빅 데이터 및 작은 행동: COVID-19 백신 활용을 촉진하기 위한 1차 진료에 대한 피드백 및 품질 개선 지원
온타리오에서는 약 6,000명의 가정의가 명단에 있는 각 환자의 COVID-19 백신 접종 상태를 나열하는 보고서를 제공하는 안전한 온라인 시스템에 액세스할 수 있습니다.

상세 설명
우리의 목표는 예방 접종을 받지 않은 환자를 효과적으로 치료하기 위해 지원이 가장 필요한 가정의에게 추가 리소스를 제공하는 것입니다. 특히 실습 촉진자는 변경 관리 및 품질 개선 방법론에서 얻은 교훈을 통합하는 참여 방법을 사용합니다.
우리의 진료 촉진자는 다음을 통해 의사를 돕습니다.
- 백신 접종 보고서에 액세스할 수 있도록 기술 지원을 제공하고(또는 대리인을 활성화하여) 사용 가능한 연락처 정보와 함께 여전히 백신 접종이 필요한 환자의 전체 목록을 준비합니다.
- 관리 직원과 함께 아웃리치 계획을 개발하도록 돕습니다. 아웃리치에는 대량 이메일, 문자 메시지, 전화 통화 또는 타운홀 초대가 포함될 수 있습니다. 그들은 백신 관련 질문에 응답할 때 가정의와 직원에게 조언과 자원을 제공할 것입니다. 주어진 진료 장소에 있는 모든 적격 의사(즉, 백신 목록에 접근할 수 있는 의사)는 진료 촉진자의 지원을 통해 혜택을 받을 수 있습니다.
