MENA-TAVI 研究は、研究者主導の前向き単群観察試験です。
介入を必要とする重度の大動脈弁狭窄症を患っている、または紹介された患者は、ソース集団を構成します。 インターベンショナル心臓専門医と心臓血管外科医で構成されるハートチームは、利用可能な臨床データ、予測される30日死亡率、フレイルなどの死亡率に影響を与える個々の要因、推定余命、最終的に到達したいという患者の希望を統合することにより、患者を評価します。経カテーテルまたは外科的大動脈弁置換術に関する最適な治療戦略に関するコンセンサス。 TAVIが予定されている患者は、適格性についてスクリーニングされます。 患者がすべての包含を満たし、除外基準を満たさない場合、研究の目的とコースについて通知され、参加と書面によるインフォームドコンセントが求められます。 同意が得られた場合、彼らは ACURATE neo2 大動脈生体プロテーゼによって治療されます。

包含基準:
次の基準がすべて満たされている場合、対象は含まれます。
- -大動脈弁面積(AVA)によって定義される重度の大動脈弁狭窄症の患者 < 1cm2または体表面積(BSA)にインデックス付けされたAVA < 0.6 cm2 / m2、1回拍出量指数(SVI)によって定義される低流量の重度の大動脈弁狭窄症を含む < 35ml /m2、心エコー検査と侵襲的測定の統合によって評価
- -被験者は症候性です(ニューヨーク心臓協会(NYHA)機能クラス> I、狭心症または失神を伴う心不全の症状)
- -患者は、心臓チームによって高リスク(STS-PROM(胸部外科医の予測リスク予測)スコア> 8)または中リスク(STS-PROMスコア> 4)と見なされます。
- 心臓チームは、参加のための患者の適格性と、経大腿アクセスによる TAVR (経カテーテル大動脈弁置換術) (TAVR) が最も適切な治療法であり、患者が最も恩恵を受ける可能性が高いことに同意します。
- 大動脈弁輪の寸法が適切である (面積範囲: 338 ~ 573 mm2 および周囲範囲: 66 ~ 85 mm)。 可能であれば、経食道心エコー検査(TEE)および従来の大動脈造影の所見を解剖学的評価に統合する必要があります。
- -マルチスライスコンピューター断層撮影血管造影法および/または従来の血管造影法によって評価された、最小アクセス血管径が6 mm以上の経大腿アクセスに適した動脈大動脈 - 腸骨 - 大腿軸

除外基準:
次の条件のいずれかが存在する場合、サブジェクトは含まれません。
- 非弁性大動脈弁狭窄症
- 先天性大動脈弁狭窄症または一尖弁または二尖弁の大動脈弁
- 非石灰性後天性大動脈弁狭窄症
- -大動脈輪のサイズ、または天然の大動脈弁の石灰化の程度または偏心、または大動脈または腸骨大腿動脈のねじれのため、経大腿経カテーテル大動脈弁移植に適していない解剖学
- 心原性ショック(低心拍出量、昇圧剤依存、機械的血行動態サポート)の患者を含む緊急処置
- 左心室 (LV) 機能の大幅な低下 (駆出率 < 20%)
- 大動脈位置の既存の人工心臓弁
- 僧帽弁プロテーゼの存在
- -経皮的冠動脈インターベンション(PCI)を除く付随する計画された手順
- -30日以内に計画された心臓以外の手術
- 処置後30日以内の脳卒中。
- -手順の30日以内の心筋梗塞(タイプ2を除く)
- 心臓内の塊、血栓または植生の証拠
- 凝固条件が厳しい
- 抗凝固療法/抗血小板療法に耐えられない
- -活動性細菌性心内膜炎または他の活動性感染症
- 肥大型心筋症
- -造影剤の禁忌またはニチノールに対するアレルギー
- -介入の準備または実施、またはこのプロトコルからの移植後の管理の逸脱につながる別の試験への参加
