この研究は、顔面中央部のしわの治療におけるDemetech Barbed PDO Sutureの安全性と有効性を評価するための極めて重要な盲検多施設共同研究となる。 包含/除外基準を満たす被験者は、Demetech 有刺 PDO 縫合糸による治療を受けます。 57 人の被験者が治療されます。 さらに、研究者が Demetech 有刺 PDO 縫合糸の特性に慣れるために、最大 2 名の慣らし被験者を Demetech 有刺 PDO 縫合糸で治療することができます。 この導入コホートは、すべての研究対象/除外基準を満たす必要があり、非導入コホートと同じ方法で追跡されます。
一次有効性は 6 か月後に評価されます。 すべての被験者は治療後少なくとも 12 か月間追跡調査されます。 被験者は治療を受けてから72時間後に安全性を確保するために電話または電子メールで追跡調査されます。
主要な有効性エンドポイントは、3人の盲検評価者からなるパネルによって評価された、ベースラインと比較して6か月の時点で顔のしわのレンパール分類スケールで1ポイント以上改善した被験者の割合によって決定されます。 レンパールスケールは、盲検パネルによる写真評価に使用されます。 二次有効性エンドポイントの評価には、盲検パネルによる写真および治験責任医師によるライブによるレンパールスケール、治験責任医師および被験者が評価した GAIS、および FACE-Q の使用が含まれます。

説明
包含基準:
- 対象者は22歳以上の男性と女性。
- 被験者は、顔面のしわのレンパールスケール分類に従って、顔面中央部のしわについてPIにより各頬において少なくともクラス1以上の評価を受けました。
- 女性に妊娠の可能性がある(過去 24 か月間無月経でないか、外科的に不妊ではない)場合は、スクリーニング時に尿妊娠検査結果が陰性であることが確認されていなければなりません。 スクリーニング来院時に治療が提供されない場合は、治療の7日以内に尿妊娠検査結果が陰性であることが必要です。
- 妊娠の可能性のある女性の場合、被験者は研究期間中少なくとも 1 つの避妊方法を利用することを確認します。
- 治療担当医師の推奨に従って、中顔面の加齢に伴うしわを修正するために頬のリフトアップを希望する被験者。
- 研究要件を理解できる被験者。研究期間中いかなる時点においても、中顔面に影響を与える他の顔面処置や治療を受けない義務など。
- 研究に参加し、目撃者/インフォームドコンセントフォームに署名する意欲のある被験者。
- すべての研究訪問に論理的に出席でき、すべての研究要件を満たす被験者。
- 30日間の日記を完了する意欲のある被験者。
- ピン刺しおよび/または綿棒による顔面神経評価に合格した被験者。
除外基準:
- 中顔面のどの部位でも 0.4 G のモノフィラメントや綿の切れ端を感じることができない、中顔面の感覚の異常評価。
- 治療部位に活動的なニキビまたは目立つニキビ跡がある。
- 活動的な炎症または感染症。
- プラスチック生体材料に対するアレルギーまたは異物感受性。
- 既知の出血性疾患。
- 授乳中、妊娠中、または妊娠の可能性がある場合は避妊をしていないか、または避妊を希望しない。
- コラーゲン皮膚疾患。
- 既存の顔の非対称性。
- 過去6か月以内の顔面中央部へのデバイスの使用歴(例:レーザー、IPL、高周波、超音波、マイクロニードル、マイクロニードルRF、または冷却ベースの治療)。
- 自己免疫疾患または結合組織疾患の病歴。
- 顔の構造とアンカーポイントを変えるフェイスリフト、ミッドフェイスリフト、ロワーフェイスリフト、またはネックリフトの病歴。
- 中顔面への脂肪移動の病歴。
- ヒアルロン酸やレディエッセなどの一時的なフィラー以外の、中顔面への注入可能なフィラーの履歴。 永久および半永久的なフィラーは許可されません (PLLA、Sculptra など)。
- 過去 6 か月間の Kybella の履歴。
- 中顔面に顔面外傷の病歴がある。
- 顔面外科手術の歴史。
- 噛んだり、頬を膨らませたり、満面の笑みを浮かべたりすることができない。
- 代謝異常。
- 免疫抑制療法について。
- 最近の体重減少、または研究期間中に大幅な体重減少の意図がある(BMI 2 ポイント)。
- -登録の2年以上前に、顔の美容整形手術(鼻形成術または上部眼瞼形成術を除く)、組織移植、または永久または半永久的なフィラーによる組織の持ち上げまたは増強を受けた被験者。
- 18か月以内にHAベースのフィラーまたはレディエッセによる眼窩縁の下への一時的な顔面真皮フィラー注射、24か月以内にブタベースのコラーゲンフィラー、または中顔面または目尻のラインへの神経調節剤注射、メソセラピー、またはリサーフェシング(レーザー)を受けた被験者、光変調、強力なパルス光、高周波、削皮術、ケミカルピーリング、またはその他のアブレーションまたは非アブレーション処置)研究参加後6か月以内、または研究期間中いつでもこれらの処置のいずれかを受ける予定の患者。
- 顔面中央部の皮膚が非常に薄い人、下まぶたに体液がたまりやすい人、または眼窩下の脂肪体が大きい人。
- 先天性欠損、外傷、全身性リポジストロフィー、部分的リポジストロフィー(バラカー・シモンズ症候群など)、遺伝性疾患、またはHIV関連疾患などの免疫介在性疾患に関連する脂肪組織の異常が原因で、顔面中央部の容積が不足している被験者。
- 創傷治癒に問題を引き起こす可能性のある重篤な併発疾患を患っている被験者(例えば、管理されていない糖尿病または血管疾患)。
- ケロイド瘢痕の問題の家族歴が既知または疑われる被験者。
- 対象が治療または処置後のケアと評価に協力することを妨げる可能性のある重篤な神経学的または心理的状態を患っている対象。
- 喫煙者は傷の治癒を妨げる可能性があるため。
- 過去 3 年以内に顔面に新生物および/または活動性新生物の既往歴がある被験者、または治験責任医師の判断によると、被験者は適切な候補者ではない可能性があります。
- 現在他の臨床試験に登録されている被験者。
- ワルファリンまたは他の抗凝固薬を服用している被験者。 これらの被験者は、術後の出血の問題が発生する重大なリスクを経験する可能性があります。
- 過去2週間以内に、出血のリスクを高める可能性のある薬またはハーブサプリメントをアドリブで摂取した被験者。 これには、出血のリスクを高めることが知られているアスピリンやその他のハーブサプリメントが含まれます。

