2차 목표:

포함 기준:

  • 환자는 조직학적으로 입증된 T1-2N0M0 침습성 항문관 또는 항문연 편평 세포 암종을 가지고 있어야 하며 무작위화 전 4주 이내에 종양 측정치가 =< 4cm여야 합니다. 이것은 basaloid, transitional cell 또는 cloacogenic histology와 같은 non-keratinizing histology의 종양을 포함할 수 있습니다. 음성 마진으로 외과적 절제를 받은 T1N0M0 항문 가장자리 편평 세포 암종 환자는 자격이 없습니다.

  • 인간 면역결핍 바이러스(HIV) 음성인 환자는 컴퓨터 단층 촬영(CT) 및 양전자 방출 단층 촬영(PET)/CT 기반 기준을 사용하여 암 침범에 대해 방사선학적으로 우려되는 림프절이 없어야 합니다. 측정 가능한 질병은 필요하지 않습니다.

    • HIV 음성이고 림프절 양성으로 분류된 림프절이 없지만 암 침범의 경계선에 있는 것으로 느껴지는 환자는 중앙 영상 검토를 받아야 합니다.

      • 참고: 중앙 영상 검토가 필요한 환자는 Arm S에 사전 등록됩니다. 림프절 침범이 없음이 중앙에서 확인되면 적격 환자는 1단계에서 무작위 배정을 진행할 수 있습니다.

    • 환자는 림프절(LN) 양성인 것으로 간주되므로 림프절이 다음 기준 중 하나를 충족하는 경우 본 연구에 적합하지 않습니다.

      • Mesorectal, presacral, internal iliac 또는 obturator LN with:

        • CT/자기공명영상(MRI) 기준 단축 측정 > 5 mm 또는
        • MRI 및 LN 측정 > 3mm 또는
        • Fludeoxyglucose F-18(FDG) 흡수 > PET/CT 기반 혈액 풀(Deauville 3-5)

      • 외부 장골 및 일반 장골:

        • 단축 측정> CT / MRI 기준 1cm 또는
        • CT/MRI에 근거한 불규칙한 경계 또는 중앙 괴사의 형태적 특징 또는
        • FDG 흡수 > PET/CT 기반 혈액 풀(Deauville 3-5)

    • 다음 기준 중 하나를 충족하는 사타구니 LN(표피 및 심부)은 FNA가 수행되고 결과 세포학이 음성이 아닌 한 부적격합니다.

      • CT/MRI에 근거한 불규칙한 경계 또는 중앙 괴사의 형태적 특징
