この申請は、RFA-AA12-009、HIV/AIDS およびアルコール関連の結果を改善するための介入 (U01) に対応しています。
提案された研究は HIV プライマリケア診療所で行われ、スクリーニングには医療計画の電子医療記録 (EMR) が使用されます。不健康な飲酒とそれに伴う合併症を減らすことで、HIV 感染者に大きな利益をもたらす可能性があります。
これまでの研究では、HIV と不健康な飲酒(閾値を超える飲酒、つまり男性の場合は毎日 4 回以上または週 14 回以上の飲酒、女性の場合は毎日 3 回以上または週 7 回以上の飲酒と定義)が高い割合で併発していることが特定されています。
このようなレベルで飲酒すると、抗レトロウイルス (ART) 治療が損なわれ、薬物使用、うつ病、安全でないセックス、死亡率が増加する可能性があります。
提案されたランダム化試験では、不健康な飲酒を減らすための、適応性のある段階的なケア要素を備えた 2 つの高度に実行可能な行動介入の有効性を比較検討します。 1) 研究臨床医との 1 回の対面セッションと 2 回のセッションで構成される動機づけ面接 (MI)電話セッション。生後6か月で不健康な飲酒を報告した人にはさらに3回の電話セッション。 2) 電子医療記録 (EMR) に統合された安全なメッセージング システムを使用した、アルコール使用のリスクに関する対話型電子メール フィードバック (EF)。生後 6 か月で不健康な飲酒を報告した人には追加の電子メールによるフィードバックが提供されます。
MI と EF は研究スタッフによって提供されます。
3番目の腕は通常のケアのみになります。
3 つの部門すべての参加者は、通常の治療クリニックのスタッフからアルコールスクリーニング、簡単な介入、治療への紹介 (SBIRT) を受ける資格があります。
また、他の同様の医療現場で広く採用される可能性がある 2 つの介入の費用対効果も評価します。
提案されている 2 つの介入、MI と EF は、行動健康および/またはプライマリケアの環境において不健康な飲酒を減らすための有望なアプローチです。
EF はまた、安全なメッセージングを使用しています。これは、HIV 感染者ではなく、アルコール使用を含む問題に対して、他の健康、行動変容、メンタルヘルス治療の現場でテストされている新興テクノロジーです。
この試験では、カイザー・パーマネント・ノーザン・カリフォルニア(KPNC)サンフランシスコから600人の患者(各群200人)が募集される。
NIAAA に基づくスクリーニング質問が EMR に記録され、退役軍人高齢化コホート研究 (VACS) を含む医療利用、ART アドヒアランス、および HIV 臨床転帰に関する包括的なデータが利用できるため、研究対象集団とクリニックはそのような介入を検討するのに理想的です。指数、日常的な臨床検査測定に基づいて最近検証された予後指数。

実験的:モチベーションを高める面接
この介入は、45 分間の対面セッション 1 回と、それに続く 20 分間の電話セッション 2 回で構成されます。最初の電話 MI セッションは、対面セッションの約 10 日後に行われます。
2 回目の通話は、最初の通話から 30 日後に行われます。
同じ研究臨床医が対面セッションと電話セッションの両方を実施します。
電話会議には、最初のセッションで取り上げられた内容のレビュー、アルコール摂取に関する質問、患者の現在のモチベーションレベルに関する自由回答式の質問、アルコール摂取に関する患者の初期目標のレビューが含まれ、所要時間は約 20 分です。
6 か月後に有害な飲酒が認められた場合は、研究臨床医によってさらに 3 回の動機付けの電話面接セッションが提供されます。

実験的:フィードバックを電子メールで送信する
各参加者は、最大 3 件の詳細な電子メールを受け取ります (参加者がアルコール使用のリスクに関する情報を含む最初の最初の電子メールに返信した場合)。
最初とその後の電子メールは長さが短く、危険な飲酒レベル、標準的な飲み物のサイズに関する具体的な情報が含まれます。飲酒を危険でないレベルまで減らすためのアドバイスも提供します。
各電子メールの最後には、患者が必要に応じてさらなる情報や支援を受けるための連絡先番号が記載されています。これには、SU 治療に簡単にアクセスする方法に関する情報が含まれます。そして、参加者が研究臨床医に質問に答えるよう促します。
6 か月後に有害な飲酒が検出された場合、最大 3 通の詳細な電子メールが参加者に配信されます。
