放射状シースが正常に挿入されると、適格な患者は、計画された単回投与グループ(0.75 mg / kg)または計画されたステージンググループのいずれかに、ブロックサイズ4で1:1の比率で連続、連続、ランダムに割り当てられます-用量群 (0.5 ± 0.25 mg/kg)。
最初の用量のエノキサパリンは、無作為化の直後に投与されます。 計画単回投与群では、その後の PCI を受けるかどうかに関係なく、CAG の前にすべての患者にエノキサパリン 0.75 mg/kg が投与されます。 計画的段階的投与群では、エノキサパリン 0.5 mg/kg が CAG の前にすべての患者に投与され、さらに 0.25 mg/kg が PCI の直前に次の PCI を受ける患者にのみ投与されます。
エノキサパリンの初回投与直後にトランスラジアルCAGを実施する。 血管造影の結果が得られた場合、患者がその後の PCI に進むかどうかは、エノキサパリン投与レジメンの割り当てを知らされていない手術者の裁量に委ねられます。 各グループでCAGのみを受ける患者は、それぞれ高用量(0.75 mg / kg)および標準用量(0.5 mg / kg)グループとして定義されます。一方、各グループでCAGとその後のPCIの両方を受ける患者は、それぞれ単回投与PCI(0.75 mg / kg)および段階的投与PCI(0.5 + 0.25 mg / kg)グループとして定義されます。
抗Xaレベルが評価されます: 1) エノキサパリンの初回投与の0分後(直前)、10分後および90分後(それぞれCAG-0分、CAG-10分、およびCAG-90分)すべての患者; 2) PCI を受ける患者では、PCI 開始の 0 分後 (直前) と 10 分後 (それぞれ PCI-0 分と PCI-10 分)、および PCI 終了時 (PCI-End)。
目標抗凝固療法は次のように定義されます: 1) 0.5-1.8 の抗 Xa レベル 高用量群および標準用量群の患者における IU/ml。 2) 抗Xa値0.5~1.2 PCI前のIU/ml、抗Xa値0.5~1.8 単回投与 PCI および段階的投与 PCI 群では、PCI の開始から最初の投与後 90 分までのエノキサパリンの IU/ml。
心虚血および出血イベントに関する情報は、無作為化の24時間後に収集されます。

説明
包含基準:
- 書面によるインフォームドコンセントの提供。
- 18歳以上の男女問わず。
- 文書化された安定冠動脈疾患(SCAD)または非ST上昇型急性冠症候群(NSTE-ACS)。
- その後の PCI の有無にかかわらず、選択的な経橈骨冠動脈造影を受けることを計画します。
- -スクリーニングから7日以内に線溶療法、抗凝固療法、または非経口抗血小板療法を受けていない。
- -スクリーニングから7日以内の心筋トロポニン検査が陰性。
- トランスラジアルアプローチの確立に成功。
- -閉経後> 1年または外科的に無菌の女性。
除外基準:
- -ST上昇型心筋梗塞または非ST上昇型心筋梗塞を含む最近(スクリーニングから30日以内)の急性心筋梗塞。
- インデックス冠動脈造影の目的は、急性冠症候群に対する初回 PCI または早期 PCI を受けることです。
- -冠動脈疾患以外の適応症(例、心房細動、人工心臓弁、静脈血栓塞栓症、心室血栓症など)研究期間中の線溶療法または抗凝固療法。
- -研究期間中のエノキサパリン、アスピリン、クロピドグレル、およびチカグレロールを除く、線維素溶解剤または抗血栓剤の計画された使用。
- -研究期間中に計画された冠動脈バイパス移植(CABG)。
以下を含む出血リスクの増加
- 頭蓋内、眼内、後腹膜、または脊髄出血の病歴;
- 最近(スクリーニングから30日以内)の胃腸(GI)出血;
- 最近(スクリーニングから30日以内)に大きな外傷または大きな手術を受けた;
- 研究期間中の計画された手術またはその他の侵襲的処置;
- -制御されていない高血圧(収縮期血圧[SBP]> 180 mmHgまたは拡張期血圧[DBP]> 100 mmHg)がスクリーニングから7日以内に持続する;
- 出血性疾患の病歴、例えば、血友病、フォン・ヴィレブランド病;
- 研究期間中に非ステロイド性抗炎症薬(NSAID)を中止できない;
