Centro specializzato di ricerca in malattie polmonari occupazionali e immunologiche
L'obiettivo generale del Centro era quello di sviluppare una banca dati scientifica per il controllo e la prevenzione delle lesioni polmonari acute e croniche causate dall'inalazione di una varietà di agenti sul posto di lavoro e nell'ambiente. C'erano due studi epidemiologici.
Effetti respiratori dell'esposizione a gas irritanti: raccogliere dati sulla funzione polmonare longitudinale e sui sintomi nei lavoratori della produzione chimica che sono stati esposti a sostanze chimiche irritanti come ammoniaca, cloro, fosgene, isocianati, anidride solforosa, protossido di azoto, formaldeide, acidi organici, fluoruri dell'acido cloridrico , altre aldeidi e anidridi acide.
Lavoratori nell'industria tessile del cotone: rilevare e quantificare il rischio di sviluppo di malattie croniche ostruttive delle vie respiratorie che si sono verificate nei lavoratori esposti alla polvere di cotone nella produzione tessile.

I due progetti facevano parte di un Centro Specializzato di Ricerca (SCOR) in Occupational and Immunologic Lung Disease. Fin dalla sua istituzione nel 1972, i ricercatori dello SCOR hanno condotto studi epidemiologici sull'asbestosi, la silicosi nelle sabbiatrici e le reazioni respiratorie nei lavoratori esposti agli enzimi detergenti e ai fumi del film di cloruro di polivinile.
Gas e vapori irritanti sono utilizzati in erbicidi, pesticidi, prodotti farmaceutici, detergenti per la casa, esplosivi, fertilizzanti, refrigerazione, plastica, vernici, fumiganti e una varietà di altri prodotti. Queste sostanze chimiche vengono prodotte e utilizzate immediatamente o spedite agli utenti finali. Il potenziale di incidenti chimici è grande. Un rapporto dell'Environmental Protection Agency stima che dal 1980 negli Stati Uniti si siano verificati 6.928 incidenti chimici, una media di cinque incidenti al giorno. Di conseguenza, 1.500 persone sono rimaste ferite e 135 uccise. Erano necessarie maggiori informazioni sulle conseguenze del danno polmonare acuto subletale indotto chimicamente e sull'esito a lungo termine del danno specifico da vapore/gas chimico al fine di trattare in modo differenziato l'esposizione a queste sostanze chimiche.
La bissinosi è stata riconosciuta come un disturbo respiratorio associato all'esposizione alla polvere di cotone da più di due secoli. Questo disturbo respiratorio è caratterizzato dai sintomi clinici della broncocostrizione con periodicità della settimana lavorativa e successivamente da un declino della funzionalità polmonare durante il turno lavorativo, più marcato dopo un'assenza dal lavoro. Il rischio di bissinosi è probabilmente influenzato dalla durata dell'esposizione e dal tipo di frantoio, come un mulino tessile, un mulino per la frantumazione dell'olio di semi di cotone o un mulino per la sgranatura. La prevalenza sembra non essere influenzata dall'età, dal sesso o dalla razza. Questo studio ha tentato di rispondere alla domanda se l'esposizione a lungo termine alla polvere tessile di cotone abbia portato a compromissione funzionale polmonare clinicamente importante.

Effetti respiratori dell'esposizione a gas e vapori irritanti:
I lavoratori hanno compilato questionari respiratori e sono stati sottoposti a test spirometrici presso i loro stabilimenti al basale con follow-up ogni uno o due anni per un periodo di cinque anni. Le IgE totali iniziali sono state misurate in un campione del 20% per determinare la sua capacità di rilevare i cambiamenti longitudinali nella funzione polmonare e se mediava l'immunoreattività. Nei lavoratori esposti a fuoriuscite di sostanze chimiche, è stato determinato se l'esposizione chimica a breve termine e ad alto livello senza danno polmonare o l'esposizione chimica a breve termine e alto livello con danno polmonare influisce longitudinalmente sulla funzione polmonare o sull'acquisizione di sintomi respiratori. Nei lavoratori infortunati e in un gruppo di controllo abbinato, la spirometria è stata eseguita immediatamente, a due settimane, a sei settimane ea un anno dopo l'episodio. Nella settimana successiva a una lesione acuta, sono state misurate le indicazioni biochimiche circolanti di lesione cellulare e tissutale. Le IgE totali e specificate sono state misurate dopo l'infortunio e confrontate con le misurazioni basali.
Lo studio longitudinale aveva due componenti. Nella prima, i ricercatori di Tulane SCOR hanno raccolto dati sulla funzionalità polmonare, sulla salute respiratoria, immunologici e sull'igiene industriale di 300 lavoratori in nove stabilimenti tessili (sei di cotone e tre sintetici) per confrontarli con i dati dell'azienda. Nella seconda componente, gli investigatori di Tulane hanno lavorato a stretto contatto con l'azienda partecipante per assicurare la qualità dei dati raccolti dall'azienda nel loro programma di sorveglianza della salute respiratoria sui lavoratori nei nove stabilimenti.
Sono stati raccolti dati su sesso, razza, fumo, età, stato atopico, grado di broncocostrizione in risposta all'esposizione alla polvere di cotone misurata dal cambio di turno del lunedì nel FEV1, durata del rapporto di lavoro, esposizione passata e attuale alla polvere di cotone, tipo di lavoro, grado di cotone lavorato, ubicazione geografica della pianta e contaminazione batterica del cotone. I test di funzionalità polmonare sono stati condotti annualmente per cinque anni ottenendo dati spirometrici prima e dopo il turno. I dati erano disponibili sulle misurazioni della polvere dell'azienda negli ultimi dieci anni. Durante lo studio sono state effettuate misurazioni delle polveri per ricostruire un profilo di esposizione completo per i vari titoli di lavoro in ciascuna area di produzione durante il periodo totale di occupazione dei lavoratori partecipanti allo studio.
