一旦成功插入径向护套,符合条件的患者将以 1:1 的比例连续、顺序和随机分配,块大小为 4 到计划单剂量组 (0.75 mg/kg) 或计划分阶段-剂量组 (0.5 ± 0.25 mg/kg)。
第一剂依诺肝素将在随机分组后立即给药。 在计划的单剂量组中,CAG 前的所有患者将给予依诺肝素 0.75 mg/kg,无论他们是否将接受后续 PCI。 在计划分阶段剂量组中,依诺肝素 0.5 mg/kg 将在 CAG 前给予所有患者,额外的 0.25 mg/kg 仅给予那些将在 PCI 前立即接受后续 PCI 的患者。

心脏缺血和出血事件的信息将在随机分组后 24 小时收集。

描述
纳入标准:
- 提供书面知情同意书。
- 年满 18 岁,男性或女性。
- 有记录的稳定型冠状动脉疾病 (SCAD) 或非 ST 段抬高急性冠状动脉综合征 (NSTE-ACS)。
- 计划接受选择性经桡动脉冠状动脉造影术,有或没有随后的 PCI。
- 筛选后 7 天内未进行纤溶、抗凝或肠外抗血小板治疗。
- 筛选后 7 天内心肌肌钙蛋白检测呈阴性。
- 经桡动脉入路成功建立。
- 绝经后 > 1 年或手术绝育的女性。
排除标准:
- 近期(筛选后 30 天内)急性心肌梗塞,包括 ST 段抬高型心肌梗塞或非 ST 段抬高型心肌梗塞。
- 指数冠状动脉造影的目的是对急性冠状动脉综合征进行直接 PCI 或早期 PCI。
- 在研究期间进行纤溶或抗凝治疗的冠状动脉疾病以外的任何适应症(例如心房颤动、人工心脏瓣膜、静脉血栓栓塞、心室血栓形成等)。
- 在研究期间计划使用任何纤维蛋白溶解剂或抗血栓剂,依诺肝素、阿司匹林、氯吡格雷和替格瑞洛除外。
- 计划在研究期间进行冠状动脉旁路移植术 (CABG)。
出血风险增加,包括
- 任何颅内、眼内、腹膜后或脊髓出血史;
- 最近(筛选后 30 天内)胃肠道 (GI) 出血;
- 最近(筛选后 30 天内)的重大创伤或重大手术;
- 研究期间计划进行的手术或其他侵入性操作;
- 筛选后 7 天内持续不受控制的高血压(收缩压 [SBP] > 180 mmHg 或舒张压 [DBP] > 100 mmHg);
- 出血性疾病史,例如血友病、血管性血友病;
- 在研究期间无法停用非甾体类抗炎药 (NSAIDs);
- 筛选后 7 天内血小板计数低于 100,000/mm3 或血红蛋白 < 10 g/dL。
- 依诺肝素禁忌症,如过敏、活动性出血、出血素质、凝血障碍、急性感染性心内膜炎、血小板减少症(包括肝素诱导的血小板减少症)、脑出血、严重肝肾病、严重高血压、中风、视网膜病变等。
- 依诺肝素不耐受史。
- 根据 Cockcroft-Gault 方程计算,患者需要透析或肌酐清除率 < 30 mL/min:Clcr = (140 - 年龄) × WT / (72 × Scr)(女性为 × 0.85),其中 WT 是以公斤为单位的体重,Scr 是在筛选后 7 天内测量的以 mg/dL 为单位的血清肌酸酐。
