在过去的几年中,同步放化疗已成为一种替代疗法。
已发表的伴随放化疗(放疗/顺铂或放疗/顺铂/5-氟尿嘧啶组合)的数据显示,根据疾病阶段,保留膀胱的 5 年局部控制率为 40% 至 65%,总生存概率为 40% 5 年时达到 50%。
为了改善局部和全身预后,评估其他具有更高放射增敏作用的化疗药物,如吉西他滨,是有道理的。

- 肌肉浸润性尿路上皮癌(一线或浅表肿瘤进展后),pT2-pT3 分期,无淋巴损伤 (N0) 且无可检测转移 (M0)。 必须进行最佳宏观切除术 (TURB)
- 肌肉浸润性肿瘤的证据应通过麻醉前不到 8 周的经尿道切除术提供,如果没有,则通过浅表活检和正式影像学检查。 还必须对膀胱进行多次活检。
- 年龄 ≥ 18 岁
- 预期寿命≥6个月
- Kanorfsky 指数 ≥ 70 % (WHO 0, 1, 2)
- 生物学标准:中性粒细胞 ≥ 1500/mm3,血小板 ≥ 100 000/mm3,血红蛋白 ≥ 10 g/dl,肌酐清除率 > 60 ml/mn
- 无远处转移(胸部、腹部和盆腔 CT 扫描、骨扫描)
- 绝经前妇女的有效避孕,在整个治疗期间和放疗完成后长达两个月内维持。
