Delphinus SoftVue Prospective Case Collection - ARM 2 (SV PCC ARM2)

SoftVue™는 UST를 사용하는 자동 스캐닝 곡선형 링 어레이 변환기가 있는 전체 유방 초음파 시스템입니다. 현재 FDA 510(k) K123209 및 K142517에 따라 B 모드 초음파 유방 이미징 시스템 및 전송 데이터(음속 및 감쇠)의 컬러 이미징으로 사용하도록 승인되었습니다. SoftVue™는 유방조영술 선별 검사를 대신하여 사용하기 위한 것이 아닙니다.
SoftVue는 비이온화 초음파 에너지를 사용하여 전체 유방의 단층 영상 볼륨을 생성합니다. 환자가 한쪽 가슴을 따뜻한 물에 편안하게 담그고 푹신한 테이블에 엎드려 있는 동안 직경 22cm의 링 모양 변환기가 유방을 둘러싸고 물을 통해 유방 조직으로 저주파 음파를 펄스합니다. . 곡선 변환기의 360도 배열에 있는 2000개 이상의 요소는 초음파 신호를 방출하고 수신하여 흉벽에서 유두까지 모든 방향에서 유방 해부 구조의 에코를 분석합니다. SoftVue는 유방 내의 조직 경계 및 구조에서 반사된 음파로부터 데이터를 캡처할 뿐만 아니라 트랜스듀서가 전체 유방을 둘러싸고 있기 때문에 SoftVue는 유방을 통해 전송되는 신호도 캡처합니다. 이 추가 전송 데이터는 유방 조직의 해부학적 구조의 시각화를 향상시키며 현재 상업적으로 판매되는 다른 유방 초음파 장치에서는 사용할 수 없습니다.
이 전향적, 다기관, 다중 암, 임상 사례 수집 프로그램은 IRB 승인을 받았으며 Good Clinical Practice, 헬싱키 선언 및 모든 적용 가능한 규제 요건을 준수하여 수행됩니다. Arm 2는 총 8개 임상 사이트에서 최대 1,000명의 여성을 집단적으로 등록하는 것을 목표로 합니다. 이 프로토콜에서 Arm 2의 설계는 사례 수집으로 엄격히 제한되며 개입하지 않습니다. 향후 분석을 위한 조사 및/또는 통계 계획은 FDAAA 801의 요구 사항에 적용되는 경우 별도로 준비 및 등록됩니다.
2군은 진단 영상 후 유방 생검(BI-RADS 4 또는 5)이 권장되었거나 영상 소견이 확인된 모든 유방 밀도 구성 범주의 진단 여성 환자 코호트로 제한됩니다. 동일한 환자의 일치하는 진단 영상 및 SV 검사 세트, 인구통계학적 정보 및 임상 결과 데이터가 진단 정밀 검사 중에 수집됩니다. 양성이든 악성이든 모든 유형의 병변에 대한 초음파 특성은 객관적이고 주관적인 유방 밀도 구성 데이터와 함께 수집됩니다. 초음파 이미지 데이터는 21 CFR 812.2(b)에 따라 선별된 임상 현장에서 Delphinus의 품질 관리 시스템(QMS)에 따른 안전 및 품질 평가를 통과하고 설계 변경 관리 표준을 충족하는 수정된 SoftVue 장치로 획득할 수 있습니다. 엔지니어링 및 제품 개발 목적을 위한 타당성, 검증 및 검증 테스트의 일상적인 부분.
이 전향적 사례 수집(PCC) 프로토콜에 축적된 시험 및 임상 데이터는 동료 검토 간행물, 사용자 교육 커리큘럼 개발, 교육 사례 구축 및 SoftVue를 위한 새로운 마케팅 자료 생성을 위해 향후 조사가 설계될 수 있는 데이터베이스를 채울 것입니다.

- 장치 수정(소프트웨어, 하드웨어, 사용자 인터페이스)이 포함된 SoftVue™, 제품 개발의 일상적인 부분으로 타당성, 확인 및 검증 테스트를 완료하기 위해 사람이 사용할 수 있도록 안전성 테스트를 거쳤으며 FDA 21 CFR 812.2(b)에 따라 수행됨, Delphinus Medical Technologies에서 제조.
- FFDM(Full-Field Digital Mammography), 유방 검사 및 진단을 위해 FDA 승인, 다양한 제조업체. (임상적으로 필요한 경우)
- Digital breast tomosynthesis (DBT), 유방 검사 및 진단을 위해 FDA 승인, 다양한 제조업체.

프로젝트 설계 및 절차:
- 잠재적 참가자는 여러 임상 현장에서 진단 평가 또는 생검을 위해 발표하는 여성 그룹에서 모집됩니다.
- 모든 참가자는 의사가 권장한 것과 동일한 표준 치료 진단 평가를 완료합니다.
- 자격이 있는 환자는 예정된 진단 약속 시간에, 바람직하게는 생검 전에 SV 스캔을 받습니다.
- 사이트의 조사관은 SV 이미지를 동반 진단 이미지 및 병리학 결과와 비교합니다.
- 임상 SV 해석은 수행되지 않으며 SV 이미지를 기반으로 임상 개입이 이루어지거나 변경되지 않습니다.
- 모든 평가 결과, 진단 및 치료 결과가 기록됩니다.
- 모든 참가자의 암 상태는 프로그램 시작부터 의사가 권장하는 진단 검사 완료까지 조사됩니다.
- 참가자에게 비악성 유방 생검 소견이 있거나 의심스럽지 않은 진단적 유방 영상 소견이 확인된 경우 사례는 암 음성으로 결정됩니다.
- 병리학적 평가에서 유방암 진단(악성)이 확인되면 사례는 암 양성으로 결정됩니다.
통계 분석:
- 인구 통계학적 특성(연령, 인종 및 민족) 및 기타 기본 특성(예: 프로젝트 부문, 프로젝트 부위, 유방 밀도, 병변별 특성)은 모든 참가자에 대해 표로 작성됩니다.
- 흐름도 또는 표는 부적격 및/또는 평가 불가능에 대한 이유를 포함하여 환자 성향("환자 코호트의 책임")을 설명합니다.
- 등록된 모든 여성에 대한 안전성이 요약됩니다. 예상되는 사건을 제외하고 등록 시점(SoftVue)부터 SoftVue 시험 종료 후 24시간 동안 발생하는 부작용(AE) 및 심각한 부작용(SAE)이 기록됩니다.
- 샘플 크기의 정당성을 포함하는 향후 분석을 위해 별도의 통계 계획이 개발될 것이며 FDAAA 801의 요구 사항에 해당하는 경우 결과를 공유하기 위해 별도로 등록됩니다.