Improvement From Baseline on the Lemperle Classification Wrinkle Scale

|
The primary effectiveness endpoint is the percentage of subjects with ≥1 point improvement on the Lemperle Classification Facial Wrinkle Scale at 6 months compared to baseline, as rated by a panel of three blinded evaluators.
The median score as rated by the blinded panel using subject photographs will be used to determine if a subject is a responder.
Clinical effectiveness will be demonstrated by superiority of this 6-month response rate over a target rate of 60%

FACE-Q Age Appraisal as Rated by the Subject.

Individual endpoints at each timepoint of 6 weeks, 3, 6, 9, and 12 months for change from baseline on the validated FACE-Q Age Appraisal scales as rated by the Subject. The FACE-Q Age Appraisal Scale is a patient-reported outcome measure that asks patients to select how many years older or younger they think they look compared with their actual age, ranging from -15 years to +15 years in increments of 1, with 0 being 'I look my age' at the center. The subject is instructed to circle only one number, and that number is used in analysis with no further derivation required. Treatment results at each individual timepoint are compared against baseline using a paired t-test. Presumably as subjects would prefer to appear younger than their age, negative values are considered a better result; a mean change from baseline less than zero is considered a favorable response. |

FACE-Q Satisfaction With Cheeks

Individual endpoints at each timepoint of 6 weeks, 3, 6, 9, and 12 months for change from baseline on the validated FACE-Q Satisfaction with Cheeks (FACE-Q Total Score) as rated by the Subject.
The FACE-Q Satisfaction with Cheeks is a patient reported outcome which consists of 5 questions pertaining to different aspects of cheek appearance.
Each is evaluated on a scale of 1 to 4, with 1 being Very Dissatisfied and 4 being Very Satisfied.
The raw scores for items that make up a scale are added to provide a total score.
The total score is converted to a RASCH-transformed score from 0 to 100.
A higher total score is indicative of greater satisfaction with the appearance of their cheeks.
Treatment results at each individual timepoint are compared against baseline using a paired t-test.
