呼吸窮迫症候群の早産新生児における修正挿管 - 界面活性剤 - 抜管(InSurE)技術の適用
この研究では、早産児の呼吸窮迫症候群 (RDS) の治療における同期鼻間欠的陽圧換気 (SNIPPV) 技術と組み合わせた侵襲性の低いサーファクタント投与 (LISA) を評価します。
変更された InSurE グループには「LISA + SNIPPV」技術が適用され、従来の InSurE グループには挿管、サーファクタント、抜管、および CPAP 技術が適用されます。

包含基準:
- 出生時体重が 2500g 未満で在胎期間が 36+6 週未満の未熟児。
- RDSの初期症状がある高リスクの未熟児、または臨床的にRDSと診断された乳児。
- 参加病院は倫理委員会の同意を得た。
- 親のインフォームドコンセントが得られました。
除外基準:
- 重度の先天性奇形。
- 全身血行動態に影響を与える重度のチアノーゼ性先天性心疾患(大動脈の転位、ファロー四徴症など)。
- 先天性遺伝性代謝疾患。
- 親のインフォームドコンセントは得られませんでした。

PSC は、FiO2 × サポート + 投薬として定義されました。ここで、FiO2 は実際のまたは「効果的な」(鼻カニューレの場合) FiO2 です。サポートは、人工呼吸器の場合は 2.5、鼻持続気道陽圧の場合は 1.5、鼻カニューレまたはフード酸素の場合は 1.0 です。 BPD の全身ステロイドでは 0.20、通常の利尿薬または吸入ステロイドではそれぞれ 0.10、メチルキサンチンまたは断続的利尿薬ではそれぞれ 0.05 です。
スコアの範囲は 0.21 ~ 2.95 です。
