Description détaillée
Le but de ce protocole est de permettre le développement et le test de nouvelles techniques d'IRM adaptées à l'évaluation des patients atteints de maladies cardiovasculaires, de cardiopathies congénitales, de structure cardiaque fœtale, de fonction et de physiologie, et d'autres analyses d'organes fœtaux. Les personnes référées au Children's National Medical Center pour une IRM cardiaque ou un cathétérisme cardiaque cliniquement indiqué par leur cardiologue ou obstétricien référent principal, et qui sont inscrites à cette étude peuvent également subir une imagerie de recherche supplémentaire.
Comme ces développements techniques reposent souvent sur des études préalables chez des volontaires sains, le protocole recrute également des sujets sains.

Population étudiée
Les personnes référées au Children's National Medical Center pour une IRM cardiaque cliniquement indiquée ou un cathétérisme cardiaque par leur cardiologue référent principal seront considérées comme des candidats potentiels à inclure dans la recherche de développement technique.
Les travaux de développement technique reposent souvent sur des études préalables chez des volontaires sains. Ce protocole inscrit également des sujets sains.

Critère d'intégration:
Examens cliniquement indiqués
- Patients qui subissent une IRM médicalement indiquée ou une procédure de cathétérisme cardiaque
- Patients jugés médicalement comme n'ayant aucune contre-indication à l'IRM
- Inclusion d'âge : du nourrisson à l'adulte
- Consentement éclairé écrit et assentiment, le cas échéant
Bénévoles
- Consentement éclairé écrit de tout volontaire normal âgé de 8 ans ou plus pour une IRM cardiaque sans contraste et sans sédation
- Volontaire adulte normal pour subir une IRM cardiaque avec agent de contraste
Examens fœtaux
- Inclusion d'âge : 18 ans et plus
- Deuxième ou troisième trimestre (plus de 13 semaines)
Critère d'exclusion:
Tous les patients qui sont médicalement jugés avoir des contre-indications à l'examen IRM seront exclus. Les contre-indications comprennent les dispositifs métalliques implantés qui sont contre-indiqués pour l'IRM :
- Agrafes d'anévrisme du système nerveux central ;
- Stimulateur neuronal implanté ;
- Stimulateur cardiaque ou défibrillateur implanté ;
- implant cochléaire ;
- Corps étranger oculaire (par ex. copeaux de métal);
- Pompe à insuline implantée ;
- Un éclat de métal ou une balle.
Critères d'exclusion pour l'IRM de stress cycliste
- Infarctus du myocarde dans les 24 heures
- Myocardite active
- Insuffisance cardiaque non contrôlée
- Obstruction sévère des voies d'éjection du ventricule gauche
Critères d'exclusion pour les volontaires adultes subissant une IRM avec administration de produit de contraste :
- 60 ans ou plus
- Maladie rénale; Débit de filtration glomérulaire (eGFR < 30 ml/min/1,73 m2) sera déterminée par une simple prise de sang faite au chevet du patient
- Utilisation de diurétiques
- Diagnostic de l'hypertension et du diabète
Critères d'exclusion pour les volontaires sous contraste et sans contraste
IRM :
Femmes enceintes:
- Les femmes en âge de procréer devront passer un test de grossesse urinaire de dépistage
Critères d'exclusion pour
- Femme enceinte pour qui 15 minutes supplémentaires à plat dans le scanner IRM représenteraient un risque (c.-à-d. claustrophobie sévère, insuffisance cardiaque congestive)
- Enceinte mineure de moins de 18 ans
- Moins de 13 semaines de gestation

Un objectif secondaire de ce protocole est de cataloguer les résultats des études d'imagerie par résonance magnétique qui peuvent être utilisés pour décrire l'histoire naturelle des cardiopathies congénitales et d'autres maladies cardiovasculaires acquises chez les jeunes.
