢性肺疾患など、喘息関連の対策の評価を妨げる可能性のある他の重大な病状を患っています。

実験的:強化された多面的プロンプト介入 (eMPI)
強化された MPI プログラム (eMPI) は、医療提供者が効果的かつ効率的に喘息治療ガイドラインを導入できるようにする革新的なマルチレベルおよびチームベースの戦略で構成されています。
eMPI は、診療所訪問時にガイドラインに基づいたプロンプトを使用して医療提供者の意思決定をサポートし、喘息管理を改善するために医療提供者が是正措置 (予防薬の処方など) を推奨する可能性を高めます。
