Associatie tussen vitamine D-receptorpolymorfisme en vitamine D-spiegels in serum bij kinderen met lage-energiefracturen

De onderzoeksgroep bestond uit 100 kinderen van 3 tot 18 jaar (78% jongens) die in 2011-2013 waren opgenomen op de afdeling Kinderorthopedie vanwege laagenergetische fracturen. De controlegroep (122 kinderen, 68% jongens) bestond uit kinderen van 3 tot 17 jaar die om andere redenen (blessures, diagnose van kniebandletsels en andere) in het ziekenhuis waren opgenomen zonder fracturen. Kinderen met osteogenesis imperfecta (OI) werden uitgesloten van het onderzoek. Van elke patiënt en hun ouder/voogd werd een schriftelijke geïnformeerde toestemming voor deelname aan het onderzoek verkregen. Een laagenergetische fractuur werd gedefinieerd als een fractuur opgelopen door een val van de eigen lengte van de patiënt of een val tijdens teamwedstrijden. Alle breuken werden radiologisch bevestigd.
Tijdens de ziekenhuisopname vulden de patiënten en hun ouders een vragenlijst in over demografische gegevens, de woonplaats (stedelijk/landelijk), eerdere fracturen en fysieke activiteit. De mate van geslachtsrijpheid werd beoordeeld met behulp van een onderzoek naar de puberteitsontwikkeling op basis van de Puberty Development Scale [20-22]. Het lichaamsgewicht en de lengte van de patiënten werden gemeten en de Cole-indicator werd berekend om de voedingsstatus van de patiënten te beoordelen.
Serumconcentraties van totaal vitamine D [25-hydroxyvitamine D3 ((25(OH)D3)) plus 25-hydroxyvitamine D2 ((25(OH)D2))] in het plasma van alle kinderen werden bepaald door elektrochemiluminescentie met behulp van paramagnetische deeltjes bekleed met streptavidine en rutheniumverbinding op het Cobas e 411-apparaat van Roche. Volgens ons laboratorium was het referentiewaardebereik voor totaal vitamine D 30,0 - 74,0 ng/ml. Volbloedmonsters werden verzameld in buisjes met EDTA en bewaard bij -20°C. DNA werd geïsoleerd met behulp van de MasterPureTM DNA Purification Kit (Akor Laboratories) en gekwantificeerd op een spectrofotometer (Nanodrop 2000, Thermo Scientific). De genotypen voor vier restrictiefragmentlengtepolymorfismen van het vitamine D-receptor (VDR)-gen werden bepaald door middel van standaard polymerasekettingreactie (PCR)-technieken en enzymatische digestie van de producten met FokI, ApaI, TaqI en BsmI (Thermo Scientific). Kortom, PCR werd uitgevoerd in een eindvolume van 20 µl met daarin 50-100 ng DNA, 0,3 µM van elke primer en JumpStartTM REDTaqTM ReadyMixTM (Sigma). Na initiële denaturatie gedurende 3 minuten bij 94 °C werden monsters onderworpen aan 35 amplificatiecycli, bestaande uit een denatureringsfase van 30 seconden bij 94 °C, een annealingsfase van 30 seconden (FokI bij 60 °C, ApaI en TaqI bij 70 °C, BsmI bij 62 °C), een verlengingsfase van 30 seconden bij 72 °C en 4 minuten bij 72 °C op een Bio-Rad thermal cycler CFX96TM. De voor FokI-polymorfisme gebruikte primers waren: voorwaartse 5'-AGC TGG CCC TGG CAC TGA CTC TGC TCT-3', omgekeerde 5'-ATG GAA ACA CCT TGC TTC TTC TCC CTC-3'; ApaI en TaqI: vooruit 5'-CAG AGC ATG GAC AGG GAG CAA-3', achteruit 5'-GCA ACT CCT CAT GGC TGA GGT CTC-3' en BsmI: vooruit 5'-AGT GTG CAG GCG ATT CGT AG-3 ', omgekeerde 5'-ATA GGC AGA ACC ATC TCT CAG-3' [25]. De PCR-producten werden gedigereerd volgens de instructies van de fabrikant en gescheiden op 2% agarosegel. De polymorfismen werden gedocumenteerd door te fotograferen onder UV-verlichting met behulp van G:Box (Syngene). Een willekeurige subset (20% van de monsters) werd herhaald om de resultaten te verifiëren. Hoofdletters "F", "A", "T" en "B" geven de afwezigheid aan van de knipplaats voor respectievelijk FokI-, ApaI-, TaqI- en BsmI-polymorfismen, terwijl kleine letters "f", "a", " t", en "b" geven de aanwezigheid ervan aan. Om de relaties tussen kwaliteitsattributen te onderzoeken, werden Chi-kwadraattest van onafhankelijkheid en Fisher's exact-test gebruikt. De normaliteit van de verdeling werd geverifieerd met behulp van de Kolmogorov-Smirnov-test met de Lilliefors-significantiecorrectie en de Shapiro-Wilk-test. Er was geen normale verdeling van de geanalyseerde kwantitatieve variabelen. Om de kwantitatieve variabelen zonder normale verdeling te vergelijken, werd een niet-parametrische Mann-Whitney U-test gebruikt. Modellen van univariate en multivariate lineaire regressie en logistische regressie werden opgesteld. De resultaten werden als statistisch significant beschouwd bij p <0,05. De berekeningen zijn uitgevoerd met behulp van Statistica 10.0 van StatSoft, IBM SPSS Statistics 21.0 van Predictive Solutions Company en Stata / IC 13.1-pakketten van StataCorp LP
