De ziekte van Huntington (HD) is een langzaam progressieve aandoening die het leven van de getroffenen en hun families verwoest. Er zijn geen behandelingen die de progressie van de ZvH vertragen, er zijn alleen mild effectieve symptomatische therapieën beschikbaar.
Het doel van deze studie is om erachter te komen of co-enzym Q10 (CoQ) effectief is in het vertragen van de verergerende symptomen van de ZvH. In deze studie zullen onderzoekers ook meer te weten komen over de veiligheid en aanvaardbaarheid van langdurig CoQ-gebruik door de effecten ervan op mensen met de ZvH te bepalen.
Deelnemers aan deze proef worden willekeurig gekozen in een van de twee groepen. Groep 1 krijgt CoQ (2400 mg/dag) en groep 2 krijgt een placebo (een inactieve stof). Onderzoekers zullen de verandering in totale functionele capaciteit (TFC) - een maatstaf voor functionele beperkingen - in de twee groepen vergelijken. De TFC is een geldige en betrouwbare maatstaf voor ziekteprogressie en reageert met name op veranderingen in de vroege en middenfasen van de ZvH. Onderzoekers zullen ook de veranderingen in andere componenten van de Unified Huntington's Disease Rating Scale '99 (UHDRS) vergelijken, waaronder: de totale motorscore, totale gedragsfrequentiescore, totale gedragsfrequentie X ernstscore, verbale fluency-test, symbol digit modalities-test, Stroop , interferentietest, functionele checklist en onafhankelijkheidsschaalscores. De groepen zullen ook worden vergeleken met betrekking tot verdraagbaarheid, bijwerkingen, vitale functies en laboratoriumtestresultaten als veiligheidsmaatstaven.

Beschrijving
Inclusiecriteria:
Om in aanmerking te komen voor deelname aan dit onderzoek, moeten proefpersonen binnen 28 dagen voorafgaand aan randomisatie aan de volgende geschiktheidscriteria voldoen:
- Proefpersonen moeten klinische kenmerken van de ZvH hebben en een bevestigde familiegeschiedenis van de ZvH, OF een CAG-herhalingsexpansie ≥ 36.
- TFC > 9.
- Moet ambulant zijn en geen bekwame verpleegkundige zorg vereisen.
- Leeftijd ≥ 16 jaar.
- Vrouwen mogen niet in staat zijn om zwanger te worden (bijvoorbeeld na de menopauze, chirurgisch steriel of door gebruik te maken van adequate anticonceptiemethoden voor de duur van het onderzoek).
- Als er psychotrope medicatie wordt ingenomen (bijv. anxiolytica, hypnotica, benzodiazepinen, antidepressiva), moeten deze gedurende vier weken voorafgaand aan randomisatie in een stabiele dosering zijn en gedurende het hele onderzoek op een constante dosering worden gehouden. (Opmerking: stabiele dosering van tetrabenazine is toegestaan.) Alle wijzigingen aan deze medicijnen die worden opgelegd door klinische omstandigheden zullen systematisch worden geregistreerd en de proefpersoon mag in het onderzoek blijven.
- In staat om geïnformeerde toestemming te geven en te voldoen aan procesprocedures
- In staat om orale medicatie in te nemen.
- Kan nodig zijn om een informant of verzorger te identificeren die bereid en in staat zal zijn om toezicht te houden op de dagelijkse dosering van onderzoeksmedicatie en om de controle over de studiemedicatie thuis te behouden.
- Een aangewezen persoon zal door de proefpersoon worden geïdentificeerd om deel te nemen aan het lopende toestemmingsproces als het cognitieve vermogen van de proefpersoon om toestemming te geven in het gedrang komt tijdens deelname aan het onderzoek.
Uitsluitingscriteria:
- Geschiedenis of bekende gevoeligheid van onverdraagzaamheid voor CoQ.
- Blootstelling aan een onderzoeksgeneesmiddel binnen 30 dagen na het baselinebezoek.
- Klinisch bewijs van onstabiele medische ziekte naar het oordeel van de onderzoeker.
- Instabiele psychiatrische aandoening gedefinieerd als psychose (hallucinaties of wanen), onbehandelde ernstige depressie of zelfmoordgedachten binnen 90 dagen na het baselinebezoek.
- Drugsmisbruik (alcohol of drugs) binnen een jaar na het baselinebezoek.
- Vrouwen die zwanger zijn of borstvoeding geven.
- Gebruik van aanvullend co-enzym Q10 binnen 30 dagen voorafgaand aan het baselinebezoek
- Klinisch ernstige afwijkingen in de screeningslaboratoriumonderzoeken (screeningcreatinine hoger dan 2,0, alanineaminotransferase (ALAT) of totaal bilirubine hoger dan 3 keer de bovengrens van normaal, absoluut aantal neutrofielen van ≤1000/ul, bloedplaatjesconcentratie van
- Bekende allergie voor FD&C geel #5 of enig ander ingrediënt in het onderzoeksgeneesmiddel (actief en placebo)

De primaire uitkomstvariabele aan het begin van het onderzoek was de verandering in TFC-score vanaf baseline tot maand 60.
De Data and Safety Monitoring Board adviseerde de onderzoeksleiding om te heroverwegen hoe ze ontbrekende gegevens van proefpersonen die overlijden in hun primaire analyse van de verandering in TFC-score kunnen verwerken.
Op basis van deze aanbevelingen veranderde de leiding van de proef de primaire analyse in die van een gezamenlijke rangordebenadering.
