상세 설명
MENA-TAVI 연구는 연구자가 시작한 전향적 단일군 관찰 시험입니다.
개입이 필요한 중증 대동맥 협착증이 있는 환자를 의뢰하거나 제시하는 환자가 소스 모집단을 구성합니다. 중재적 심장 전문의와 심혈관 외과 전문의로 구성된 Heart Team은 이용 가능한 임상 데이터, 예측된 30일 사망률, 허약함과 같은 사망률에 영향을 미치는 개별 요인 및 예상 수명 및 환자의 소망을 통합하여 환자를 평가합니다. 경피적 또는 외과적 대동맥 판막 교체와 관련하여 최적의 치료 전략에 대한 합의. TAVI에 대해 계획된 환자는 적격성 검사를 받게 됩니다. 환자가 모든 포함을 충족하고 제외 기준을 충족하지 않는 경우 연구의 목적과 과정에 대해 알리고 참여 및 서면 동의를 요청받게 됩니다. 동의하는 경우 ACURATE neo2 대동맥 생체 인공 삽입물로 치료를 받게 됩니다.
퇴원 시 및 30일에 1차 안전성 종점을 구성하는 임상 결과가 수집됩니다. 추가 임상, 절차 및 심초음파 데이터는 퇴원 시 및 2차 종료점 평가를 위해 30일에 수집됩니다.

포함 기준:
다음 기준을 모두 충족하는 경우 대상이 포함됩니다.
- 대동맥 판막 면적(AVA) < 1cm2 또는 체표면적(BSA)에 대한 AVA 지수 < 0.6 cm2/m2로 정의되는 중증 대동맥 협착증 환자 /m2, 심초음파 및 침습 측정의 통합으로 평가됨
- 피험자는 증상이 있습니다(뉴욕 심장 협회(NYHA) 기능 등급 > I, 협심증 또는 실신의 심부전 증상)
- Heart Team은 환자를 고위험(STS-PROM(Society of Thoracic Surgeons-Predicted Risk of Mortality) 점수 >8) 또는 중간(STS-PROM 점수 >4) 위험으로 간주합니다.
- 심장 팀은 참여에 대한 환자의 적격성에 동의하고 경대퇴 접근에 의한 TAVR(Transcatheter aortic valve replacement)(TAVR)이 가장 적절한 치료 양식을 구성하며 환자가 가장 많은 혜택을 받을 가능성이 있음에 동의합니다.
- ECG-gated multislice 컴퓨터 단층 촬영 측정을 기준으로 적합한 대동맥륜 치수(면적 범위: 338-573 mm2 AND 둘레 범위: 66-85 mm). 가능한 경우 경식도 심초음파(TEE) 및 기존 대동맥조영술의 소견을 해부학적 평가에 통합해야 합니다.
- 다중 절편 컴퓨터 단층 촬영 혈관조영술 및/또는 기존 혈관조영술로 평가할 때 최소 접근 혈관 직경 ≥ 6mm로 경대퇴 접근에 적합한 동맥 대동맥-장골-대퇴 축
- 환자 또는 법정대리인의 서면 동의서
- 환자는 시험의 목적, 잠재적 위험 및 이점을 이해하고 후속 조치의 모든 부분에 기꺼이 참여합니다.
제외 기준:
다음 조건 중 하나가 존재하는 경우 대상이 포함되지 않습니다.
- 비 판막 대동맥 협착증
- 선천성 대동맥 협착증 또는 단첨판 또는 이첨판 대동맥 판막
- 비석회성 후천성 대동맥 협착증
- 대동맥 고리의 크기 또는 자연 대동맥 판막의 석회화 정도 또는 이심률 또는 대동맥 또는 대퇴 장골 동맥의 비틀림으로 인해 대퇴 경피 대동맥 판막 이식에 적합하지 않은 해부학
- 심인성 쇼크(낮은 심박출량, 승압제 의존성, 기계적 혈류역학 지원) 환자를 포함한 응급 처치
- 심각하게 감소된 좌심실(LV) 기능(박출률 < 20%)
- 대동맥 위치의 기존 인공 심장 판막
- 승모판 인공 삽입물의 존재
- 경피적 관상동맥 중재술(PCI)을 제외한 동시 계획 절차
- 30일 이내에 계획된 비심장 수술
- 시술 후 30일 이내의 뇌졸중.
- 시술 후 30일 이내의 심근경색(제2형 제외)
- 심장 내 종괴, 혈전 또는 식생의 증거
- 심한 응고 상태
- 항응고/항혈소판 요법을 견딜 수 없음
- 활동성 세균성 심내막염 또는 기타 활동성 감염
- 비대성 심근병증
- 조영제에 대한 금기 또는 니티놀에 대한 알레르기
- 이 프로토콜에서 개입 또는 이식 후 관리의 준비 또는 수행에 편차를 초래할 수 있는 다른 시험에 참여
