Bekende hypercholesterolemie (FH) is de meest voorkomende genetische aandoening die verband houdt met de ontwikkeling van ernstige en premature coronaire hartziekte (CAD). De aandoening wordt veroorzaakt door mutaties in het gen dat codeert voor de low-density lipoprotein receptor (LDL-r), wat resulteert in een lagere expressie van functioneel LDL-r in de lever. FH heeft autosomaal dominante transmissie met hoge penetrantie en de prevalentie van heterozygote individuen is één op de 400 tot 500 in de algemene bevolking. Tot op heden zijn wereldwijd meer dan 800 verschillende functionele mutaties van het LDL-r-gen beschreven en in Spanje zijn er meer dan 200 gedocumenteerd. Bovendien is een veel minder vaak voorkomende aandoening die op FH lijkt de familiaire defectieve apoB 100-aandoening (FDB) die wordt veroorzaakt door een mutatie in het Apo B 100-gen. FDB is goed voor een aanzienlijk deel van FH in sommige lokale regio's van Spanje.
De levensverwachting wordt met 20 tot 30 jaar verkort bij FH-patiënten, en plotseling overlijden en een hartinfarct zijn de belangrijkste doodsoorzaken. Het Simon Broome Register van FH in Groot-Brittannië heeft aangetoond dat FH een 100-voudige toename in coronaire sterfte en een bijna 10-voudige toename in totale sterfte heeft, vooral bij jonge volwassenen.
Sinds de jaren '90 zijn de coronaire mortaliteit en de totale mortaliteit bij FH-patiënten opmerkelijk gedaald, deels door het gebruik van effectievere lipidenverlagende therapieën zoals statines. De analyse van het Nederlandse FH-cohort toonde aan dat een vroege behandeling met statines na de diagnose van de aandoening leidt tot bijna normalisatie van het risico op coronaire hartziekten vergelijkbaar met de algemene bevolking. Daarom hebben de meeste patiënten een vroege, continue en intensievere lipidenverlagende therapie nodig.
Ondanks het gebruik van statines loopt deze populatie nog steeds een hoog risico op het ontwikkelen van premature CAD. Daarom is de noodzaak om de verwanten van een bekend FH-geval, bekend als cascadescreening, te bestuderen essentieel om die gevallen op te sporen die jonger zijn, waarschijnlijk met een minder ernstige vorm van FH en die geen behandeling krijgen om de ontwikkeling van hart- en vaatziekten te voorkomen.
Hoewel het genetische defect waarschijnlijk de belangrijkste factor is bij de klinische expressie van FH, kunnen andere genetische (gen-gen interacties), omgevingsfactoren (vooral die met betrekking tot voeding, tabaksconsumptie en lichamelijke activiteit) en metabole factoren een belangrijke rol spelen bij het moduleren van de atherosclerotische belasting in deze populatie.
Om inzicht te krijgen in de prognostische factoren en mechanismen die de ontwikkeling van CAD en mortaliteit bij FH beïnvloeden, is een langetermijn prospectieve follow-up van een moleculair goed gedefinieerd FH-cohort met behulp van een multidisciplinaire aanpak noodzakelijk. Dit cohort is een uitstekend hulpmiddel voor translationeel onderzoek om de belangrijkste prognostische factoren met betrekking tot CAD morbiditeit en mortaliteit te evalueren en te bepalen.

Bij inschrijving is een centraal kernlab verantwoordelijk voor de analyse.
Tijdens de follow-up wordt een jaarlijks telefoongesprek gebruikt om contact op te nemen met elke proefpersoon en wordt een volledig lipidenprofiel verkregen van de patiënt (verkregen in zijn/haar gebruikelijke medische zorg).
