SNIFF デバイス研究では、デバイスを使用して各参加者の鼻または鼻腔内にインスリンを投与します。

血液中のグルコース(糖)レベルを下げることによって作用します。
この研究では、デバイスがどのくらいのインスリンを投与するかを測定しています。
さらに、この研究では、ネブライザーのような装置を使用して鼻腔内に投与されたインスリンまたはプラセボが記憶、血液、脳脊髄液に及ぼす影響を調べる予定です。

この研究の目的は、脳脊髄液 (CSF) 中のインスリンのレベルを増加させる鼻腔内送達デバイスの能力を決定することです。
インスリンが正常な記憶プロセスに役割を果たしており、インスリンの異常がアルツハイマー病(AD)に関連する認知および脳の変化に寄与している可能性があることを示唆する証拠が増えている。 興味深いことに、鼻腔に投与されたインスリンは数分以内に脳に輸送されますが、血糖値やインスリンレベルには影響を与えません。
この研究は、CSFのインスリンレベル、アルツハイマー病バイオマーカー、および記憶に対するINI(20国際単位)またはネブライザーのような装置で送達されるプラセボの急性効果を比較する単一施設のランダム化二重盲検試験で構成されます。 研究参加時に、参加者は最初に急性用量のインスリンまたはプラセボのいずれかを投与され、2回目の訪問で他の物質を投与されるように無作為に割り付けられます。 認知的に正常な参加者、またはaMCIを有する参加者(n=30)が登録されます。 主要評価項目は、ネブライザーのような装置で送達される 20 国際単位用量のインスリン投与後 30 分で CSF インスリンレベルが、プラセボの 30 分後に達成されるレベルと比較して増加するという仮説を検証することです。

- 英語が上手
- 認知機能が正常であるか、aMCIと診断されている
- スクリーニング来院前3か月間安定した病状
- スクリーニングおよび研究訪問前の4週間は安定した薬を服用する
- 臨床検査値は正常範囲内である必要があり、異常な場合は治験医師によって臨床的に重要ではないと判断されなければなりません
除外基準:
- 認知症の診断
- 臨床的に重大な脳卒中の病歴
- 過去2年間のてんかん、意識喪失を伴う頭部外傷、精神病、大うつ病、双極性障害を含む主要な精神疾患の現在の証拠または病歴
- 糖尿病 (I 型または II 型) インスリン依存性および非インスリン依存性糖尿病
- スクリーニング来院から5年以内のインスリンまたはその他の抗糖尿病薬の現在または過去の使用。
- スクリーニングの5年前のがんの病歴(皮膚黒色腫または安定した前立腺がんの病歴は除外されません)
- 過去5年以内の発作歴
- 妊娠または妊娠の可能性。
- 抗凝固薬ワルファリン(クマディン)およびダビガトラン(プラダクサ)の使用
- スクリーニング時に熟練した介護施設に居住していること
- スクリーニング訪問後 2 か月以内の治験薬の使用
- アルコール、麻薬、抗けいれん薬、抗パーキンソン病薬、またはその他の除外薬の定期的な使用
