Innocuité et efficacité de l'hectorol chez les patients pédiatriques atteints d'insuffisance rénale chronique de stade 3 et 4 atteints d'hyperparathyroïdie secondaire pas encore sous dialyse
Une étude ouverte, randomisée et en groupes parallèles pour évaluer l'innocuité et l'efficacité d'Hectorol® (capsules de doxercalciférol) chez des patients pédiatriques atteints d'insuffisance rénale chronique de stades 3 et 4 avec une hyperparathyroïdie secondaire pas encore sous dialyse
Objectif principal:
Évaluer l'effet des capsules Hectorol® sur la réduction des taux élevés d'hormone parathyroïdienne intacte (iPTH).
Objectifs secondaires :
- Évaluer le profil d'innocuité des capsules Hectorol® par rapport aux capsules Rocaltrol® (calcitriol).
- Déterminer le profil pharmacocinétique de la 1,25-dihydroxyvitamine D2 après administration d'Hectorol®.

Critère d'intégration :
- Homme ou femme de 5 à 18 ans.
- Poids ≥15 kg.
- Maladie rénale chronique (MRC) Stade 3 ou 4 sans dialyse, définie comme un débit de filtration glomérulaire (DFG) entre 15 et 59 ml/min/1,73 m^2 (établi par l'équation de Schwartz) lors de la visite de la semaine -2.
- Valeur d'hormone parathyroïdienne intacte (iPTH) > 100 pg/mL pour le stade 3 de l'IRC ou > 160 pg/mL pour le stade 4 de l'IRC, lors de la visite de la semaine -2.
- Formulaire de consentement éclairé signé.
Critère d'exclusion:
- Le patient a un taux sérique de 25-hydroxyvitamine D
- Le patient a un taux de calcium corrigé ≥ 10 mg/dL à la visite de la semaine -2.
- Le patient a un phosphore sérique > 4,5 mg/dL pour les enfants de 13 à 18 ans ; > 5,8 mg/dL pour les enfants de 5 à 12 ans lors de la visite de la semaine -2.
- Le patient devrait nécessiter une hémodialyse d'entretien dans les 3 mois.
- Le patient a utilisé des thérapies au cinacalcet ou à la vitamine D stérol telles que le calcitriol, le doxercalciférol ou le paricalcitol dans les 14 jours précédant la visite de référence.
- Le patient a des antécédents ou une maladie cardiaque symptomatique active dans les 12 mois précédant la visite de référence (semaine 0).
- Le patient souffre actuellement d'une maladie gastro-intestinale chronique (c.-à-d. malabsorption, diarrhée chronique grave, rectocolite hémorragique chronique ou iléostomie).
- Le patient présente actuellement une hyperparathyroïdie primaire ou a subi une parathyroïdectomie totale.
- Le patient a une tumeur maligne active.
- Le patient est incapable d'avaler une capsule de taille similaire aux capsules Hectorol® et Rocaltrol®.
- Le patient a des antécédents de sensibilité ou d'allergie au doxercalciférol, au calcitriol ou à d'autres analogues de la vitamine D.
- Le patient utilise actuellement des liants à base d'aluminium ou de magnésium.

Expérimental: Hectorol
Hectorol (Doxercalciferol) was administered orally two to three times weekly dependent on participant age.
A dose titration scheme was used to individualize the dose to the participant's iPTH management.

Percentage of Participants Who Achieved 2 Consecutive >=30% Reductions in Intact Parathyroid Hormone From Baseline up to Week 12

Blood samples were collected for assessment of iPTH levels.
The percentage of participants meeting the iPTH >=30% reduction from baseline at 2 consecutive study visits up to Week 12 was calculated.
Two consecutive >=30% reductions in iPTH from baseline up to Week 12 was defined as two consecutive 30% or greater reductions at any two consecutive measurements from baseline up to Week 12 with on-treatment strategy applied.
The confidence interval (CI) was estimated using Clopper-Pearson method.
The baseline value is defined as the last available value before the first dose of study treatment.
Percentages are rounded off to the tenth decimal place.

An adverse event (AE) was any untoward medical occurrence in a participant or clinical investigation participant who was administered a pharmaceutical product, and which did not necessarily have a causal relationship with this treatment.
An AE occurred or was detected from the date the participant signed the informed consent form, irrespective of study periods without administration of the study treatment.
TEAEs were defined as the AEs that developed, worsened or became serious during the treatment-emergent period (defined as time from administration of study treatment [Day 1] to last administration of study treatment + 4 days).
SAE: Any untoward medical occurrence that at any dose: resulted in death, was life-threatening, required inpatient hospitalization or prolongation of existing hospitalization, resulted in persistent or significant disability/incapacity, was a congenital anomaly/birth defect, was a medically important event.
