HINTERGRUND:
Fortschritte in der Molekulargenetik, der genetischen Epidemiologie, der Populationsgenetik und der Identifizierung neuer Risikofaktoren und Risikofaktorcluster für Herz-Kreislauf-Erkrankungen machten 1991 zu einem günstigen Zeitpunkt, um die umfangreichen Informationen über Herz-Kreislauf-Erkrankungen, präklinische Arteriosklerose und Risikofaktoren zu nutzen Personen, die in laufenden, hochmodernen epidemiologischen Studien untersucht wurden, die vom NHLBI unterstützt wurden. Durch die Rekrutierung von Verwandten ersten Grades aus Zufallsstichproben solcher definierten Populationen erhielt FHS Informationen über familiäre Aggregation, genetische und umweltbedingte Beiträge zur Varianz in kontinuierlichen Variablen sowie die Häufigkeit und Verteilung erhöhter Risikofaktoren und ausgewählter wichtiger Gene in der Allgemeinbevölkerung .
Das FHS wurde von Mitarbeitern ins Leben gerufen und im Mai 1990 vom Clinical Applications and Prevention Advisory Committee genehmigt. Die Ausschreibungen wurden im Juli 1991 veröffentlicht. Die Auftragsvergabe erfolgte im Juni 1992.
DESIGN-NARRATIVE:
Während der Phasen I und II von Mai 1992 bis Mai 1996 wurden Probanden im Alter von 45 bis 69 Jahren zusammen mit ihren Verwandten aus der Framingham Heart Study, der Utah Family Tree Study und den Standorten North Carolina und Minnesota der ARIC Study rekrutiert Teilnahme an der Family Heart Study. Es wurden zwei Gruppen von Probanden ausgewählt, entweder nach dem Zufallsprinzip oder aufgrund eines hohen familiären KHK-Risikos, das anhand der Daten der Elternstudie berechnet wurde. Zusätzliche Daten zur Familienstruktur und Krankheitsgeschichte wurden bei 3.150 Probanden und 22.909 ihrer Verwandten erhoben. Klinische Untersuchungen und Nachuntersuchungen dieser zufällig ausgewählten Familien mit hohem KHK-Risiko wurden an insgesamt 1.253 Familien durchgeführt, darunter 5.975 Personen, von denen 102 Familien, darunter 265 Personen, Afroamerikaner waren. Die Untersuchungen umfassten Informationen zu Anthropometrie, Blutdruck, EKG, Karotisultraschall, Lungenfunktion und Blutchemie. Zu den Fragebogendaten gehörten medizinische und reproduktive Vorgeschichte, Ernährung, körperliche Aktivität, Tabak- und Alkoholkonsum, Bildung, Einkommen und psychosoziale Faktoren wie Feindseligkeit, soziale Unterstützung und Stress. Die Phasen I und II umfassten vier Feldzentren, ein Koordinierungszentrum und ein zentrales Blutlabor.
Im August 1996 begann Phase III, als Kooperationsvereinbarungen an ein Konsortium aus sieben von Forschern initiierten Zuschüssen zur Durchführung molekulargenetischer und genetischer Epidemiologiestudien unter Verwendung der in den Phasen I und II gesammelten Daten vergeben wurden. Phase III endete im Juli 2001. Ziel der Phase III war die Durchführung molekulargenetischer und genetischer epidemiologischer Studien unter Verwendung der umfangreichen Daten zu Familien- und Krankengeschichte, Risikofaktoren, Lebensstil, Blutproben und DNA-Banking, die zuvor von der FHS gesammelt wurden. Die Studien umfassten neuartige Molekulargenetik von Kandidatengenen und genomweite Suchen mit anonymen Markern zur Erkennung, Kartierung und Charakterisierung von Genen für koronare Herzkrankheit und Atherosklerose. Es wurden genetische epidemiologische Analysen durchgeführt, die neue Informationen zu den familiären Aspekten von Atherosklerose und intermediären Phänotypen bei Afroamerikanern lieferten. Phase III umfasste außerdem vier Feldzentren, ein Zentrallabor, ein Labor für Molekulargenetik und ein Koordinierungszentrum.
Die Studie wurde 2001 als Family Heart Study – Subclinical Atherosclerosis Network (FHS-SCAN) erneuert, um Analysen genomweiter Scandaten durchzuführen und vielversprechende Marker zu genotypisieren. Die Studie wird voraussichtlich 401 informative Stammbäume (3.027 Personen) umfassen, die zuvor im Rahmen der NHLBI Family Heart Study untersucht und genotypisiert wurden, um das koronare und aortale Kalziumvolumen zu quantifizieren und so mit Atherosklerose assoziierte Gene zu identifizieren. Darüber hinaus werden 315 afroamerikanische Geschwister (770 Personen), die zuvor vom Hypertension Epidemiology Network (HyperGEN) untersucht und vergleichbar genotypisiert wurden, in einem Studienzentrum untersucht, um diese Studienfragen in dieser Minderheitsbevölkerung zu beantworten.
