ACHTERGROND:
In de zomer van 1981 waarschuwden de Centers for Disease Control (CDC) de medische gemeenschap voor een onverwachte uitbraak van Pneumocystis carinii-pneumonie en Kaposi-sarcoom bij jonge homoseksuele mannen, personen waarvan niet bekend is dat ze risico lopen op deze zeldzame ziekten. Studies van immunologische functies bij deze patiënten toonden lymfopenie, cutane anergie, verminderde subpopulaties van helper-T-lymfocyten (T4), verlaagde verhoudingen van helper-T-lymfocyten tot suppressor-T-lymfocyten (T4:T8), abnormale lymfocytenresponsen op mitogeenstimulatie, verminderde natuurlijke killer-celactiviteit en vaak hypergammaglobulinemie. De naam verworven immunodeficiëntiesyndroom (AIDS) werd gegeven aan deze stoornis van immunologische regulatie met opportunistische infecties of Kaposi-sarcoom of beide.
Sinds die tijd is AIDS gemeld bij patiënten met hemofilie en bij anderen die bloedtransfusies kregen. De NHLBI kreeg de verantwoordelijkheid voor studies om door middel van een systematische prospectieve studie het mogelijke verband tussen het gebruik van bloedproducten en aids op te helderen. De incidentie van door transfusie opgelopen aids was zo laag dat een algemene prospectieve studie van ontvangers van transfusies om de algehele incidentie van de ziekte te bepalen, onmogelijk was. Door zich te richten op patiëntengroepen die een grotere kans leken te hebben op het ontwikkelen van aids omdat zij chronische, herhaalde transfusietherapie nodig hadden, kon sneller informatie over de epidemiologie van de aandoening worden verkregen. Voortdurende medische evaluatie van deze patiënten uit ogenschijnlijk risicovolle groepen op een prospectieve manier maakte het mogelijk significante opkomende fysiologische veranderingen te identificeren en het tijdsverloop van pathologische ontwikkelingen te volgen.
Het doel van deze studie was om een duidelijk beeld te krijgen van welke immunologische veranderingen optraden bij patiënten die een transfusie kregen en om te bepalen wanneer deze veranderingen optraden en hoe lang ze aanhielden. Aangezien het grootste deel van de patiëntenpopulatie zware transfusies kreeg, ontwikkelden sommige van de proefpersonen uiteindelijk AIDS. De informatie die tijdens het onderzoek over deze patiënten werd verkregen, werd vervolgens gebruikt om vragen te beantwoorden als: welke factoren bepaalden het risico van een bepaalde persoon om aids te krijgen en in welke mate? Welke veranderingen vonden plaats vóór de klinische manifestaties van AIDS? Was de progressie van normale naar defecte celgemedieerde immuniteit naar AIDS strikt unidirectioneel of werd de veranderde celgemedieerde immuniteit in het studiecohort ooit weer normaal? Als progressie niet unidirectioneel was, wat bepaalde dan de uitkomst? Ook kon het belang van het opzetten van een serum- en cellenrepository niet genoeg worden benadrukt, aangezien deze specimens, naarmate nieuwe wetenschappelijke informatie beschikbaar kwam, de uitvoering van een aantal verschillende retrospectieve onderzoeken mogelijk maakten.
In 1984 werd een serumopslagplaats voor hiv-testen opgericht. De taak van de repository was het verzamelen en opslaan van serummonsters van bloeddonoren in vier bloedtransfusie-eenheden in vier belangrijke risicogebieden voor aids, namelijk New York, San Francisco, Miami en Los Angeles. Bloed werd verkregen van het New York Blood Center in New York City, de Irwin Memorial Blood Bank in San Francisco, het Rode Kruis in Los Angeles en het Amerikaanse Rode Kruis, South Florida Region, in Miami. Het doel van de repository was om het resultaat te bepalen van de transfusie van bloedproducten waarvan achteraf bleek dat ze hiv-infectie van de donor vertoonden. De verzamelde sera werden gescheiden, geïndexeerd, ingevroren en opgeslagen.
In 1984 werd het tweede deel van de Transfusion Safety Study, de prospectieve studie, geïnitieerd door NHLBI op grond van een afzonderlijk verzoek tot voorstel. Het testen van de 201.000 serumspecimens in de donoropslagplaats voor antilichaam tegen HIV werd uitgevoerd door laboratoria in de prospectieve studie. Die donoren waarvan werd bevestigd dat ze anti-hiv-positief waren en overeenkomende donoren waarvan was bevestigd dat ze anti-hiv-negatief waren, werden gerekruteerd als TSS-proefpersonen. Ontvangers van bloed van de positieve donoren en de negatieve donoren werden ook gerekruteerd als proefpersonen.
In september 1984 begon de TSS met een planningsfase van negen maanden waarin de protocollen en datasystemen voor de prospectieve studie werden ontwikkeld. Onderwerpen werden ingevoerd in juli 1985 tot en met juni 1989. De analyse ging door tot november 1997. Er waren klinische centra in Los Angeles, San Francisco, New York City en Miami. Het coördinatiecentrum bevond zich aan de Universiteit van Zuid-Californië. De centrale laboratoria, waaronder het immunologische standaardisatielaboratorium, het immunologische reagentialaboratorium en het centrale verwerkingslaboratorium, bevonden zich in Los Angeles, met een van de immunologielaboratoria in Seattle, Washington in het Puget Sound Blood Center.
ONTWERP VERHAAL:
Bij aanvang onderging elke proefpersoon een lichamelijk onderzoek, werd ondervraagd over de medische geschiedenis en werden bloedmonsters afgenomen voor immunologisch en ander onderzoek. Elke proefpersoon keerde met een bepaald interval terug voor een herhaling van onderzoeken die in het protocol zijn gespecificeerd. Bloedonderzoek omvatte volledige bloedceltelling, karakterisering van immunologische cellen zoals T- en B-lymfocyten en natural killer-cellen. Plasma- en Buffy-coat-celsuspensies werden ingevroren bewaard. De niveaus van alanine-aminotransferase werden bepaald. Serologische tests werden uitgevoerd voor hepatitis, Epstein-Barr-virus en cytomegalovirus. Markers van de immuunstatus, waaronder de niveaus van immunoglobuline, werden gemeten. De studie eindigde in november 1997.
