引言 据报道,肌肉骨骼疾病是导致全球残疾寿命年数的第二大因素,而髋关节和膝关节的骨关节炎 (OA) 最为普遍 (1)。 髋骨关节炎的诊断应基于影像学检查结果和症状证据 (2),据报道挪威的患病率为 5.8% (3)。 OA 随着年龄的增长而增加,每个 60 岁以上的人都至少在一个关节中表现出骨关节炎的迹象 (3)。

患者教育 (PE) 通过信息和咨询赋予患者权力是最新保守治疗计划的重要组成部分。 PE 由 Klässbo 等人在瑞典开发。 (23) 旨在赋予患者权力,称为骨关节炎患者的更好管理 (BOA www.boaregistret.se)。 在瑞典,PE 在全国范围内实施,并且在提供全髋关节置换术之前需要参与(Socialstyrelsen 2012)。 在丹麦,PE(丹麦骨关节炎患者的美好生活,GLA:D (www.glaid.dk))与为期六周的个性化监督神经肌肉锻炼相结合。 与 BOA 相比,该项目在疼痛方面的效果甚至更好 (24)。 目前正在对严重髋骨关节炎患者开展一项研究,检查监督锻炼与单独教育的补充效果 (25)。 一种名为 ActiveA(活跃的下肢骨关节炎患者)的 PE 也已在奥斯陆试用用于髋关节和膝关节 OA 患者 (26)。 基于 ActiveA 原则的 PE 现在已经在 Haukeland 大学医院 (HUS) 的“Lærings- og mestringssenteret”建立,用于治疗来自霍达兰县的髋关节和膝关节骨关节炎患者。
髋关节 OA 的运动疗法 髋关节 OA 的非手术治疗有益效果的潜在机制鲜为人知。 无论关节如何,都没有证据表明运动对 OA 的发病机制有影响 (27)。 建议运动通过稳定受 OA 影响的关节周围的肌肉组织来发挥作用。 由于肌肉无力导致骨关节炎,锻炼可能会阻止疾病的进展。 耐力训练可以使患者控制体重减轻,并改善一般身体机能。 Beckwée 等人。 (28) 提出神经肌肉、关节周围、关节内、一般健身和健康以及心理社会因素可能解释运动引起的疼痛和功能改善。推荐治疗髋关节 OA 运动的科学证据已在几项系统评价中得到检验和荟萃分析 (18, 29-35)。 强有力的证据表明,陆上和水上有氧运动和力量训练计划对患有轻度至中度 OA 的成年人具有有益效果 (35),但这主要适用于短期效果。 一些 RCT 无法显示接受 PE 结合传统锻炼或徒手治疗的患者与单独进行 PE (36, 37) 或假治疗 (38) 的患者在疼痛的长期变化方面存在差异。 Svege 等人。 (39) 然而,证明体育锻炼和锻炼相结合可能会推迟关节置换术。
建议髋关节或膝关节 OA 的治疗性锻炼计划应侧重于加强整个下肢,并且质疑非负重锻炼的使用 (32)。 建议进行神经肌肉训练,旨在通过改善动态运动控制和功能稳定性来恢复下肢的中立功能排列 (24)。 尽管体育锻炼计划主要推荐给 OA 早期患者,但目前有中等质量的证据表明术前锻炼可改善 THA 前后的功能 (40, 41)。
基本身体意识疗法 (BBAT) 如何对髋骨关节炎患者产生有益影响?
通过运动意识学习赋予患者权力被认为对于将功能失调的运动转变为更具功能性的运动习惯很重要 (42-44)。 BBAT 使用具体策略来提高日常生活中的运动质量。 髋骨关节炎等长期病症会影响一个人活动能力和功能的多个方面。 BBAT计划中整合了人体运动的四个方面;生物力学、生理学、心理-社会-文化和存在主义以及运动意识被认为是运动质量的先决条件。 人体运动的基本要素,如与地面和垂直轴相关、自由呼吸和找到任务所需的适当能量,都在 BBAT 中实现。 动作是在躺着、坐着、站着、走路时进行的,也与另一个人互动时进行 (45,46)。 在基本身体意识疗法课程之间,鼓励患者通过 a) 在家中实施训练计划和 b) 在日常生活中实施运动质量来练习运动。
BBAT 以团体干预的形式组织 (47, 48),重点是参与者自己寻找更佳的平衡、核心稳定性、自由呼吸和意识,解决全身运动协调问题。 物理治疗师与患者一起持续评估治疗进展。
试点研究 2014 年对 7 名髋骨关节炎患者进行了试点研究,参与了患者教育(2 小时)和基本身体意识疗法(每周 12 次,每次 1.5 小时)。 四个月后,七名患者中有四名在行走时表现出明显的临床疼痛减轻。 在六个月后的随访中,五名患者中的三名仍然报告行走时临床疼痛明显减轻(两名与髋关节问题无关的中途退出)。 一名患者报告没有变化,一名患者情况更糟,两者的最小关节间隙 = 0。 只有报告病情恶化的患者在 10 个月后才有动力进行手术。
RCT研究的目标:
通过比较单独患者教育与患者教育和 BBAT 结合的效果,检查 BBAT 对髋关节 OA 患者的补充作用。
方法设计 随机对照试验将包括原发性髋骨关节炎患者,随机分配到两个研究组之一; 1) 患者教育和基本身体意识疗法,或 2) 仅患者教育。
参与者 在从初级卫生保健转诊到豪克兰大学医院骨科的患者中招募已证实髋骨关节炎(通过 X 光检查和临床症状)的患者,以参加“Lærings- og mestringssenteret”的患者教育。 书面知情同意是参与的先决条件。
数字评定量表(0-100 分)的样本量疼痛:在 0-100 数字评定量表上,组间变化的预期差异为 17 分,这被认为是改善的最小重要差异 (53)。 根据之前的一项研究 (54),研究人员假设参与者之间的变化标准差为 30 点。 所需的样本量、80% 的功效和 0.05 的 I 类错误是每组 44 个。 考虑到 15% 的退出率,总共需要 100 名患者。
HOOS ADL 子量表 (0-100) 造成的残疾:需要 74 名患者才能检测到 HOOS ADL 子量表上 10 分的临床显着变化,其中 SD=30,功效=0.80 且 alpha=0.05 (55).100 因此,患者将是 HOOS 的足够样本量。

干预 患者教育:患者教育的总体重点是通过增加患者对髋关节 OA 状况的了解并学习如何处理它来增强患者的能力。 患者自己的髋关节 OA 体验和在小组中的积极分享将处于最前沿。 基本身体意识疗法 (BBAT):BBAT 将作为团体疗法实施 (48)。 一个小组将包括 5-10 名患者,由具有 BBAT 资格的专业物理治疗师领导,每周提供一次。 每节课时长90分钟,包括动作练习(70分钟)和讲座(20分钟),让小组成员分享动作经验。 学员连续被录取到班级,参加12届。 通过这种方式,有经验的参与者将分享经验并激励新手。 BBAT 将作为初级保健中的团体疗法提供。
RCT 分析:通过线性回归 (ANCOVA) 分析比较随机化组之间的变化,控制基线分数。 将按照方案和意向治疗分析进行。
