Genetische gevoeligheid om gewichtsverlies te voorspellen na bariatrische chirurgie (GENBASU)
Obesitas is een complexe chronische ziekte, waarbij zowel genetische als omgevingsfactoren een rol spelen, die een grote heterogeniteit vertoont in de reactie op verschillende gewichtsverliesprogramma’s. Het identificeren van patiënten als wel of niet reagerend op de verschillende behandelingsopties voor obesitas is een concept van groot belang, zowel vanwege de hoge prevalentie van obesitas als vanwege het hoge verbruik van hulpbronnen. Jaarlijks worden wereldwijd ruim 500.000 operaties uitgevoerd, waarvan ongeveer 30% onbevredigende resultaten zal opleveren.
Het algemene doel is het uitvoeren van een multi-omics-aanpak voor de ontdekking en validatie van markers van gewichtsrespons op bariatrische chirurgie (BS) bij een grote steekproef van mensen met ernstige obesitas (n=6.966 mannen en vrouwen die een sleeve gastrectomie of een maagoperatie ondergingen). bypass, inclusief een extra externe validatieset). We willen dus de geïntegreerde bijdrage kennen van verschillende genomische markers (Genome Wide Association study, GWA's), nieuwe klinische en analytische variabelen (menselijk exposoomconcept) en genderperspectief aan de voorspelling van de respons op de interventie na 12 maanden en de lange termijn ervan. termijn longitudinaal onderhoud (3 jaar). We zijn daarom van plan nieuw bewijs te leveren om vooruitgang te boeken in de richting van precisiegeneeskunde. We zullen onze aandacht ook richten op het identificeren van die patiënten die, nadat ze als responders op het dieptepunt van het gewichtsverlies waren geclassificeerd, gewichtstoename ervoeren.

Inclusiecriteria:
- Mannen en vrouwen tussen de 18 en 65 jaar op het moment van de operatie
- SG of RYBP als bariatrische procedure.
- Met een BMI≥35,0 kg/m2 met één of meerdere comorbiditeiten of een BMI>40,0 kg/m2 op het moment van de operatie.
- Verstrekking van een door de patiënt ondertekend en persoonlijk gedateerd toestemmingsformulier.
- Met beschikbare basisgegevens (antropometrische, analytische, chirurgische procedure, farmacologische therapieën en aan obesitas gerelateerde comorbiditeiten).
Uitsluitingscriteria:
- Elke eerdere gastro-intestinale operatie
- Behandeling met een goedgekeurd geneesmiddel tegen obesitas in Spanje (orlistat, liraglutide 3,0 mg en/of naltrexon/bupropion met verlengde afgifte) of systemische glucocorticoïden gedurende meer dan 4 opeenvolgende weken gedurende de 3 jaar na BS
- Nierziekte in het eindstadium (eGFR <15 ml/min/1,73m2) of levercirrose bij baseline
- Patiënten die een ernstige medische aandoening hebben ontwikkeld die de praktijk van een gezond leven binnen normale grenzen beperkt (gevorderde hart- en vaatziekten of hartfalen, beroerte met neurologische gevolgen, chronische longobstructie met kortademigheid bij minimale inspanning, ernstige gewrichtspathologie, nierfalen in het eindstadium, of actieve kanker)
- Bekende diabetes type 1 of LADA-diabetes
- Vrouwen die zwanger worden tijdens de follow-upperiode
- Huidig drugs- of alcoholmisbruik
- Tijdens de follow-upperiode ontwikkelden zich ongecontroleerde psychiatrische ziekten of eetstoornissen.
