Uno studio prospettico longitudinale multicentrico di 3 anni per identificare nuovi endpoint e confrontarli con il volume espiratorio forzato in 1 secondo (FEV1) per la loro capacità di misurare e prevedere la gravità della BPCO e la sua progressione nel tempo
Questo è uno studio longitudinale di 3 anni per identificare nuovi endpoint e confrontarli con misure standard come il volume espiratorio forzato in 1 secondo (FEV1) per la loro capacità di misurare e prevedere la gravità della BPCO (broncopneumopatia cronica ostruttiva) e la sua progressione nel tempo.
I soggetti di controllo (fumatori e non fumatori) saranno reclutati come comparatori con i soggetti con BPCO.

Descrizione
Criterio di inclusione:
- Soggetti con BPCO
- Un soggetto con BPCO sarà idoneo per l'inclusione in questo studio solo se si applicano tutti i seguenti criteri:
- Soggetti maschi o femmine, di età compresa tra 40 e 75 anni
- Un FEV1 al basale (post-broncodilatatore) <80% del valore normale previsto e un rapporto FEV1/FVC al basale (post-broncodilatatore) 70%
- Fumatori attuali o ex con una storia di fumo di almeno 10 pacchetti-anno (numero di pacchetti-anno = (numero di sigarette al giorno / 20) x numero di anni fumati, ad esempio, 20 sigarette al giorno per 10 anni o 10 sigarette al giorno per 20 anni).
- Un consenso informato scritto firmato e datato viene ottenuto prima della partecipazione
- In grado di soddisfare i requisiti del protocollo ed essere disponibile per visite di studio oltre 3 anni
Soggetti di controllo - Attuali/ex fumatori
- Un soggetto di controllo sarà idoneo per l'inclusione in questo studio solo se si applicano tutti i seguenti criteri:
- Soggetti di sesso maschile o femminile, di età compresa tra 40 e 75 anni inclusi, che non presentano malattie significative come determinato dall'anamnesi, dall'esame fisico e dalle indagini di screening
- FEV1 al basale (post-broncodilatatore) >85% del valore normale previsto. Rapporto FEV1/FVC >70%
- Fumatori attuali o ex con una storia di fumo di almeno 10 pacchetti-anno (numero di pacchetti-anno = (numero di sigarette al giorno / 20) x numero di anni fumati, ad esempio, 20 sigarette al giorno per 10 anni o 10 sigarette al giorno per 20 anni).
- Un consenso informato scritto firmato e datato viene ottenuto prima della partecipazione.

- Un soggetto di controllo non fumatore sarà idoneo per l'inclusione in questo studio solo se si applicano tutti i seguenti criteri:
- Soggetti di sesso maschile o femminile, di età compresa tra 40 e 75 anni inclusi, che non presentano malattie significative come determinato dall'anamnesi, dall'esame fisico e dalle indagini di screening
- FEV1 al basale (post-broncodilatatore) >85% del valore normale previsto. Rapporto FEV1/FVC >70%
- Non fumatori con una storia di fumo < 1 pacchetto-anno (numero di pacchetti-anno = (numero di sigarette al giorno/20) x numero di anni fumati).
- Un consenso informato scritto firmato e datato viene ottenuto prima della partecipazione.
- In grado di soddisfare i requisiti del protocollo ed essere disponibile per visite di studio oltre 3 anni
Criteri di esclusione:
Soggetti con BPCO
- Un soggetto con BPCO non sarà idoneo per l'inclusione in questo studio se si applica uno dei seguenti criteri:
- Disturbi respiratori noti o disturbi identificati allo screening/visita 1 (inclusa l'identificazione alla prima TAC), diversi dalla BPCO (ad esempio: cancro ai polmoni, sarcoidosi, tubercolosi, fibrosi polmonare, fibrosi cistica)
- Anamnesi nota di malattia infiammatoria significativa, diversa dalla BPCO (ad es. artrite reumatoide e Lupus)
- Noto per essere gravemente carente di alfa-1-antitripsina (PI SZ o ZZ)
- Dopo aver subito un intervento chirurgico ai polmoni (ad es. riduzione polmonare, trapianto polmonare)
- Avere il cancro o ha avuto il cancro nei 5 anni prima dell'ingresso nello studio
- Malattia grave e incontrollata (inclusi gravi disturbi psicologici) che potrebbe interferire con lo studio o avere un impatto sulla sicurezza dei soggetti
- È arruolato in uno studio farmacologico in cieco a lungo termine (possono essere presi in considerazione soggetti in studi in aperto e soggetti in studi in cieco brevi (possono essere presi in considerazione circa meno di 12 settimane dopo la consultazione con lo sponsor) o uno studio in cui vi è una significativa esposizione alle radiazioni (ad es.: scansioni TC)
- Avere, secondo l'opinione dell'investigatore, prove di abuso di alcol, droghe o solventi
- - Aver ricevuto una trasfusione di sangue nelle 4 settimane precedenti l'inizio dello studio
- Ha avuto una riacutizzazione moderata o grave (che ha richiesto corticosteroidi orali, antibiotici o ricovero in ospedale) nelle ultime 4 settimane. Tutti i cicli di corticosteroidi orali e antibiotici devono essere completati almeno 2 settimane prima dell'inizio dello studio
- È in terapia con corticosteroidi orali a lungo termine (a lungo termine è considerato l'uso per più di 3 mesi consecutivi)
- Impossibile camminare
- Il soggetto è un ricercatore partecipante, un sub-ricercatore, un coordinatore dello studio o un dipendente di un ricercatore partecipante o è un parente stretto del suddetto.
Soggetti di controllo
- Un soggetto di controllo non sarà idoneo per l'inclusione in questo studio se si applica uno dei seguenti criteri:
