はじめに 筋骨格障害は、世界中で障害を抱えて生きている年数の 2 番目に大きな原因であると報告されており、股関節と膝の変形性関節症 (OA) は最も一般的です (1)。 股関節 OA の診断は、X 線所見と症状の証拠に基づいて行う必要があり (2)、ノルウェーでは 5.8% の有病率が報告されています (3)。 OA は年齢とともに増加し、60 歳以上のすべての人が少なくとも 1 つの関節に変形性関節症の徴候を示します (3)。
OAの病因における支配的な要因は、関節の変形、骨硬化症、被膜の収縮、筋萎縮、およびさまざまな程度の滑膜炎を伴う関節軟骨の喪失です(4)。 身体活動は痛みによって制限され、患者は筋力の低下によりますます体調を崩す傾向があります。 関節内の変化は、腰椎の前弯および胸椎後弯の減少、および骨盤と胴体の非対称性を含む、体の姿勢と筋肉の緊張の代償的調整を伴います (5)。 症状と代償運動の調整は、患者の日常の運動と機能、社会生活と自信に影響を与えます (6)。 過去 20 年間の主要な股関節挿入の大幅な増加は、ノルウェーの関節形成登録簿からのデータによってノルウェーで示されています (7)。 2014 年の年次報告書は、8,099 件の初期人工股関節と 1,284 件の修正を示しており、関節形成術は男性よりも女性でより頻繁に行われています (8)。 補綴コンポーネントの無菌的な緩みが少なくなったため、修正のリスクは年々減少していますが (9)、感染は依然として課題です (10)。
過体重または肥満の影響 過体重または肥満であることは、体重負荷関節における OA の重要な危険因子であることがますます認識されています (11,12)。 しかし、肥満と股関節 OA の発症との関連の証拠は矛盾しています (3,13,14)。 過体重または肥満の人々の体脂肪の減少は、関節の変性に寄与する機械的および生化学的ストレッサーの両方を減少させるために依然として推奨されています (15-17)。
股関節 OA の非外科的管理に関する推奨事項。 股関節 OA の非薬理学的および非外科的介入に関する質の高いエビデンスは不十分ですが (18-21)、臨床ガイドラインの推奨事項はかなり類似しています (19、21、22)。患者教育、ライフスタイルの変化、運動様式、減量、支援技術と適応、履物と仕事の改善などの幅広いトピックと、評価と治療への生物心理社会的アプローチ、および個別化された治療計画 (21)。
患者教育 (PE) 情報とカウンセリングによる患者のエンパワーメントは、最新の保守的な治療計画の重要な要素です。 PE は Klässbo らによってスウェーデンで開発されました。 (23)変形性関節症患者のより良い管理(BOA www.boaregistret.se)と呼ばれる、患者に力を与えることを目指しています。 スウェーデンでは、PE は全国で実施されており、人工股関節全置換術が提供される前に参加が必要です (Socialstyrelsen 2012)。 デンマークでは、PE (デンマークの変形性関節症のグッド ライフ、GLA:D (www.glaid.dk) ) が、6 週間の個別管理された神経筋運動と組み合わされています。 このプログラムの後は、BOA の後よりも痛みに関してさらに良い結果が示されました (24)。 監督下での運動と教育のみの補助効果を調べる研究が現在、重度の股関節 OA 患者に対して行われている (25)。 オスロでは、ActiveA (下肢変形性関節症を伴う活動的な生活) と呼ばれる PE も、股関節および膝の OA 患者に対して試行されています (26)。 ActiveA の原則に基づく PE は、ホルダラン県の股関節および膝の変形性関節症の患者のために、Haukeland University Hospital (HUS) の "Lærings-og mestringssenteret" に設立されました。
股関節 OA の運動療法 股関節 OA の非外科的治療の有益な効果の根底にあるメカニズムはほとんど理解されていません。 関節に関係なく、運動が OA の病因に影響を与えるという証拠はありません (27)。 運動は、OA に冒された関節の周囲の筋肉組織を安定させることによって機能することが示唆されています。 筋力低下は変形性関節症の原因となるため、運動によって病気の進行を止めることができます。 持久力トレーニングにより、患者は減量を管理し、一般的な身体機能を改善できます。 Beckwee等。 (28) は、神経筋、関節周囲、関節内、一般的なフィットネスと健康、および心理社会的要素が、運動による痛みと機能の改善を説明する可能性があると提案しました。およびメタ分析 (18、29-35)。 軽度から中等度の OA の成人における陸上および水中ベースの有酸素運動および強化運動プログラムの有益な効果について強力な証拠が主張されていますが (35)、これは主に短期的な効果に適用されます。 一部の RCT では、PE と従来のエクササイズまたは手技療法を併用した患者と、PE 単独 (36, 37) または偽療法 (38) との長期的な痛みの変化の違いを示すことができませんでした。 Svege等。 (39) しかし、体育と運動を組み合わせると、関節形成術が延期される可能性があることが実証されました。
股関節または膝の OA の治療的運動プログラムは下肢全体の強化に焦点を当てるべきであるという勧告がなされており、体重を支えない運動の使用は疑問視されている (32)。 動的運動制御と機能的安定性を改善することにより、下肢のニュートラルな機能的配置を回復することを目的として、神経筋トレーニングが推奨されます (24)。 身体運動プログラムは主に OA の初期段階の患者に推奨されますが、現在、術前の運動が THA 前後の機能を改善するという中等度の質のエビデンスがあります (40, 41)。
Basic Body Awareness Therapy (BBAT) は、股関節 OA 患者にどのように有益な効果をもたらすのでしょうか?
動作認識学習によって患者に力を与えることは、機能不全の動作をより機能的な動作習慣に変えるために重要であると考えられています (42-44)。 BBAT では、日常生活における動作の質を向上させるための具体的な戦略が使用されます。 股関節 OA などの長期にわたる状態は、人の動きと機能のさまざまな側面に影響を与えます。 BBAT プログラムに統合されているのは、人間の動きの 4 つの視点からの側面です。生体力学的、生理学的、心理社会文化的、実存的、そして運動認識は、運動の質の前提条件と見なされます。 地面と垂直軸に関連する、呼吸を解放する、タスクに必要なエネルギーの適切な量を見つけるなど、人間の動きの基本的な要素が BBAT に実装されています。 動きは、横になっているとき、座っているとき、立っているとき、歩いているとき、そして他の人と相互作用しているときにも行われます(45,46). Basic Body Awareness Therapy セッションの合間に、患者は、a) トレーニング プログラムを自宅で実施すること、および b) 日常生活の状況に質の高い動作を導入することによって、動作を練習するように勧められます。
BBAT はグループ介入として編成され (47, 48)、より最適なバランス、体幹の安定性、自由な呼吸と意識を参加者自身で探し、全身の動きの調整に取り組むことに焦点を当てています。 治療の進行状況の評価は、患者とともに理学療法士によって継続的に行われます。
パイロット研究 2014 年に、患者教育 (2 時間) と基礎身体認識療法 (週 12 回 1.5 時間) に参加する 7 人の股関節 OA 患者のパイロット研究が行われました。 4 か月後、患者 7 人中 4 人が、歩行中の痛みが臨床的に有意に減少したことを示しました。 6 か月後のフォローアップで、5 人の患者のうち 3 人が歩行中の痛みが臨床的に有意に減少したと報告しました (2 人は股関節の問題とは関係なくドロップアウトしました)。 1 人の患者は変化がなく、もう 1 人は悪化していると報告されており、両方とも最小関節腔 = 0 でした。 悪化したと報告した患者のみが、10 か月後に手術の動機を与えられました。
RCT研究の目的:
患者教育単独の効果と患者教育と BBAT を組み合わせた効果を比較することにより、股関節 OA 患者における BBAT の補助効果を調べること。
方法の設計 無作為化対照試験には、2 つの研究群のいずれかに無作為に割り付けられた原発性股関節 OA 患者が含まれます。 1) 患者教育と基礎身体認識療法、または 2) 患者教育のみ。
参加者 検証された股関節 OA の患者 (X 線および臨床症状によって) は、「Lærings-og mestringssenteret」での患者教育に参加するために、プライマリ ヘルス ケアから Haukeland 大学病院の整形外科部門に紹介された患者の中から募集されます。 書面によるインフォームド コンセントは、参加の前提条件です。
サンプルサイズ 数値評価スケールによる痛み (スケール 0-100): グループ間の変化の予想差は、0-100 数値評価スケールで 17 ポイントであり、改善における最小の重要な差と見なされます (53)。 以前の研究 (54) に基づいて、研究者は参加者間の変化の標準偏差を 30 ポイントと仮定しています。 80% の検出力と 0.05 のタイプ I エラーで必要なサンプル サイズは、各グループで 44 です。 15% のドロップアウトを考慮して、合計 100 人の患者が必要です。
HOOS ADL サブスケールによる障害 (0-100): SD=30、検出力=0.80、アルファ= 0.05 (55.100) の HOOS ADL サブスケールで 10 ポイントの臨床的に有意な変化を検出するには、74 人の患者が必要です。 したがって、患者は HOOS の十分なサンプルサイズになります。
無作為化と盲検化: コンピューターで生成されたブロック無作為化スケジュールを使用して、1) 研究に参加するための書面によるインフォームド コンセントが与えられた後、2) アンケートに記入し、盲検化された評価者によってテストされた後、参加者を 2 つのグループのいずれかに割り当てます。 ) PE に参加した。 無作為化手順に関与していない研究コーディネーターが、グループに割り当てられた不透明な封筒を準備します。
介入 患者教育: 患者教育の全体的な焦点は、股関節 OA 状態に関する知識を増やし、それに対処する方法を学ぶことによって、患者のエンパワーメントを行うことです。 患者自身の股関節 OA の経験とグループ内での積極的な共有が最前線に置かれます。 ベーシック ボディ アウェアネス セラピー (BBAT): BBAT はグループ セラピーとして実施されます (48)。 グループには5〜10人の患者が含まれ、BBATの資格を持つ専門の理学療法士が率い、週に1回提供されます。 各セッションは 90 分間続き、グループ メンバーが動きの経験を共有できるように、動きの練習 (70 分) とその後の話し合い (20 分) が含まれます。 参加者は連続してクラスに入学し、12 のセッションに参加します。 このようにして、経験豊富な参加者が経験を共有し、初心者をやる気にさせます。 BBAT は、プライマリ ヘルス ケアのグループ療法として提供されます。
RCT の分析: 無作為化されたグループ間の変化の比較は、ベースライン スコアを制御する線形回帰 (ANCOVA) 分析によって実行されます。 プロトコルごとに、治療意図分析が実行されます。
倫理的配慮:プロジェクトは「ヘルシンキ宣言」に準拠して実施されます。 地域倫理委員会からの承認は、2015 年 9 月に受領されました。
