せん妄は頻繁に起こる術後合併症です。系統的レビューでは、術後せん妄が手術患者の 36.8% で発生し、その有病率は年齢とともに増加することが明らかになりました。 せん妄の発生は、機能回復の悪化、認知機能の低下、死亡率の増加など、長期的な転帰の悪化と関連しています。 手術のストレス、痛み、睡眠障害は、高齢者の術後せん妄を引き起こす重要な要因です。
デクスメデトミジンは、呼吸抑制を最小限に抑えながら、抗不安、鎮静、適度な鎮痛を提供する、選択性の高いα-2 アドレナリン受容体アゴニストです。 人工呼吸器を備えた集中治療室 (ICU) の患者にとって、デクスメデトミジン鎮静は睡眠の質を改善し、せん妄の発生率を減少させ、ICU 滞在期間を短縮します。 手術を受ける患者にとって、周術期のデクスメデトミジンはストレス反応の重症度を軽減し、麻酔薬の必要性を軽減し、術後の鎮痛を改善します。
現在の研究の前段階では、心臓以外の手術後にICUに入院した700人の高齢患者が募集され、無作為に2つのグループ(デクスメデトミジングループとプラセボ(生理食塩水)グループ)に分けられた。 その結果、術後早期の予防的低用量デクスメデトミジン注入により主観的睡眠の質が改善され、せん妄の発生率が減少したことが示されました(プラセボ群 22.6% [79/350] 対 デクスメデトミジン群 9.1% [32/350]; オッズ比) 0.35、95% 信頼区間 0.22 ~ 0.54; p < 0.0001)。 研究者らは、低用量のデクスメデトミジン注入もこの患者集団の長期転帰を改善する可能性があると仮説を立てています。
この3年間の追跡調査の目的は、術後早期の予防的低用量デクスメデトミジン注入が、以前のランダム化比較試験で募集された高齢患者の3年間の転帰を改善できるかどうかを評価することである。

説明
包含基準:
以下の基準をすべて満たす患者が含まれます。
- 年齢は65歳以上。
- 全身麻酔下で待機的非心臓手術を受けた。
- 手術後ICUに入院。
除外基準:
以下の基準のいずれかを満たした患者は除外されました。
- -統合失調症、てんかん、パーキンソン病または重症筋無力症の術前病歴;
- 術前にコミュニケーション能力がない(昏睡、重度の認知症、または言語障害のため)。
- 脳損傷または脳神経外科。
- 術前の左心室駆出率<30%、洞不全症候群、重度の洞性徐脈(毎分50拍未満)、またはペースメーカーなしの第2度以上の房室ブロック。
- 重篤な肝機能障害 (Child-Pugh クラス C);
- 重度の腎機能障害(手術前に透析を受けている)。また
- 24時間以上生存する可能性は低い。

実験的:デクスメデトミジングループ
挿管されていない患者には、手術当日の研究募集から手術後初日の午前 8 時まで、デクスメデトミジンを 1 時間あたり 0.1 マイクログラム/kg の速度で注入しました。
挿管され人工呼吸器が装着されている患者については、集中治療室入室後リッチモンド興奮鎮静スケールが-2以上になった後、術後初日の午前8時までにデクスメデトミジン注入を開始した。
