내시경 초보자를 위한 시뮬레이터 기반 교육 과정이 얼마나 효과적일 수 있습니까? (EFFEKTRA)

상세 설명
수술과 내시경을 포함한 중재적 의학 훈련은 일반적으로 어느 정도 자격을 갖춘 감독 하에 환자에게 실시됩니다. 모델이나 시뮬레이터에 대한 이전 연습은 여전히 예외이며 정의된 커리큘럼과 지침이 있는 국가의 동료 중 약 40%만 도달할 수 있습니다. 그러나 시뮬레이터 기반 교육은 환자에게 위험을 주지 않고 초기 학습 성공이 있는 보호 영역을 제공합니다. 유연한 내시경에서 "시행 착오" 원칙에 따라 달성할 수 있습니다. 다양한 연구에서 시뮬레이터에 대한 초기 교육이 유익한 것으로 나타났습니다. 시뮬레이터에 대한 교육을 받은 의사는 환자에 대한 첫 번째 검사 중에 지원이 덜 필요하고 검사 시간이 더 짧으며 해부학적 랜드마크에 더 잘 도달하고 식별할 수 있으며 손과 눈의 협응력이 더 좋습니다. 시뮬레이터 교육은 구조화된 커리큘럼에 포함되어 있으며 구조화되지 않은 교육에 비해 더 큰 성공을 거둔 것으로 보입니다. 교육 중 피드백도 긍정적인 영향을 미치는 요소입니다. 이것은 이상적으로는 각 단원의 끝에서 이루어져야 합니다. 시뮬레이터를 사용하여 정의된 부분 수행을 훈련하고 이를 필요한 양만큼 반복하는 것도 가능합니다. 숙달 학습이라는 의미에서 정의된 중간 목표가 있는 단계별 구조도 효과를 높입니다. 시뮬레이터 교육에서 학습 곡선의 최적 기간 또는 포화도에 대한 데이터는 거의 없습니다. 한 연구에서 학습 곡선은 60번의 모의 대장내시경 후에 평평해졌습니다. 또 다른 연구에서는 시뮬레이터에서 최대 6시간까지 점진적인 개선을 보여주었습니다. 또한 성능 점수에 대한 임계값을 사용하여 환자 기반 교육으로 전환하기 위한 최적의 시점을 정의했습니다.
독일 소화기 학회(DGVS)의 전국 내시경 교육 과정 표준화에 대한 현재 인증은 1시간의 소개를 포함하여 시뮬레이터에 대한 4시간의 교육을 제공합니다. 또한 권장되는 최대 그룹 크기는 시뮬레이터당 참가자 4명으로 이는 교육생당 효과적인 시뮬레이터 교육 시간이 1시간에 불과합니다. 시뮬레이터 교육 구성 또는 사용되는 시뮬레이터 유형에 대한 권장 사항은 없습니다.
본 연구의 주요 목표는 비현실적으로 교육 및 과정 시간을 연장하지 않고 이러한 상황을 개선할 수 있는지 알아보는 것입니다.
