統合失調症の陰性症状は陽性症状よりも現在および将来の機能の強力な指標であり、薬物療法や既存の心理的介入に対する反応が乏しいため、統合失調症の陰性症状を対象とした介入を開発することには明確な理論的根拠があります。 これは、持続的な陰性症状は統合失調症患者にとって満たされていない治療ニーズを表していることを強調したNIMH-MATRICSのコンセンサスステートメントに反映されています。
この研究の目的は、Swanson らが開発した介入を科学的な方法で評価することです。 2021年:メタ認知トレーニング(MCT)マイナス。 MCT は、精神障害における陰性症状(例: 統合失調症、統合失調感情性または非感情性機能性精神病)の介入のオリジナルバージョンとしては、陽性症状のみに焦点を当てていました。 具体的な目的は、陰性症状、敗北主義的態度の重症度、内面化された偏見、うつ病の軽減、ならびに反射能力と全体的な機能の改善の観点から、MCTマイナスが対象集団にとって有望な治療法であるかどうかを研究することです。

臨床医は統合失調症の主な治療目標として陽性症状に焦点を当てる傾向がありますが、患者はうつ病と陰性症状の治療を優先します。 持続的な陰性症状は、統合失調症と診断された人の約 20% ~ 40% が経験します。 現在の研究では、陰性症状は陽性症状、うつ病、認知機能障害、混乱とは独立していることが示されています。 しかし、薬物療法も既存の心理社会的介入も、陰性症状を軽減する効果があるとは証明されていません。 その結果、著者らは、陰性症状を直接標的とする心理的介入を支える治療メカニズムをより深く理解する必要性を強調した。
心理学的概念化によれば、陰性症状の発現は、場合によっては、有害な経験に対する反応として理解できることが示唆されています。 例えば、ベックらの認知モデルは、陰性症状は、個人が認知的感情体験を「シャットダウン」する対処戦略を採用するプロセスから現れることを示唆しています。 これにより、個人は短期的には圧倒的な状況や嫌悪感のある状況に対処できますが、長期的には否定的な経験への曝露やその影響を軽減するために、否定的な症状(社会的引きこもり、意欲のなさ、表現力の低下など)に依存することになります。 Griffiths and McLeod (2019) は、愛着の枠組みから、陰性症状は「自己安全への脅威から生じる感情的および社会的引きこもりを伴う反応」として見られる可能性があることを示唆しています。 陰性症状を認知および発達の枠組みの中で理解できれば、その治療のための理論に基づいた介入を開発できる可能性があります。
陰性症状の認知モデル:

メタ認知と精神病:
メタ認知は当初、自分の精神プロセスについて考え、監視する能力と呼ばれていました。 しかし、現代の研究では、その定義は「特定の思考や感情に気づくことを含む離散的なプロセスから、情報が自己と他者の複雑な表現に統合されるより総合的な行為」にまで広がっています。 これにより、メタ認知プロセスを対象とした介入が生まれ、その中にはメタ認知トレーニング(MCT)など、精神病のために特別に開発されたものもある。メタ認知的反省と洞察療法 (MERIT) は、この集団向けに修正されたものもあります。 MCT は、認知バイアスが精神病症状の発症と維持に関与しており、根底にある認知プロセスを標的にすることで症状を軽減できるという前提に基づいています。 目的は、洞察を得て、苦痛な症状を管理するための実践的な戦略を学ぶことです。 MCT は妄想と陽性症状を軽減し、認知的洞察と偏見を改善することが示されています。 予備的な証拠は、生活の質と病気の洞察への影響も示唆しています。

実験的:陰性症状に対するメタ認知トレーニング
当初の MCT 介入は、陰性症状の発症および/または維持に関与すると認知モデル (Beck et al., 2009) によって示唆された認知を標的とする心理教育と戦略を組み込むことによって、陰性症状に適応されました。
一部の戦略は伝統的に陽性症状をターゲットにするために使用されてきましたが、同じ推論スタイルが、以前に議論した機能不全の認知を通じて陰性症状を引き起こすと想定されています(例:社会的拒絶に関する結論に飛びつき、社会的拒絶を強化する機能不全の帰属スタイル)撤退)。
陰性症状に対するメタ認知トレーニングは 8 つのセッションで構成され、このアプローチはグループ形式での提供よりも強力な効果量につながる可能性があることを示す証拠があるため、個別に提供されます (Liu et al., 2018)。
