Vereenvoudigd diagnostisch algoritme voor evaluatie van pasgeborenen met prenataal gedetecteerde hydronefrose

Patiënten met congenitale hydronefrose zullen worden geïdentificeerd door klinische consultaties uitgevoerd door Dr. Assadi voor congenitale hydronefrose en door poliklinische probleemlijsten met congenitale hydronefrose van de bestaande patiënten van Dr. Assadi. Eenmaal geïdentificeerd, wordt de graad van hydronefrose van de patiënt geregistreerd vanaf de eerste echografie op basis van de classificatie van de Society of Foetal Urology (SFU). Als er geen initiële SFU-classificatie is toegewezen aan de hydronefrose, zal een kinderradioloog de eerste echografiebeelden beoordelen en de graad van hydronefrose classificeren op basis van SFU-criteria. De patiënten na evaluatie van hun hydronefrose die plaatsvond, zullen vervolgens worden geregistreerd en geanonimiseerd. De gegevens die moeten worden verzameld, zijn onder meer: echografie van de buik, het retroperitoneum of het niersysteem, leegmakend cysto-urethrogram (VCUG's), nucleair onderzoek: diuretisch verbeterd technetium-99m dietyleentriaminepenta-azijnzuurrenogram (DTPA) of technetium-99m Mercapto -acetyl-triglycine-renogram (MAG3), verwijzing naar pediatrische urologie voor pyeloplastie, serumelektrolyten, bloedureumstikstof (BUN) en creatinine, urineonderzoek, urineweginfecties en plaatsing op langdurige antibiotische profylaxe.
De follow-upevaluatie die voor elke patiënt plaatsvond, zal vervolgens worden vergeleken met ons voorgestelde algoritme voor evaluatie en behandeling van congenitale hydronefrose. Dit algoritme zal worden ontworpen op basis van klinische ervaring, de mening van experts en literatuuronderzoek van elke stap in de evaluatie en behandeling. Primaire uitkomsten zijn progressieve achteruitgang van de nierfunctie en progressie naar verwijzing naar een kinderuroloog voor pyeloplastie. Secundaire uitkomsten zijn de incidentie van urineweginfecties en plaatsing op langdurige antibiotische profylaxe voor profylaxe van urineweginfecties.

Inclusiecriteria:
- Inclusiecriteria voor de studie waren bevestiging van de diagnose van prenatale hydronefrose door middel van postnatale renale echografie
Uitsluitingscriteria:
- voorgeschiedenis van UTI
- diagnose van cystische nierdysplasie met slecht functionerende nier
- eerdere operatie aan het urinewegstelsel
- en andere misvormingen van de uitwendige geslachtsorganen of anomalieën in het cardiopulmonale en centrale zenuwstelsel
