Fremme retningslinjer og behandling av astma gjennom teknologibasert intervensjon og omsorgskoordinering (PRAGMATIC)

Detaljert beskrivelse
Etterforskere vil gjennomføre en randomisert klyngestudie som sammenligner intervensjonen med forbedret vanlig omsorg (eUC) i 20 Bronx-praksiser som betjener over 5000 barn i alderen 2-12 år med vedvarende eller ukontrollert astma. Elleve eUC-praksiser vil motta veiledende informasjon og vurdere alvorlighetsgrad og kontroll av astma hos barn, men aktive intervensjonskomponenter vil ikke bli gitt. Praksis vil bli med i studiet i 4 bølger over 4 år (4-6 praksiser per år). Leverandørens vedtak av retningslinjer og bruk av omsorg hos alle pasienter (~5000) i alderen 2-12 år med vedvarende eller ukontrollert astma fra intervensjon og eUC-praksis vil bli evaluert ved hjelp av elektroniske helsejournaler (EPJ)-data og praksisbasert screening for astmaens alvorlighetsgrad og kontroll. Etterforskere vil også registrere en tilfeldig undergruppe av 512 omsorgspersoner for barn med vedvarende/ukontrollert astma fra begge studiearmene for systematisk å evaluere omsorgsgiverrapporterte barnesykelighetsutfall og oppnå tiltak som ikke er tilgjengelige i EPJ.
Intervensjon består av akademisk detaljering i EPJ som følger nasjonale astmaretningslinjer og koordinering av oppsøkende arbeidstakeromsorg for pasienter med vedvarende eller ukontrollert astma.

Inklusjonskriterier:
Kriteriene som er oppført nedenfor vil gjelde for ~5000 barn fra eMPI og eUC praksis:
- Legediagnostisert astma (basert på EPJ).
- Vedvarende eller ukontrollert astma i henhold til klinikkvurdering. Basert på NHLBI-retningslinjer, ett av følgende: i siste måned, > 2 dager/uke med astmasymptomer, >2 dager/uke med redningsmedisinbruk, >2 dager/måned med nattesymptomer, eller > 2 episoder i det siste år som krevde systemiske kortikosteroider.
- Alder 2 og 12 år inkludert.
Ytterligere inklusjonskriterier vil gjelde for en undergruppe av 512 barn hvis omsorgspersoner vil bli intervjuet for å få omsorgsrapporterte sykelighetsutfall:
- Omsorgspersonen kan snakke og forstå enten engelsk eller spansk. Deltakere som ikke kan lese vil være kvalifisert ettersom alle undersøkelser vil bli administrert muntlig av forskningspersonell.
- Samtykke fra primær omsorgsperson, omsorgsgivers tillatelse til at barnet deltar samt samtykke fra barnet (>7 år). Hvis det er kvalifiserte søsken, vil kun ett barn bli tilfeldig valgt.
Ekskluderingskriterier:
- Barnet har andre betydelige medisinske tilstander, som medfødt hjertesykdom, cystisk fibrose eller annen kronisk lungesykdom, som kan forstyrre vurderingen av astma-relaterte tiltak.
Ytterligere eksklusjonskriterier vil gjelde for en undergruppe av 512 barn/omsorgspersoner som beskrevet ovenfor:
- Ingen tilgang til telefon for å gjennomføre oppfølgingsundersøkelser.
- Barn i fosterhjem eller andre situasjoner der samtykke ikke kan innhentes fra verge.

Eksperimentell: Enhanced Multifaceted Prompting Intervention (eMPI)
Det forbedrede MPI-programmet (eMPI) består av innovative flernivå- og teambaserte strategier for å gjøre det mulig for tilbydere å effektivt vedta retningslinjer for astmabehandling.
eMPI bruker retningslinjebaserte forespørsler på tidspunktet for et kontorbesøk for å støtte leverandørenes beslutningstaking, noe som øker sannsynligheten for at de vil anbefale korrigerende tiltak (dvs. reseptbelagte forebyggende medisiner) for å forbedre astmabehandlingen.

Ingen inngripen: Praksis for Enhanced Usual Care (eUC).
Deltakerne vil motta en gjennomgangspakke av National Asthma Education and Prevention Program (NAEPP) retningslinjer og utdanningsressurser for familier.
Barn vil bli vurdert for alvorlighetsgrad av astma og kontrollnivå ved hvert besøk som en del av best-praksis omsorg, men aktive intervensjonskomponenter vil ikke bli gitt.

Tilgangskriterier for IPD-deling
Tilgang til BDC kontrolleres av NHLBI Data Access Committee (DAC), som bruker databasen med Genotyper og Fenotyper (dbGaP) tillatelsesinfrastruktur. For å få tilgang til data med kontrollert tilgang i BDC, må en etterforsker ha en godkjent datatilgangsforespørsel (DAR) i dbGaP. DAC vil gjennomgå og godkjenne (eller nekte) etterforsker-innsendte DARer for å sikre etterforskerens overholdelse av retningslinjene for datadeling.
